    • We report the first measurement of the magnetic dipole and electric quadrupole moment of the exotic nucleus Sn-133 by high-resolution laser spectroscopy at ISOLDE/CERN. These, in combination with state-of-the-art shell-model calculations, demonstrate the single-particle character of the ground state of this short-lived isotope and, hence, the doubly-magic character of its immediate neighbor Sn-132. The trend of the electromagnetic moments along the N = 83 isotonic chain, now enriched with the values of tin, are discussed on the basis of realistic shell-model calculations.

    • Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is commonly diagnosed by reverse transcription polymerase chain reaction (RT-PCR) to detect viral RNA in patient samples, but RNA extraction constitutes a major bottleneck in current testing. Methodological simplification could increase diagnostic availability and efficiency, benefitting patient care and infection control. Here, we describe methods circumventing RNA extraction in COVID-19 testing by performing RT-PCR directly on heat-inactivated or lysed samples. Our data, including benchmarking using 597 clinical patient samples and a standardised diagnostic system, demonstrate that direct RT-PCR is viable option to extraction-based tests. Using controlled amounts of active SARS-CoV-2, we confirm effectiveness of heat inactivation by plaque assay and evaluate various generic buffers as transport medium for direct RT-PCR. Significant savings in time and cost are achieved through RNA-extraction-free protocols that are directly compatible with established PCR-based testing pipelines. This could aid expansion of COVID-19 testing.

  • Improved metabolic control using glucose monitoring systems leads to improvement in vibration perception thresholds in type 1 diabetes patients

    • Aims: Few studies have examined how improved metabolic control might influence vibration perception thresholds (VPTs). The aim of this study was to evaluate if improved HbA1c can influence vibration thresholds in adults with type 1 diabetes (T1DM). Methods: VPTs were investigated at six frequencies (4–125 Hz) using VibroSense Meter in the sole of the foot at two occasions in 159 T1DM patients, at the heads of the first and fifth metatarsal bones, i.e. MTH1 and MTH5, respectively. The participants were divided into three groups: group A: HbA1c improved by more than 1 mmol/mol (n = 95), group B: HbA1c deteriorated by more than 1 mmol/mol (n = 48) and group C: HbA1c unchanged (± 1 mmol/mol) (n = 16) compared to baseline. Results: In group A, the mean z-score, reflecting the combined effect of all VPTs, improved being lower at the follow-up than at the baseline [0.2 (− 0.3 to 1.2) vs. −0.1 (− 0.7 to 0.8), p = 0.00002]. VPTs improved at 4 and 64 Hz at both MTH1 (metatarsal head 1) and MTH5. The VPTs at 125 Hz frequency improved at MTH5, but not at MTH1. No significant differences were seen in group B or group C. Conclusions: Lower HbA1c and lower VPTs in T1DM patients were associated with improved VPT, suggesting a reversible effect on nerve function by improved metabolic control.

    • The SCAC-2 program was initiated to provide an extended scientific knowledge base in national and international discussions and negotiations on the development of new air pollution policies and measures. Specifically, the program was focused on four main areas where additional knowledge was needed to support further actions: air pollution and climate interactions and hemispheric transport; air pollution and human health with focus on particles from transport and domestic wood burning; ecosystem effects (and air pollution – climate interactions) of ozone and nitrogen, the latter with emphasis on national nitrogen budgets and biodiversity. Finally, integrated assessment modelling and identification of the most efficient abatement strategies was included.

  • Costing analysis of a digital first-line treatment platform for patients with knee and hip osteoarthritis in Sweden

    • Osteoarthritis (OA) constitutes a major and increasing burden on patients, health care systems and the broader society. It is estimated that around a quarter of the adult population is affected by OA in the knee and hip and that the prevalence of OA will increase over the coming decades largely due to aging and adverse life-style factors. Prevention and effective care are critical to manage the challenges posed by OA. Digital technologies offer opportunities to deliver cost-effective care for chronic diseases, including for OA. We report the results of a costing analysis of a new digital platform for delivering first-line care including disease information and physiotherapy to patients with OA and compare this with an existing face-to-face model of treatment. Both models are in accordance with National Treatment Guidelines in Sweden. The results show that overall the digital model costs around 25% of the existing face-to-face model of care. Based on existing evidence on the effects of these models, our findings also suggest that the digital platform offers a cost-effective alternative to the existing model of OA care. Depending on the extent to which the digital model substitutes for the existing model of care, significant resources can be saved.

  • Evaluation of small nerve fiber dysfunction in type 2 diabetes

    • Objectives: To assess potential correlations between intraepidermal nerve fiber densities (IENFD), graded with light microscopy, and clinical measures of peripheral neuropathy in elderly male subjects with normal glucose tolerance (NGT), impaired glucose tolerance (IGT), and type 2 diabetes (T2DM), respectively. Materials and methods: IENFD was assessed in thin sections of skin biopsies from distal leg in 86 men (71-77 years); 24 NGT, 15 IGT, and 47 T2DM. Biopsies were immunohistochemically stained for protein gene product (PGP) 9.5, and intraepidermal nerve fibers (IENF) were quantified manually by light microscopy. IENFD was compared between groups with different glucose tolerance and related to neurophysiological tests, including nerve conduction study (NCS; sural and peroneal nerve), quantitative sensory testing (QST), and clinical examination (Total Neuropathy Score; Neuropathy Symptom Score and Neuropathy Disability Score). Results: Absent IENF was seen in subjects with T2DM (n = 10; 21%) and IGT (n = 1; 7%) but not in NGT. IENFD correlated weakly negatively with HbA1c (r = −.268, P =.013) and Total Neuropathy Score (r = −.219, P =.042). Positive correlations were found between IENFD and sural nerve amplitude (r =.371, P =.001) as well as conduction velocity of both the sural (r =.241, P =.029) and peroneal nerve (r =.258, P =.018). Proportions of abnormal sural nerve amplitude became significantly higher with decreasing IENFD. No correlation was found with QST. Inter-rater reliability of IENFD assessment was good (ICC = 0.887). Conclusions: Signs of neuropathy are becoming more prevalent with decreasing IENFD. IENFD can be meaningfully evaluated in thin histopathological sections using the presented technique to detect neuropathy.

    • Aims. The Lande g-factor is an important parameter in astrophysical spectropolarimetry, used to characterize the response of a line to a given value of the magnetic field. The purpose of this paper is to present accurate Lande g-factors for states in B II, C I-IV, Al I-II, Si I-IV, P II, S II, Cl III, Ar IV, Ca I, Ti II, Zr III, and Sn II.Methods. The multiconfiguration Dirac-Hartree-Fock and relativistic configuration interaction methods, which are implemented in the general-purpose relativistic atomic structure package GRASP2K, are employed in the present work to compute the Lande g-factors for states in B II, C I-IV, Al I-II, Si I-IV, P II, S II, Cl III, Ar IV, Ca I, Ti II, Zr III, and Sn II. The accuracy of the wave functions for the states, and thus the accuracy of the resulting Lande g-factors, is evaluated by comparing the computed excitation energies and energy separations with the National Institute of Standards and Technology (NIST) recommended data.Results. All excitation energies are in very good agreement with the NIST values except for Ti II, which has an average difference of 1.06%. The average uncertainty of the energy separations is well below 1% except for the even states of Al I; odd states of Si I, Ca I, Ti II, Zr III; and even states of Sn II for which the relative differences range between 1% and 2%. Comparisons of the computed Lande g-factors are made with available NIST data and experimental values. Analysing the LS-composition of the wave functions, we quantify the departures from LS-coupling and summarize the states for which there is a difference of more than 10% between the computed Lande g-factor and the Lande g-factor in pure LS-coupling. Finally, we compare the computed Lande g-factors with values from the Kurucz database.
