  • Reflection photoelasticity: a new method for studies of clinical mechanics in prosthetic dentistry

    • Objectives. This research was conducted to demonstrate the effectiveness of reflective photoelasticity as an in vivo technique for monitoring the strain/stress distribution in prosthetic devices during function. Methods. Seven subjects were selected for this study and divided into three groups according to the design of the prostheses. The buccal surfaces of the experimental prostheses were coated with a birefringent resin 2 mm thick. Using a reflection polariscope, fringe patterns were documented on video tape during standardized in vivo loadings. Strain and stress levels (gamma(max) and tau(max)) were calculated from isochromatic fringe order determinations. The intra-observer variability in counting fringe orders and the accuracy level of reflective photoelasticity as compared to strain gauge results were also studied. The strain results of in vivo loading of the prostheses were analyzed with ANOVA and Scheffes tests. The intra-observer variability was analyzed with the Friedman nonparametric test. Comparisons of photoelastic data and strain gauges were tested with Pearson's correlation. Results. Coated areas accessible to normally incident light, produced comprehensive maps of strain distribution. Significant differences were found in the in vivo gamma(max) values for the prostheses designs (p < 0.05). The test for intra-observer variability showed that no significant differences occurred in counting fringe orders. Strain values obtained with reflective photoelasticity showed a high correlation r = 0.98 and 0.99) with values obtained at the same areas using strain gauges. Significance. Reflective photoelasticity is a valid, reliable and accurate technique to be used for in vivo studies on the biomechanical behavior of prosthetic devices. (C) 2003 Academy of Dental Materials.     • The incidence of pancreatic cancer has fallen during the last ten years in Sweden. Early signs and symptoms of the disease are still undiscovered and when diagnosis is made the disease is incurable in most patients. Transabdominal ultrasonography is the first-line imaging test followed by spiral computed tomography (CT) and magnetic resonance imaging (MRI) if required for definite diagnosis. Spiral CT is also the imaging test of choice for assessment of resectability of the tumor. Surgical removal of the tumor is the only chance of cure. Markedly improved hospital mortality after pancreaticoduodenectomy is reported and an association between hospital volume and outcome of the operation has been established. Longterm survival after attempted curative resection continues to be dismal, however. Adjuvant treatment should not be given outside clinical studies. Palliative treatment has improved thanks to progress in the field of endoscopy, interventional radiology and in management of pain and nutrition. Palliative chemotherapy should only be given selectively outside clinical studies. Radiotherapy has no proven effects on survival. Special pancreatic cancer treatment teams with catchment areas of 2-4 million inhabitants are recommended by international authorities.

  • Food-borne outbreak of gastroenteritis associated with genogroup I calicivirus.

    • An outbreak of gastroenteritis affecting 158 of 219 (72%) guests and employees at a hotel is described. Food served at the hotel restaurant is believed to have been the source of the outbreak and to have been contaminated by sick employees working in the restaurant. A secondary attack rate of 22% was seen involving 43 persons in all. In stool specimens from seven of eight patients, Norwalk-like viruses (NLVs) were detected by electron microscopy. While NLV-specific PCR using primers JV12 and JV13 were negative, all specimens examined with primers NVp69 and NVp110 were positive. The failure of primers JV12 and JV13 was attributed to several mismatches in the JV12 primer. Genotyping and sequence analysis revealed that all samples had identical sequences and clustered with genogroup I, and the most closely related well-characterized genotype is Desert Shield. This is the first described food-borne outbreak associated with genogroup I virus in Sweden.

  • Swedish-Lithuanian telemedicine Litmed project in otolaryngology

    • Telemedicine is becoming a reality as a result of improvements in technology and telecommunications. The task of the otolaryngological part of the pilot international telemedicine Litmed project was devoted to the establishment of telemedicine training and demonstration facilities in cooperation between the Departments of Otolaryngology of the Kaunas University of Medicine (Lithuania) and the Lund University (Sweden). The main areas of action of the Litmed project in otolaryngology were: (1) remote rehabilitation of cochlear implant (CI) patients, and (2) phonosurgery and phoniatrics. The main results of the project were as follows: (a) establishment of telemedicine environment for remote on-line collaboration and planned off-line collaboration by use of recorded video laryngostroboscopic images, voice samples, and graphical and computed tomography (CT) data; (b) possibility to carry through telemedicine sessions for pedagogical and speech training support and for cooperative work of speech therapists from Lund and Kaunas with CI patients; (c) remote consultations and discussion of challenging laryngeal cases; and (d) establishment of a technical environment and practical routines for on-line consultations during laryngeal surgery at the Departments of Otolaryngology of Kaunas and Lund. The Litmed project served to assist medical education and research. Tele-otolaryngology helps to keep constant professional contacts with the specialists from abroad and supports an establishment of a center of reference in tele-otolaryngology in Lithuania.
