  • Effects of nitrogen enrichment on zooplankton biomass and N:P recycling ratios across a DOC gradient in northern-latitude lakes

    • We used data from whole-lake studies to assess how changes in food quantity (phytoplankton biomass) and quality (phytoplankton community composition, seston C:P and N:P) with N fertilization affect zooplankton biomass, community composition and C:N:P stoichiometry, and their N:P recycling ratio along a gradient in lake DOC concentrations. We found that despite major differences in phytoplankton biomass with DOC (unimodal distributions, especially with N fertilization), no major differences in zooplankton biomass were detectable. Instead, phytoplankton to zooplankton biomass ratios were high, especially at intermediate DOC and after N fertilization, implying low trophic transfer efficiencies. An explanation for the observed low phytoplankton resource use, and biomass responses in zooplankton, was dominance of colony forming chlorophytes of reduced edibility at intermediate lake DOC, combined with reduced phytoplankton mineral quality (enhanced seston N:P) with N fertilization. N fertilization, however, increased zooplankton N:P recycling ratios, with largest impact at low DOC where phytoplankton benefitted from light sufficiently to cause enhanced seston N:P. Our results suggest that although N enrichment and increased phytoplankton biomass do not necessarily increase zooplankton biomass, bottom-up effects may still impact zooplankton and their N:P recycling ratio through promotion of phytoplankton species of low edibility and altered mineral quality.

  • Multicenter study on postcardiotomy venoarterial extracorporeal membrane oxygenation

    • Objectives: The aim of this study was to identify the risk factors associated with early mortality after postcardiotomy venoarterial extracorporeal membrane oxygenation. Methods: This is an analysis of the postcardiotomy extracorporeal membrane oxygenation registry, a retrospective multicenter cohort study including 781 patients aged more than 18 years who required venoarterial extracorporeal membrane oxygenation for cardiopulmonary failure after cardiac surgery from 2010 to 2018 at 19 cardiac surgery centers. Results: After a mean venoarterial extracorporeal membrane oxygenation therapy of 6.9 ± 6.2 days, hospital and 1-year mortality were 64.4% and 67.2%, respectively. Hospital mortality after venoarterial extracorporeal membrane oxygenation therapy for more than 7 days was 60.5% (P = .105). Centers that had treated more than 50 patients with postcardiotomy venoarterial extracorporeal membrane oxygenation had a significantly lower hospital mortality than lower-volume centers (60.7% vs 70.7%, adjusted odds ratio, 0.58; 95% confidence interval, 0.41-0.82). The postcardiotomy extracorporeal membrane oxygenation score was derived by assigning a weighted integer to each independent pre–venoarterial extracorporeal membrane oxygenation predictors of hospital mortality as follows: female gender (1 point), advanced age (60-69 years, 2 points; ≥70 years, 4 points), prior cardiac surgery (1 point), arterial lactate 6.0 mmol/L or greater before venoarterial extracorporeal membrane oxygenation (2 points), aortic arch surgery (4 points), and preoperative stroke/unconsciousness (5 points). The hospital mortality rates according to the postcardiotomy extracorporeal membrane oxygenation score was 0 point, 45.6%; 1 point, 40.5%; 2 points, 51.1%; 3 points, 57.8%; 4 points, 70.7%; 5 points, 68.3%; 6 points, 77.5%; and 7 points or more, 89.7% (P < .0001). Conclusions: Age, female gender, prior cardiac surgery, preoperative acute neurologic events, aortic arch surgery, and increased arterial lactate were associated with increased risk of early mortality after postcardiotomy venoarterial extracorporeal membrane oxygenation. Center experience with postcardiotomy venoarterial extracorporeal membrane oxygenation may contribute to improved results.

  • Hyaluronic acid spacer in prostate cancer radiotherapy : dosimetric effects, spacer stability and long-term toxicity and PRO in a phase II study

    • BACKGROUND: Perirectal spacers may be beneficial to reduce rectal side effects from radiotherapy (RT). Here, we present the impact of a hyaluronic acid (HA) perirectal spacer on rectal dose as well as spacer stability, long-term gastrointestinal (GI) and genitourinary (GU) toxicity and patient-reported outcome (PRO).METHODS: In this phase II study 81 patients with low- and intermediate-risk prostate cancer received transrectal injections with HA before external beam RT (78 Gy in 39 fractions). The HA spacer was evaluated with MRI four times; before (MR0) and after HA-injection (MR1), at the middle (MR2) and at the end (MR3) of RT. GI and GU toxicity was assessed by physician for up to five years according to the RTOG scale. PROs were collected using the Swedish National Prostate Cancer Registry and Prostate cancer symptom scale questionnaires.RESULTS: There was a significant reduction in rectal V70% (54.6 Gy) and V90% (70.2 Gy) between MR0 and MR1, as well as between MR0 to MR2 and MR3. From MR1 to MR2/MR3, HA thickness decreased with 28%/32% and CTV-rectum space with 19%/17% in the middle level. The cumulative late grade ≥ 2 GI toxicity at 5 years was 5% and the proportion of PRO moderate or severe overall bowel problems at 5 years follow-up was 12%. Cumulative late grade ≥ 2 GU toxicity at 5 years was 12% and moderate or severe overall urinary problems at 5 years were 10%.CONCLUSION: We show that the HA spacer reduced rectal dose and long-term toxicity.

  • Challenging cultures of rejection

    • In this article, Bojanic, Jonsson, Neergaard and Sauer present a synthetic overview of the five country cases included in the special issue that analyse the emergence of cultures of rejection since 2015. In general, they discuss the conceptual framework of ‘Cultures of Rejection’, elaborated throughout the issue as a more encompassing approach that is sensitive to the values, norms and affects that underlie different or similar patterns of exclusion and rejection in different contexts. These cultures are located in the everyday lives of people. The article, therefore, first identifies contexts, objects of rejection­—often migrants and racialized Others, but also ‘the political’ or state institutions—narratives and components of cultures of rejection that we label reflexivity, affect, nostalgia and moralistic judgement. The contrasting reading of the five cases shows that people struggle for agency under precarious and insecure conditions, and fight against imagined enemies. As Bojanić, Jonsson, Neergaard and Sauer conclude, cultures of rejection mirror ongoing processes of neoliberal dispossession, authoritarization and depolitization that culminate in a wish for agency and resovereignization. Second, and based on this overview, trends in cultures of rejection are detected against different national contexts as well as against common trends of social and economic transformations and crises, such as, for instance, the COVID-19 pandemic. This results, finally, in a discussion of ways of challenging the cultures of rejection towards more democratic and solidaristic societies. One starting point might be the ‘re-embedding’ of the economy in society, that is, a more equal distribution of resources and future perspectives.

  • European Multicentric Experience With Fenestrated-branched ENDOvascular Stent Grafting After Previous FAILed Infrarenal Aortic Repair The EU-FBENDO-FAIL Registry

    • Objective:To report the mid-term outcomes of fenestrated-branched endovascular aneurysm repair (F-BEVAR) following a failed previous endovascular aneurysm repair (pEVAR) or previous open aneurysm repair (pOAR).Methods:Data from consecutive patients who underwent F-BEVAR for pEVAR or pOAR from 2006 to 2021 from 17 European vascular centers were analyzed. Endpoints included technical success, major adverse events, 30-day mortality, and 5-year estimates of survival, target vessel primary patency, freedom from reinterventions, type I/III endoleaks, and sac growth >5 mm.Background:Treatment of a failed previous abdominal aortic aneurysm repair is a complex undertaking. F-BEVAR is becoming an increasingly attractive option, although comparative data are limited regarding associated risk factors, indications for treatment, and various outcomes.Results:There were 526 patients included, 268 pOAR and 258 pEVAR. The median time from previous repair to F-BEVAR was 7 (interquartile range, 4-12) years, 5 (3-8) for pEVAR, and 10 (6-14) for pOAR, P<0.001. Predominant indication for treatment was type Ia endoleak for pEVAR and progression of the disease for pOAR. Technical success was 92.8%, pOAR (92.2%), and pEVAR (93.4%), P=0.58. The 30-day mortality was 6.5% overall, 6.7% for pOAR, and 6.2% for pEVAR, P=0.81. There were 1853 treated target vessels with 5-year estimates of primary patency of 94.4%, pEVAR (95.2%), and pOAR (94.4%), P=0.03. Five-year estimates for freedom from type I/III endoleaks were similar between groups; freedom from reintervention was lower for pEVAR (38.3%) than for pOAR (56.0%), P=0.004. The most common indication for reinterventions was for type I/III endoleaks (37.5%).Conclusions:Repair of a failed pEVAR or pOARis safe and feasible with comparable technical success and survival rates. While successful treatment can be achieved, significant rates of reintervention should be anticipated, particularly for issues related to instability of target vessels/bridging stents.

  • Absolute Quantification of Donor-Derived Cell-Free DNA in Pediatric and Adult Patients After Heart Transplantation: A Prospective Study.

    • In this prospective study we investigated a cohort after heart transplantation with a novel PCR-based approach with focus on treated rejection. Blood samples were collected coincidentally to biopsies, and both absolute levels of dd-cfDNA and donor fraction were reported using digital PCR. 52 patients (11 children and 41 adults) were enrolled (NCT03477383, clinicaltrials.gov), and 557 plasma samples were analyzed. 13 treated rejection episodes >14 days after transplantation were observed in 7 patients. Donor fraction showed a median of 0.08% in the cohort and was significantly elevated during rejection (median 0.19%, p < 0.0001), using a cut-off of 0.1%, the sensitivity/specificity were 92%/56% (AUC ROC-curve: 0.78). Absolute levels of dd-cfDNA showed a median of 8.8 copies/mL and were significantly elevated during rejection (median 23, p = 0.0001). Using a cut-off of 7.5 copies/mL, the sensitivity/specificity were 92%/43% for donor fraction (AUC ROC-curve: 0.75). The results support the feasibility of this approach in analyzing dd-cfDNA after heart transplantation. The obtained values are well aligned with results from other trials. The possibility to quantify absolute levels adds important value to the differentiation between ongoing graft damage and quiescent situations.

    • Zinkgruvanite, ideally Ba4Mn2+4Fe3+2(Si2O7)2(SO4)2O2(OH)2, is a new member of the ericssonite group, found in Ba-rich drill core samples from a sphalerite+galena- and diopside-rich metatuffite succession from the Zinkgruvan mine, Örebro county, Sweden. Zinkgruvanite is associated with massive baryte, barytocalcite, diopside and minor witherite, cerchiaraite-(Al) and sulfide minerals. It occurs as subhedral to euhedral flattened and elongated crystals up to 4 mm. It is almost black, semi-opaque with a dark brown streak. The luster is vitreous to sub-adamantine on crystal faces, resinous on fractures. The mineral is brittle with an uneven fracture. VHN100 = 539 and HMohs ~4½. In thin fragments, it is reddish-black, translucent and optically biaxial (+), 2Vz > 70°. Pleochroism is strong, deep brown-red (E ⊥ {001} cleavage) to olive-pale brown. Chemical point analyses by WDS-EPMA together with iron valencies determined from Mössbauer spectroscopy, yielded the empirical formula (based on 26 O+OH+F+Cl anions): (Ba4.02Na0.03)Σ4.05(Mn1.79Fe2+1.56Fe3+0.42Mg0.14Ca0.10Ni0.01Zn0.01)Σ4.03 (Fe3+1.74Ti0.20Al0.06)Σ2.00Si4(S1.61Si0.32P0.07)Σ1.99O24(OH1.63Cl0.29F0.08)Σ2.00. The mineral is triclinic, space group P–1, with unit-cell parameters a = 5.3982(1) Å, b = 7.0237(1) Å, c = 14.8108(4) Å, α = 98.256(2)º, β = 93.379(2)º, γ = 89.985(2)º and V = 554.75(2) Å3 for Z = 1. The eight strongest X-ray powder diffraction lines are [d Å (I%; hkl)]: 3.508 (70; 103), 2.980(70; 11–4), 2.814 (68; 1–22), 2.777 (70; 121), 2.699 (714; 200), 2.680 (68; 20–1), 2.125 (100; 124, 204), 2.107 (96; –221). The crystal structure (R1 = 0.0379 for 3204 reflections) is an array of TS (titanium silicate) blocks alternating with intermediate blocks. The TS blocks consist of HOH sheets (H = heteropolyhedral, O = octahedral) parallel to (001). In the O sheet, the Mn2+-dominant MO(1,2,3) sites give ideally Mn2+4 pfu. In the H sheet, the Fe3+-dominant MH sites and AP(1) sites give ideally Fe3+2Ba2 pfu. In the intermediate block, SO4 oxyanions and eleven coordinated Ba atoms give ideally 2 × SO4Ba pfu. Zinkgruvanite is related to ericssonite and ferro-ericssonite in having the same topology and type of linkage of layers in the TS block. Zinkgruvanite is also closely compositionally related to yoshimuraite, Ba4Mn4Ti2(Si2O7)2(PO4)2O2(OH)2, via the coupled heterovalent substitution 2 Ti4+ + 2 (PO4)3- →2 Fe3+ + 2 (SO4)2-, but presents a different type of linkage. The new mineral probably formed during a late stage of regional metamorphism of a Ba-enriched, syngenetic protolith, involving locally generated oxidized fluids of high salinity.

  • Compact heat exchangers for hydrogen-fueled aero engine intercooling and recuperation

    • This study investigates the application of compact heat exchangers for the purpose of intercooling and recuperation systems for short-to-medium range aircraft equipped with hydrogen-fueled turbofan engines. The primary objective is to assess the potential effects of engine-integrated compact heat exchangers on fuel consumption and emissions. The paper encompasses the conceptual design of integrated heat exchangers and associated ducts, followed by aerodynamic optimization studies to identify suitable designs that minimize air-side pressure losses and ensure flow uniformity at the inlet of the high-pressure compressor. Pressure drop correlations are then established for selected duct designs and incorporated into a system-level performance model, allowing for a comparison of their impact on specific fuel consumption, NOx emissions, and fuel burn against an uncooled baseline engine. The intercooled-recuperated engine resulted in the most significant improvement in take-off specific fuel consumption, with a reduction of up to 7.7% compared to the baseline uncooled engine, whereas the best intercooled engine resulted in an improvement of about 4%. Furthermore, the best configuration demonstrated a decrease in NOx emissions by up to 37% at take-off and a reduction in mission fuel burn by 5.5%. These enhancements were attributed to reduced compression work, pre-heating of the hydrogen fuel, and lower high-pressure compressor outlet temperatures.

    • Hydrogen is a promising fuel for future aviation due to its CO2-free combustion. In addition, its excellent cooling properties as it is heated from cryogenic conditions to the appropriate combustion temperatures provides a multitude of opportunities. This paper investigates the heat transfer potential of stator surfaces in a modern high-speed low-pressure compressor by incorporating cooling channels within the stator vane surfaces, where hydrogen is allowed to flow and cool the engine core air. Computational Fluid Dynamics simulations were carried out to assess the aerothermal performance of this cooled compressor and were compared to heat transfer correlations. A core air temperature drop of 9.5 K was observed for this cooling channel design while being relatively insensitive to the thermal conductivity of the vane and cooling channel wall thickness. The thermal resistance was dominated by the air-side convective heat transfer, and more surface area on the air-side would therefore be required in order to increase overall heat flow. While good agreement with established heat transfer correlations was found for both turbulent and transitional flow, the correlation for the transitional case yielded decent accuracy only as long as the flow remains attached, and while transition was dominated by the bypass mode. A system level analysis, indicated a limited but favorable impact at engine performance level, amounting to a specific fuel consumption improvement of up to 0.8 % in cruise and an estimated reduction of 3.6 % in cruise NOx. The results clearly show that, although it is possible to achieve high heat transfer rate per unit area in compressor vanes, the impact on cycle performance is constrained by the limited available wetted area in the low-pressure compressor.
