    • OBJECTIVE: Primary hyperparathyroidism (PHPT) is associated with heart disease. The aims of the present study were to evaluate how cardiac function and secretion of N-terminal pro-B-type natriuretic peptide (NT-proBNP) correlate in patients with mild PHPT, and how the plasma level of NT-proBNP is influenced by cure of the parathyroid disease. DESIGN AND PATIENTS: Forty-two patients with PHPT without symptoms of heart disease were examined before and 1 year after curative parathyroidectomy. MEASUREMENTS: Plasma or serum concentrations of NT-proBNP, calcium, PTH, creatinine, oestradiol, testosterone and SHBG were measured. Cardiac function was evaluated by equilibrium radionuclide angiography (ERNA). RESULTS: At baseline, NT-proBNP levels correlated negatively with systolic function [left ventricular ejection fraction (LVEF), P < 0.001]. Twelve per cent of the patients had NT-proBNP levels above normal reference values preoperatively. One year postoperatively, the corresponding proportion was 21%. The mean plasma concentration of NT-proBNP increased after parathyroidectomy (P < 0.01) in parallel with a dip in diastolic function (peak filling rate, P < 0.05) and a falling trend in systolic function (LVEF, P = 0.08). The postoperative percentage changes in circulating NT-proBNP and total oestradiol correlated positively (P < 0.05). CONCLUSIONS: Patients with mild PHPT and normal renal function may have high levels of circulating NT-proBNP despite the absence of symptomatic heart disease. Cure of the parathyroid disease is followed by a further increase in NT-proBNP secretion in parallel with ERNA measures, indicating subclinical changes in heart function. These results are in line with data indicating an association between PHPT and increased risk of premature death.

  • A catalog of genetic loci associated with kidney function from analyses of a million individuals

    • Chronic kidney disease (CKD) is responsible for a public health burden with multi-systemic complications. Through transancestry meta-analysis of genome-wide association studies of estimated glomerular filtration rate (eGFR) and independent replication (n = 1,046,070), we identified 264 associated loci (166 new). Of these,147 were likely to be relevant for kidney function on the basis of associations with the alternative kidney function marker blood urea nitrogen (n = 416,178). Pathway and enrichment analyses, including mouse models with renal phenotypes, support the kidney as the main target organ. A genetic risk score for lower eGFR was associated with clinically diagnosed CKD in 452,264 independent individuals. Colocalization analyses of associations with eGFR among 783,978 European-ancestry individuals and gene expression across 46 human tissues, including tubulo-interstitial and glomerular kidney compartments, identified 17 genes differentially expressed in kidney. Fine-mapping highlighted missense driver variants in 11 genes and kidney-specific regulatory variants. These results provide a comprehensive priority list of molecular targets for translational research.

  • Structure liming reduces draught requirement on clay soil

    • Liming with ‘structure lime’, comprising approximately 80–85% ground limestone and 15–20% slaked lime, has been promoted in subsidised environmental schemes in Sweden since 2010 to increase clay aggregate stability and mitigate particulate phosphorus losses to surface waters. To date, approximately 65,000 ha have been structure-limed. Apart from stabilising aggregates, liming may also improve other physical properties, such as soil strength. This study examined the effect of increasing application rate (0–16 t ha-1) of structure lime on soil strength, approximated by horizontal (draught requirement) and vertical (penetrometer resistance) measurements, in eight field soils (clay content 26–38%) to which structure lime had been applied two, three, four or six years previously. Draught requirement when cultivating with a multipurpose cultivator significantly decreased (by 11%) with the highest application rate of structure lime (16 t ha-1) compared with an unlimed control. This reduced the wheel power requirement by 7.1 kW and diesel consumption by 1.2–1.4 L ha-1, and lowered CO2 emissions by 3–4 kg ha-1. To clarify the general effect of structure liming, the mean value of all limed treatments was compared with that of the unlimed control. This showed that structure liming in general significantly reduced the draught requirement (by 7%). However, penetrometer resistance measurements revealed no significant effects of structure liming and no relation between draught requirement and penetrometer resistance measurements. Overall, the results indicate that structure liming can reduce fuel consumption, due to easier soil tillage, and thus lower CO2 emissions.

  • Area of ischemia assessed by physicians and software packages from myocardial perfusion scintigrams

    • Background: The European Society of Cardiology recommends that patients with greater than 10% area of ischemia should receive revascularization. We investigated inter-observer variability for the extent of ischemic defects reported by different physicians and by different software tools, and if inter-observer variability was reduced when the physicians were provided with a computerized suggestion of the defects. Methods: Twenty-five myocardial perfusion single photon emission computed tomography (SPECT) patients who were regarded as ischemic according to the final report were included. Eleven physicians in nuclear medicine delineated the extent of the ischemic defects. After at least two weeks, they delineated the defects again, and were this time provided a suggestion of the defect delineation by EXINI Heart(TM) (EXINI). Summed difference scores and ischemic extent values were obtained from four software programs. Results: The median extent values obtained from the 11 physicians varied between 8% and 34%, and between 9% and 16% for the software programs. For all 25 patients, mean extent obtained from EXINI was 17.0% (+/- standard deviation (SD) 14.6%). Mean extent for physicians was 22.6% (+/- 15.6%) for the first delineation and 19.1% (+/- 14.9%) for the evaluation where they were provided computerized suggestion. Intra-class correlation (ICC) increased from 0.56 (95% confidence interval (CI) 0.41-0.72) to 0.81 (95% CI 0.71-0.90) between the first and the second delineation, and SD between physicians were 7.8 (first) and 5.9 (second delineation). Conclusions: There was large variability in the estimated ischemic defect size obtained both from different physicians and from different software packages. When the physicians were provided with a suggested delineation, the inter-observer variability decreased significantly.

    • The aim of this myocardial perfusion imaging (MPI) study was to compare the diagnostic performance of two computer-aided diagnosis (CAD) systems, EXINI Heart(TM) (EXINI), and PERFEXTM (PERFEX) Emory Cardiac Toolbox (ECT), and the summed stress score (SSS) values from both software packages. We studied 1,052 consecutive patients who underwent 2-day stress/rest Tc-99m-sestamibi MPI studies. The reference standard classifications for the MPI studies were obtained from three experienced physicians who separately classified all cases regarding the presence or absence of ischemia and/or infarction. Automatic processing was carried out using EXINI and PERFEX to obtain CAD results and SSS values based on the 17-segment model. The three experts' classifications showed ischemia in 257 patients and abnormal studies, i.e., either ischemia or infarction or both, in 318 patients. Accuracy was significantly higher in EXINI than in PERFEX, regarding both the detection of ischemia (87.4 vs 77.6%; P < 0.0001) and the detection of abnormal studies (91.6 vs 67.9%; P < 0.0001). EXINI's CAD system showed a higher specificity than its SSS values (86.8 vs 73.6%; P < 0.0001) at the same level of sensitivity. EXINI demonstrated greater diagnostic accuracy for detection of ischemia and abnormal studies than did PERFEX. EXINI CAD also outperformed its SSS analysis.

  • Diagnostic evaluation of three cardiac software packages using a consecutive group of patients

    • Purpose: The aim of this study was to compare the diagnostic performance of the three software packages 4DMSPECT (4DM), Emory Cardiac Toolbox (ECTb), and Cedars Quantitative Perfusion SPECT (QPS) for quantification of myocardial perfusion scintigram (MPS) using a large group of consecutive patients. Methods: We studied 1,052 consecutive patients who underwent 2-day stress/rest 99mTc-sestamibi MPS studies. The reference/gold-standard classifications for the MPS studies were obtained from three physicians, with more than 25 years each of experience in nuclear cardiology, who re-evaluated all MPS images. Automatic processing was carried out using 4DM, ECTb, and QPS software packages. Total stress defect extent (TDE) and summed stress score (SSS) based on a 17-segment model were obtained from the software packages. Receiver-operating characteristic (ROC) analysis was performed. Results: A total of 734 patients were classified as normal and the remaining 318 were classified as having infarction and/or ischemia. The performance of the software packages calculated as the area under the SSS ROC curve were 0.87 for 4DM, 0.80 for QPS, and 0.76 for ECTb (QPS vs. ECTb p = 0.03; other differences p < 0.0001). The area under the TDE ROC curve were 0.87 for 4DM, 0.82 for QPS, and 0.76 for ECTb (QPS vs. ECTb p = 0.0005; other differences p < 0.0001). Conclusion: There are considerable differences in performance between the three software packages with 4DM showing the best performance and ECTb the worst. These differences in performance should be taken in consideration when software packages are used in clinical routine or in clinical studies.

    • There is a worldwide emphasis to increase the share of renewable transportation fuels. When using agricultural land for production of renewable transportation fuels, the energy output per hectare for different crops and transportation fuels is a crucial factor. In this study, the gross methane energy yield per hectare from anaerobic digestion of industrial hemp (Cannabis sativa L.), was determined at four different harvest times between July and October in Southern Sweden, a cold climate region. The biomass yield was determined for three years and the methane yield was determined for two years through the biochemical methane potential test. The highest biomass yield, 16 tonnes dry matter per hectare on an average, and the highest methane energy yield per hectare was achieved when the hemp was harvested in September or October, with an average gross methane energy yield of 136 +/- 24 GJ per hectare. There was no significant difference in the specific methane yield between the harvest times; the average being 234 +/- 35 m(3) per tonne volatile solids. Biogas from hemp turned out to be a high yielding alternative to the currently dominating renewable transportation fuels produced from crops grown in Sweden: ethanol from wheat and biodiesel from rapeseed.     • Vallar på lägre avkastande marginaljordar har en stor potential till hållbar odling av biomassa för biogasproduktion, samtidigt som landskapet kan bevaras öppet. Enligt officiell statistik från 2011 finns ca 180 000 ha åkermark i träda, men arealen som ligger ”outnyttjad” kan enligt andra bedömningar vara så stor som 800 000 ha. Detta tyder på att det i nuläget finns stora arealer tillgängliga för produktion av biogassubstrat, vilket kan bidra till att öka biodiversiteten, förse samhället med hållbara drivmedel och recirkulerad växtnäring till livsmedelproduktion. Målet med studien var att undersöka spridningen i resultat från metanpotentialbestämningar av vall på marginalmark för att undersöka möjligheten till minskning av analyskostnaderna genom en förbättrad provtagningsstrategi. Studien visade att antalet analyser för metanpotentialbestämning kan reduceras avsevärt när samlingsprover används istället för delprov.

  • Vall som biogassubstrat : utvärdering av skördesystemets och odlingsintensitetens påverkan på biogasutbytet

    • I ett fältförsök på marginalmark odlades och skördades olika vallblandningar som biogassubstrat. Syftet med studien var att (a) undersöka biomassa- och metanavkastningen för de olika vallblandningarna och (b) analysera spridningen av metanpotentialbestämningarnas resultat för att undersöka möjligheter till minskning av analyskostnaderna genom annan provtagnings- och provberedningsstrategi. En gödslad kommersiell vall gav med 8,1 ton ts/ha den högsta biomassaavkastningen vid två skördar per år. Vid bara en skörd per år var det den gödslade kommersiella vallen och den befintliga vegetationen som gav högst biomassaavkastning. Metanpotentialsskillnaderna mellan vallblandningarna var ganska små vid samma skördetidpunkt. Däremot ökar den specifika metanpotentialen (liter metan per kg VS) med två skördar per år. Beräkningarna visade att antalet analyser för metanpotentialbestämningen kan reduceras betydligt när samlingsprover används i analyserna. På detta sätt minskar man variationen inom leden, vilket bidrar till att skillnader mellan olika behandlingar enklare kan upptäckas.
