    • High-dose methotrexate (HDMTX) is a central component in the treatment of acute lymphoblastic leukemia, osteosarcoma, and some lymphomas and brain tumors. MTX is given at lethal doses and then is followed by rescue treatment with folinic acid (FA). Despite FA rescue, many patients suffer severe toxicity. The pharmacokinetics of FA rescue have not been sufficiently studied. However, optimization of FA rescue could potentially increase anti-tumor effects, whilst decreasing organ toxicity. Here, we describe our efforts to establish and optimize a liquid chromatography tandem mass spectrometric (LC-MS/MS) method for the simultaneous determination of five essential components of the folate cycle, as well as MTX and its two metabolites. The method was applied to 6 individual patients receiving HDMTX, with 3 or 4 measurements for each patient. The method allows analysis of samples that were initially frozen. This notion, together with the test results in the 6 pilot patients, shows the feasibility of this method to study MTX and FA pharmacokinetics during HDMTX treatment. The method has the potential to optimize HDMTX and FA rescue treatment in individual patients.

  • Computer model of mechanical imaging acquisition for virtual clinical trials

    • Malignant breast tumours can be distinguished from benign lesions and normal tissue based on their mechanical properties. Our pilot studies have demonstrated the potential of using Mechanical Imaging (MI) combined with mammography to reduce recalls and false positives in breast cancer screening by more accurately identifying benign lesions. To enable further optimization of MI we propose a computer simulation of the MI acquisition, for use in a Virtual Clinical Trial (VCT) framework. VCTs are computer simulated clinical trials used to efficiently evaluate clinical imaging systems. A linear elastic finite element (FE) model of the breast under dynamic compression was implemented using an open-source FE solver. A spherical tumour (15 mm in diameter) was inserted into the simulated predominantly adipose breast. The location and stiffness of the tumour was varied. The average stress on the compressed breast surface was calculated and compared with the local average stress at the tumour location and the Relative Mean Pressure over lesion Area (RMPA) was calculated. Preliminary results were within a realistic range with an average stress on the breast (tumour) of 5.9-16.6 kPa which is in agreement with published values between 1.0 - 22.5 kPa. This corresponds to RMPA values of 0.96-2.15 depending on stiffness and location of the tumour. This can lead to more detailed validation of various MI acquisition schemes through VCTs before their use in clinical studies.

  • Outcome of Radical Surgical Treatment of Abdominal Aortic Graft and Endograft Infections Comparing Extra-anatomic Bypass with In Situ Reconstruction : A Nationwide Multicentre Study

    • Objective: Abdominal aortic graft and endograft infection (AGI) is primarily treated by resection of the infected graft and restoration of distal perfusion through extra-anatomic bypass (EAB) or in situ reconstruction/repair (ISR). The aim of this study was to compare these surgical strategies in a nationwide multicentre retrospective cohort study.Methods: The Swedish Vascular Registry (Swedvasc) was used to identify surgically treated abdominal AGIs in Sweden between January 1995 and May 2017. The primary aim was to compare short and long term survival, as well as complications for EAB and ISR.Results: Some 126 radically surgically treated AGI patients were identified – 102 graft infections and 24 endograft infections – treated by EAB: 71 and ISR: 55 (23 neo-aorto-iliac systems, NAISs). No differences in early 30 day (EAB 81.7% vs. ISR 76.4%, p =.46), or long term five year survival (48.2% vs. 49.9%, p =.87) were identified. There was no survival difference comparing NAIS to other ISR strategies. The frequency of recurrent graft infection during follow up was similar: EAB 20.3% vs. ISR 17.0% (p =.56). Survival and re-infection rates of the new conduit did not differ between NAIS and other ISR strategies. Age ≥ 75 years (odds ratio [OR] 4.0, confidence interval [CI] 1.1 – 14.8), coronary artery disease (OR 4.2, CI 1.2 – 15.1) and post-operative circulatory complications (OR 5.2, CI 1.2 – 22.5) were associated with early death. Prolonged antimicrobial therapy (> 3 months) was associated with reduced long term mortality (HR 0.3, CI 0.1 – 0.9).Conclusion: In this nationwide multicentre study comparing outcomes of radically treated AGI, no differences in survival or re-infection rate could be identified comparing EAB and ISR.

    • Uterine sarcomas are rare but deadly malignancies without effective treatment. Immunotherapy is a promising new approach to treat these tumors but has shown heterogeneous effects in sarcoma patients. With the goal of identifying key factors for improved patient treatment, we characterized the tumor immune landscape in 58 uterine sarcoma cases with full clinicopathological annotation. Immune cell characterization revealed the overall prevalence of FOXP3+ cells and pro-tumor M2-like macrophages. Hierarchical clustering of patients showed four tumor type-independent immune signatures, where infiltration of FOXP3+ cells and M1-like macrophages associated with favorable prognosis. High CD8+/FOXP3+ ratio in UUS and ESS correlated with poor survival, upregulation of immunosuppressive markers, extracellular matrix (ECM)-related genes and proteins, and YAP activation. This study shows that uterine sarcomas present distinct immune signatures with prognostic value, independent of tumor type, and suggests that targeting the ECM could be beneficial for future treatments.

  • The influence of initial business models on early business performance – a study of 589 new high-tech firm

    • The initial business model of start-ups affects the performance and development of new ventures. Therefore, new technology-based firms (NTBFs) must choose a suitable initial business model to survive, perform, and grow. We propose a measurement framework comprising of nine elements described using 16 variables for NTBFs' initial business models. We test our framework by surveying 589 young manufacturing and knowledge-intensive high-tech firms established in 2013 in Sweden, Finland, and France. We identify differences between the business models across countries and measure sales growth during 2014-2016. We find significant differences between high-tech manufacturing firms and high-tech knowledge-intensive firms regarding key partners, activities, and resources, as well as value propositions and the ranking of elements. In the second part of the analysis, we apply correlation and regression tests for the nine elements regarding early business performance (sales growth) for the manufacturing and knowledge-intensive high-tech firms.

  • Venture capital firms valuation in bull and bear markets: Evidence from Sweden

    • This study uses an experimentally designed case study to investigate Swedish venture capital firms’ valuation practices in two different economic contexts—the economic boom (bull market) of 1999 and the downturn (bear market) of 2002. Studying these periods enables an investigation of changes in valuations, and implicitly, required rate of return, rules of thumb, and valuation models used. Contrary to expectations, in times of heightened stringency and economic downturn, venture capital firms employed fewer valuation models than during boom times. This study thus enriches the knowledge of venture capitalists’ valuation practices, in general, and the effect of market conditions on them. Furthermore, the results can also aid researchers developing more relevant theories of valuation, valuation models, and valuation practices.
