  • Simulation-driven product development of cast components with allowance for process-induced material behaviour
  • 2020
  • Ingår i: Journal of Computational Design and Engineering. - : Oxford University Press. - 2288-5048. ; 7:1, s. 78-85
  • Tidskriftsartikel (refereegranskat)abstract
    • This paper presents a methodology that can be used to consider local variations in thermomechanical and thermophysical material properties, residual stresses, and strength-differential effects in finite element analyses of cast components. The methodology is based on applying process simulations and structural analyses together with experimentally established, or already available literature data, in order to describe element-specific material variations. A cast-iron cylinder head was used in order to evaluate the influence of several simplifications that are commonly performed in computer aided engineering. It is shown that non-trivial errors of a potentially large magnitude are introduced by not considering residual stresses, compressive behaviour, temperature dependence, and process-induced material property variations. By providing design engineers with tools that allow them to consider the complex relationships between these aspects early in the development phase, cast components have the potential to be further optimized with respect to both weight and performance.

  • Does In Vitro Potency Predict Clinically Efficacious Concentrations?
  • 2020
  • Ingår i: Clinical Pharmacology and Therapeutics. - : Wiley. - 0009-9236 .- 1532-6535. ; 108:2, s. 298-305
  • Tidskriftsartikel (refereegranskat)abstract
    • The in vitro affinity of a compound for its target is an important feature in drug discovery, but what remains is how predictive in vitro properties are of in vivo therapeutic drug exposure. We assessed the relationship between in vitro potency and clinically efficacious concentrations for marketed small molecule drugs (n = 164) and how they may differ depending on therapeutic indication, mode of action, receptor type, target localization, and function. Approximately 70% of compounds had a therapeutic unbound plasma exposure lower than in vitro potency; the median ratio of exposure in relation to in vitro potency was 0.32, and 80% had ratios within the range of 0.007 to 8.7. We identified differences in the in vivo-to-in vitro potency ratio between indications, mode of action, target type, and matrix localization, and whether or not the drugs had active metabolites. The in vitro-assay variability contributions appeared to be the smallest; within the same drug target and mode of action the within-variability was slightly broader; but both were substantially less compared with the overall distribution of ratios. These data suggest that in vitro potency conditions, estimated in vivo potency, required level of receptor occupancy, and target turnover are key components for further understanding the link between clinical drug exposure and in vitro potency.

  • Drivers of outsourcing and backsourcing in the public sector : From idealism to pragmatism
  • 2021
  • Ingår i: Financial Accountability and Management. - : Wiley. - 0267-4424 .- 1468-0408. ; 37:3, s. 262-278
  • Tidskriftsartikel (refereegranskat)abstract
    • Local governments are bringing previously outsourced services back in‐house. Research into explanations for sourcing decisions in the public sector is growing, however, few researchers have investigated drivers of both outsourcing and backsourcing in local public‐governance organizations. In this study we utilize transaction cost economic theory (TCE) and political ideology to investigate underlying motives of sourcing in local public governance organizations. Based on a 2018 survey of chief financial officers (CEO) in all of Sweden's 290 municipalities, this study shows that backsourcing is strongly associated with outsourcing and that outsourcing and backsourcing should not be understood as opposite phenomena, rather as interdependent phenomena in a dynamic sourcing strategy. Outsourcing and backsourcing are driven in part by different factors: Outsourcing by political ambitions and economic factors relating to TCE, while managerial and pragmatic concerns are foregrounded for backsourcing.

  • Kontinuerlig glukosmätning alternativ vid typ 2-diabetes [Continuous glucose monitoring (CGM) in type 2 diabetes]
  • 2021
  • Ingår i: Läkartidningen. - : Läkartidningen Förlag AB. - 0023-7205 .- 1652-7518. ; 118
  • Tidskriftsartikel (refereegranskat)abstract
    • Continuous glucose monitoring (CGM) tracks glucose levels in real-time using a subcutaneous sensor, replacing intermittent blood sampling for self-monitoring of blood glucose (SMBG). CGM is a routine tool in type 1 diabetes management. In 2019, patients with type 2 diabetes were given the indication of CGM use by Swedish authorities; CGM can be considered when a patient despite multi-dose treatment with insulin does not achieve good glucose control and/or has problems with hypoglycaemia. Studies show that CGM provides improved glycaemic control compared to SMBG. The cost of CGM is higher than for SMBG and requires effort both by caregivers and patients. This should be gauged against the possible long-term health economic benefits of preventing diabetes complications.

  • När HbA1c inte stämmer : Olika metoder kan ge olika resultat – och ibland kan även resultat som stämmer överens ge fel bild av glukosbalansen
  • 2020
  • Ingår i: Läkartidningen. - : Läkartidningen Förlag AB. - 0023-7205 .- 1652-7518. ; 117:37
  • Forskningsöversikt (refereegranskat)abstract
    • Compared to the 1990s when HbA1c was established as a marker for glycemic control, the accuracy of the HbA1c assays has greatly improved and HbA1c is currently used also for the diagnosis of type 2 diabetes. For most patients, the agreement is excellent between glycemic status and HbA1c results achieved by various methods. However, for patients with increased erythrocyte turnover, HbA1c does not reflect the glycemic status. For patients with rare haemoglobin variants the HbA1c value might be falsely decreased or increased, depending on which HbA1c assay has been used. Recently, falsely increased HbA1c results due to aspirin interference in an ion-exchange method were reported. When misleading results are suspected comparison of results from different HbA1c methods using different analytical principles or continuous glucose monitoring might be useful. HbA1c is likely to remain the most important marker for glycemic control, but complementary tests have to be established.

  • Effects on body weight, eating behavior, and quality of life of a low-energy diet combined with behavioral group treatment of persons with class II or III obesity : A 2-year pilot study
  • 2021
  • Ingår i: Obesity Science & Practice. - : John Wiley & Sons. - 2055-2238. ; 7:1, s. 4-13
  • Tidskriftsartikel (refereegranskat)abstract
    • Objective: Obesity is associated with reduced health-related quality of life (HRQoL). Outcomes of nonsurgical weight loss treatment on HRQoL are inconsistent and it is unclear how much weight reduction, or what type of treatment, is required for significant improvements. This study aimed to evaluate the effects of a lifestyle intervention program on weight, eating behaviors, and HRQoL, and to describe participants' experiences of treatment. Methods: This 2-year intervention trial in persons with class II or III obesity comprised a 3-month liquid low-energy diet (880 kcal/d) followed by a 3-month reintroduction to regular foods, combined with behavioral group treatment. Results: Fifty-five participants (73% women) were included, mean (SD) age 43.2 (12.4) years, and mean body mass index 42.0 (6.0) kg/m(2). Mean weight loss at 6, 12, and 24 months was 18.9%, 13.7%, and 7.2%, respectively. Short- and long-term effects on eating behavior were favorable. Twelve of 14 HRQoL domains were improved at 6 months, compared to eight domains at 12 months. After 24 months, 2 of 14 domains, physical and psychosocial functioning, were improved. The treatment program was well accepted by the participants. Conclusions: Substantial weight loss after 6 months was associated with extensive improvements in HRQoL, comprising the physical, psychosocial, and mental domains. Significant weight regain was observed between 6 and 24 months follow-up. Modest weight loss after 24 months was associated with moderate improvement in physical functioning and large improvement in psychosocial functioning. The effect on psychosocial functioning is most likely related to both weight loss and behavioral treatment.

  • Assessment of Global Lung Function Initiative (GLI) reference equations for diffusing capacity in relation to respiratory burden in the Swedish CArdioPulmonary bioImage Study (SCAPIS)
  • 2020
  • Ingår i: European Respiratory Journal. - Lausanne, Switzerland : European Respiratory Society (ERS). - 0903-1936 .- 1399-3003. ; 56:2
  • Tidskriftsartikel (refereegranskat)abstract
    • The Global Lung Function Initiative (GLI) has recently published international reference values for diffusing capacity of the lung for carbon monoxide (DLCO). Lower limit of normal (LLN), i.e. the 5th percentile, usually defines impaired DLCO. We examined if the GLI LLN for DLCO differs from the LLN in a Swedish population of healthy, never-smoking individuals and how any such differences affect identification of subjects with respiratory burden.Spirometry, DLCO, chest high-resolution computed tomography (HRCT) and questionnaires were obtained from the first 15 040 participants, aged 50–64 years, of the Swedish CArdioPulmonary bioImage Study (SCAPIS). Both GLI reference values and the lambda-mu-sigma (LMS) method were used to define the LLN in asymptomatic never-smokers without respiratory disease (n=4903, of which 2329 were women). Both the median and LLN for DLCO from SCAPIS were above the median and LLN from the GLI (p<0.05). The prevalence of DLCO DLCO >GLI LLN but DLCO >GLI LLN but versus 4.5%, p<0.001), chronic airflow limitation (8.5% versus 3.9%, p<0.001) and chronic bronchitis (8.3% versus 4.4%, p<0.01) than subjects (n=13 600) with normal DLCO (>GLI LLN and >SCAPIS LLN). No differences were found with regard to physician-diagnosed asthma. The GLI LLN for DLCO is lower than the estimated LLN in healthy, never-smoking, middle-aged Swedish adults. Individuals with DLCO above the GLI LLN but below the SCAPIS LLN had, to a larger extent, an increased respiratory burden. This suggests clinical implications for choosing an adequate LLN for studied populations.
