  • Translation and further validation of a global rating scale for the assessment of clinical competence in prehospital emergency care

    • Global rating scales are useful to assess clinical competence at a general level based on specific word dimensions. The aim of this study was to translate and culturally adapt the Paramedic Global Rating Scale, and to contribute validity evidence and instrument usefulness in training results and clinical competence assessments of students undergoing training to become ambulance nurses and paramedics at Swedish and Finnish universities. The study included translation, expert review and inter-rater reliability (IRR) tests. The scale was translated and culturally adapted to clinical and educational settings in both countries. A content validity index (CVI) was calculated using eight experts. IRR tests were performed with five registered nurses working as university lecturers, and with six clinicians working as ambulance nurses. They individually rated the same simulated ambulance assignment. Based on the ratings IRR was calculated with intra-class correlation (ICC). The scale showed excellent CVI for items and scale. The ICC indicated substantial agreement in the group of lecturers and a high degree of agree- ment in the group of clinicians. This study provides validity evidence for a Swedish version of the scale, sup- porting its use in measuring clinical competence among students undergoing training to become ambulance nurses and paramedics.

  • Numerical modelling of flow in parallel spillways

    • Mathematical modelling of single spillways is well documented in literature. For parallel spillways however, there is a lack of documented, verified, and validated cases. Here, in this article, ANSYS-CFX is used to simulate the flow over three parallel ogee-crested spillways. For mesh size verification, a grid convergence study is performed by Richardson extrapolation. The turbulence model chosen for this simulation is the k-ε model and the volume of fluid method is used to simulate the water-air interface. This article details the models ability to accurately predict flow distribution at the spillways, and the water levels. The mesh is kept relatively coarse at the channel inlet with increased mesh density at the spillways. The results are validated against experimental data from Vattenfall AB, R&Ds laboratories. The geometry and boundary conditions of the experiment are tailored for CFD. The flow rate of each spillway is measured separately with high accuracy, and for several different inlet volumetric flows. The simulation results lie within the error estimates of the measuring tools used in the experiments, within ±1%. The volume flow rate differences between the three outlets is very small, within ±1%.

  • Long-term exposure to particulate air pollution and black carbon in relation to natural and cause-specific mortality: a multicohort study in Sweden

    • Objectives To estimate concentration-response relationships for particulate matter (PM) and black carbon (BC) in relation to mortality in cohorts from three Swedish cities with comparatively low pollutant levels. Setting Cohorts from Gothenburg, Stockholm and Umea, Sweden. Design High-resolution dispersion models were used to estimate annual mean concentrations of PM with aerodynamic diameter <= 10 mu m (PM10) and <= 2.5 mu m (PM2.5), and BC, at individual addresses during each year of follow-up, 1990-2011. Moving averages were calculated for the time windows 1-5 years (lag1-5) and 6-10 years (lag6-10) preceding the outcome. Cause-specific mortality data were obtained from the national cause of death registry. Cohort-specific HRs were estimated using Cox regression models and then meta-analysed including a random effect of cohort. Participants During the study period, 7 340 cases of natural mortality, 2 755 cases of cardiovascular disease (CVD) mortality and 817 cases of respiratory and lung cancer mortality were observed among in total 68 679 individuals and 689 813 person-years of follow-up. Results Both PM10 (range: 6.3-41.9 mu g/m(3)) and BC (range: 0.2-6.8 mu g/m(3)) were associated with natural mortality showing 17% (95% CI 6% to 31%) and 9% (95% CI 0% to 18%) increased risks per 10 mu g/m(3) and 1 mu g/m(3) of lag1-5 exposure, respectively. For PM2.5 (range: 4.0-22.4 mu g/m(3)), the estimated increase was 13% per 5 mu g/m(3), but less precise (95% CI -9% to 40%). Estimates for CVD mortality appeared higher for both PM10 and PM2.5. No association was observed with respiratory mortality. Conclusion The results support an effect of long-term air pollution on natural mortality and mortality in CVD with high relative risks also at low exposure levels. These findings are relevant for future decisions concerning air quality policies.

  • Mortality with Paclitaxel-Coated Devices in Peripheral Artery Disease.

    • The results of a recent meta-analysis aroused concern about an increased risk of death associated with the use of paclitaxel-coated angioplasty balloons and stents in lower-limb endovascular interventions for symptomatic peripheral artery disease.We conducted an unplanned interim analysis of data from a multicenter, randomized, open-label, registry-based clinical trial. At the time of the analysis, 2289 patients had been randomly assigned to treatment with drug-coated devices (the drug-coated-device group, 1149 patients) or treatment with uncoated devices (the uncoated-device group, 1140 patients). Randomization was stratified according to disease severity on the basis of whether patients had chronic limb-threatening ischemia (1480 patients) or intermittent claudication (809 patients). The single end point for this interim analysis was all-cause mortality.No patients were lost to follow-up. Paclitaxel was used as the coating agent for all the drug-coated devices. During a mean follow-up of 2.49 years, 574 patients died, including 293 patients (25.5%) in the drug-coated-device group and 281 patients (24.6%) in the uncoated-device group (hazard ratio, 1.06; 95% confidence interval, 0.92 to 1.22). At 1 year, all-cause mortality was 10.2% (117 patients) in the drug-coated-device group and 9.9% (113 patients) in the uncoated-device group. During the entire follow-up period, there was no significant difference in the incidence of death between the treatment groups among patients with chronic limb-threatening ischemia (33.4% [249 patients] in the drug-coated-device group and 33.1% [243 patients] in the uncoated-device group) or among those with intermittent claudication (10.9% [44 patients] and 9.4% [38 patients], respectively).In this randomized trial in which patients with peripheral artery disease received treatment with paclitaxel-coated or uncoated endovascular devices, the results of an unplanned interim analysis of all-cause mortality did not show a difference between the groups in the incidence of death during 1 to 4 years of follow-up.   • Energinyckeltal och växthusgasutsläpp baserade på industrins energianvändande processer

    • Svensk industri bör strategiskt arbeta mot ökad energi- och resurseffektivitet på en global marknad med knappare resurser. I detta sammanhang spelar beslutsunderlag och nyckeltal en central roll för att nå ökad effektivitet. Även för tillsynsmyndigheter är rättvisande nyckeltal avseende slutenergianvändning av mycket stor vikt för att kunna bedriva ett rättvist förebyggande och proaktivt arbete med svenska företag. De nyckeltal som finns på internationell och nationell nivå är baserade på tillförd energi och ofta relaterade till en ekonomisk output, till exempel förädlingsvärde. Det saknas emellertid nyckeltal kring slutenergianvändningen inom svensk industri fördelat på energibärare såsom el och olja och fördelat på slutenergiprocesser såsom ugnar, tryckluftskompressorer, etc. De siffror som ibland anges är baserade på grova uppskattningar. Projektets mål har därför varit att generera ett processträd avseende flera av de största, till slutenergianvändning räknat, svenska industribranscherna avseende hur slutenergianvändningen är fördelad på processnivå och olika energibärare, samt att allokera växthusgasutsläpp på dessa olika processer. Resultaten indikerar att nyckeltal baserade på energianvändning och indirekta växthusgasutsläpp på processnivå kan bidra till bättre kunskap om i vilka industriella energianvändande processer den största potentialen för energieffektivisering och minskning av växthusgasutsläpp finns. För att upprätthålla kunskap om var den största potentialen för förbättring finns krävs att energidata regelbundet samlas in efter en standardiserad kategorisering av energianvändande processer. Även om projektet har avgränsats till svensk industri kan resultatet vara till nytta också för andra medlemsstater inom EU liksom globalt.

    • The first amplitude analysis of the decay D-s(+)-> K- K+ pi(+) pi(0) is presented using the data samples, corresponding to an integrated luminosity of 6.32 fb(-1), collected with the BESIII detector at e(+)e(-) center-of-mass energies between 4.178 and 4.226 GeV. More than 3000 events selected with a purity of 97.5% are used to perform the amplitude analysis, and nine components are found necessary to describe the data.

    • By using 6.32 fb(-1) of data collected with the BESIII detector at center-of-mass energies between 4.178 and 4.226 GeV, we perform an amplitude analysis of the decay D-s(+) -> K-S(0) pi(+)pi(0) and determine the relative fractions and phase differences of different intermediate processes, which include K-S(0) rho(770)(+), K-S(0)rho(1450)(+), K*(892)(0) pi(+), K* (892)(+) pi(0), and K* (1410)(0) pi(+). With the detection efficiency based on the amplitude analysis results, the absolute branching fraction is measured to be B(D-s(+) -> K-S(0) pi(+) pi(0)) = (5.43 +/- 0.30(stat) +/- 0.15(syst)) x 10(-3).

    • Using 6.32  fb−1 of e+e− collision data collected by the BESIII detector at the center-of-mass energies between 4.178 and 4.226 GeV, an amplitude analysis of the D+s → K0SK−π+π+ decays is performed for the first time to determine the intermediate-resonant contributions. The dominant component is the D+s → K∗(892)+¯K∗(892)0 decay with a fraction of (40.6±2.9stat±4.9sys)%. Our results of the amplitude analysis are used to obtain a more precise measurement of the branching fraction of the D+s → K0SK−π+π+ decay, which is determined to be (1.46±0.05stat±0.05sys)%.

    • We report an amplitude analysis and branching fraction measurement of D-s(+) -> K+K-pi(+) decay using a data sample of 3.19 fb(-1) recorded with BESIII detector at a center-of-mass energy of 4.178 GeV. We perform a model-independent partial wave analysis in the low K+K- mass region to determine the K+K- S-wave line shape, followed by an amplitude analysis of our very pure high-statistics sample. With the detection efficiency based on the amplitude analysis results, the absolute branching fraction is measured to be B(D-s(+) -> K+K-pi(+)) = (5.47 +/- 0.08(stat) +/- 0.13(sys))%.

    • The cross section of the process e(+) e(-) -> eta ' J/ psi is measured at center-of-mass (c.m.) energies from root S = 4.178 to 4.600 GeV using data samples corresponding to a total integrated luminosity of 11 fb(-1) collected with the BESIII detector operating at the BEPCII storage ring. The dependence of the cross section on root S shows an enhancement around 4.2 GeV. While the shape of the cross section cannot be fully explained with a single psi(4160) or psi(4260) state, a coherent sum of the two states does provide a reasonable description of the data.
