    • The Computer Aided Learning (CAL) working group of the SOCRATES thematic network in Speech Communication Science have studied how the Internet is being used and could be used for the provision of self-study materials for education. In this paper we follow up previous recommendations for the design of Internet tutorials with recommendations for their evaluation. The paper proposes that evaluation should be seen as a necessary quality assurance mechanism operating within the life-cycle of CAL materials development. We propose a structured set of criteria for evaluation, based on the features of good tutorials, against which a tutorial might be judged. Since evaluation against fixed criteria is only one possible approach, we outline how evaluation could also be performed by student users and in controlled trials.

    • The specification of a model for selection to labour market training is studied. Based on present value maximization, it is demonstrated that frequently adopted specifications of wage equations yield selection models containing a difference between alternative wage rates paid in the training period. In addition, the training effect on future wage rates can be sign determined. Using this, we test the hypothesis of present value maximization. A test that is robust against omitted variables and heteroskedasticity is used. Based on a sample of unemployed Swedish workers, we find positive training effects for most sub-groups.

    • Formal methods – the use of exact mathematical methods to describe and analyse systems – have reached a level of maturity where they can be used in practice to formally describe functional safety requirements on signalling systems as well as to verify the compliance of actual systems with these requirements.Interest in formal methods is growing among suppliers and administrations, partly due to standardisation work within CENELEC. However, an important factor for the success of formal methods is the availability of generally accepted description languages, methods, and tools. In the work done so far, there has been little common ground apart from basic principles.We propose the development of an Interlocking Specification Language (ISL). ISL is envisioned as a general purpose specification language augmented with a set of concepts and principles specifically designed to simplify the development of specifications of signalling systems.We will give an overview of the role of formal methods in quality assurance of signalling systems and of how it will be affected by the ISL.

    • The aim of this article is to discuss some of the problems with the feminist theoretical concepts sex and gender and the distinction between them. The focus is on the American historian Thomas Laqueur's use of sex and gender and his concept of the one-sex model. In the critique of Laqueur and in the search for a more ''inclusive'' sex/gender concept, I draw primarily from the work of Lundgren and Kroon and their version of the concept of the symbolic body, which implies that it is impossible to distinguish between ''only'' body and the symbols of the body.

  • Noninvasive haemodynamic evaluation of aortic valve prostheses : Doppler echocardiographic studies at rest and during exercise

    • The present series of investigations was designed to supply information on in vivo pressure-flow relationships and to analyse the incidence of regurgitation in different heart valve prostheses in the aortic position. Patients and methods: The study population comprised 185 patients with mechanical Björk-Shiley Monostrut prostheses, 54 patients with Biocor stented porcine bioprostheses, 50 patients with extended Biocor stentless bioprostheses and 59 patients with cryopreserved aortic homografts. Transvalvular pressure differences and volume flow estimates were derived noninvasively from Doppler measurement of blood velocity across the prosthesis and in the left ventricular outflow tract. Measurements were performed at rest and during supine symptom-limited bicycle exercise. Aortic regurgitation was evaluated semi-quantitatively using colour flow Doppler and continuous-wave Doppler. Patients were investigated after surgery, prior to hospital discharge (baseline) and approximately every year postoperatively. Results: Reference values for baseline Doppler echocardiographic variables for normally functioning prostheses in the aortic position were established. The newly-developed extended Biocor stentless bioprosthesis showed low pressure differences associated with a significant regression of left ventricular hypertrophy during the first six months after aortic valve replacement, and low incidence of aortic regurgitation. A decrease of 40% in pressure differences from baseline to six-month examination was observed in patients with stentless prostheses, partly depending on different haemodynamic states of patients early after the operation. Subcoronarily implanted cryopreserved aortic homografts. demonstrated excellent haemodynamic performance with low pressure differences and large effective orifice areas. Timerelated increasing risk of progression of aortic regurgitation observed during follow-up justifies more frequent echocardiographic monitoring in patients with homograft valves, especially when aortic regurgitation is detected on baseline examination. Different types of prosthesis of similar size were compared at rest and during exercise. The lowest pressure differences and the largest effective orifice areas were demonstrated in homograft valves, followed by extended Biocor stentless bioprostheses. With a 65-70% increase in cardiac output induced by exercise, pressure differences and valve resistance increased significantly while effective orifice area remained unchanged in all four prosthesis types studied. Doppler-derived haemodynamic variables can be obtained at rest and during supine symptom-limited exercise with low variability. The exercise performed in this study was interrupted at a high level of subjective exertion; thus the exercise pressure differences obtained are likely to represent maximal values for pressure differences occurring in the patients' daily activities. Conclusions: In patients undergoing aortic valve replacement, Doppler echocardiography at rest provides basic information on prosthetic valve function. In addition, exercise Doppler echocardiography provides further valuable insight into valve haemodynamics during physiologically increased volume flow. These results can be obtained with low variability. The stentless porcine bioprostheses and cryopreserved aortic homograft valves exhibit low valve resistance to flow at rest and exercise, reflecting the low haemodynamic load imposed by these valves on the left ventricle.

  • Oral glucose and venepuncture reduce blood sampling pain in newborns

    • The objectives of this study were to measure pain symptoms in healthy fullterm newborns undergoing routine blood sampling with different methods. The 120 study subjects were randomly allocated to one of four groups with 30 babies in each, namely venepuncture or heel stick, with or without oral glucose administration. Pain was assessed from the duration of crying within the first 3 min, the Premature Infant Pain Profile (PIPP) and changes in heart rate. When the babies received 1 ml 30% glucose prior to skin puncture there was no significant difference between the heel stick and venepuncture group either in mean crying time (12.9 and 11.6 s, respectively) or in PIPP score (3.9 and 3.3). When no glucose was given crying time was 57.3 s in the heel stick group and 26.8 s in the venepuncture group (P=0.0041) and the mean PIPP scores were 8.4 and 6.0, respectively (P=0.0458). This study suggests that if oral glucose is given prior to skin puncture the choice of blood sampling method has no impact on the pain symptoms.
