  • Plasma erythropoietin concentrations in polycythaemia vera with special reference to myelosuppressive therapy.

    • In 80 patients with polycythaemia vera (PV) a total of 108 venous blood samples were obtained and analysed for EDTA-plasma erythropoietin (EPO) concentration. At the time of study 21 of the PV patients were newly diagnosed and had prior to blood sampling neither received phlebotomy treatment nor therapy with myelosuppressive agents; these subjects had a mean plasma EPO concentration of 0.5+/-0.9 IU/L. Thirty-seven patients treated with phlebotomy only had a mean plasma EPO concentration of 2.5+/-2.9 IU/L. The mean plasma EPO concentrations for 26 patients treated with hydroxyurea, 13 patients treated with radiophosphorous and 11 patients given a combination of myelosuppressive agents were 8.9+/-8.0, 10.9+/-12.6 and 7.2+/-7.4 IU/L, respectively. Untreated patients and patients on phlebotomy only had significantly lower values for plasma EPO than patients on therapy with myelosuppressive drugs. This finding persisted also after a correction for differences in haemoglobin levels had been introduced. Thereby, the present results would suggest a difference in the EPO feedback system in untreated and phlebotomised PV patients compared to PV patients treated with myelosuppressive agents.

  • Transient corneal edema after phacoemulsification : comparison of 3 viscoelastic regimens

    • PURPOSE: To evaluate the effect of different viscoelastic substances on the grade and time course of postoperative corneal edema. SETTING: Department of Clinical Sciences/Ophthalmology, Umeå University Hospital, Umeå, Sweden. METHODS: This study comprised 62 patients with otherwise healthy eyes who had routine phacoemulsification and intraocular lens (IOL) implantation. Patients were divided into 3 groups. Group 1 was given Healon GV (sodium hyaluronate 1.4%) at phacoemulsification and IOL implantation. Group 2 was given Viscoat (sodium hyaluronate 3.0%-chondroitin sulfate 4.0%) at phacoemulsification and Healon GV at IOL implantation. Group 3 was given Viscoat at phacoemulsification and Provisc (sodium hyaluronate 1.0%) at lens implantation. The central corneal thickness was measured with ultrasonic pachymetry before surgery and 5 and 24 hours, 1 week, and 1 month after surgery. RESULTS: The mean increase in corneal thickness was significantly greater in Group 1 than in the other 2 groups 5 and 24 hours and 1 week after surgery. CONCLUSIONS: The transient postoperative increase in central corneal thickness was greater in patients receiving Healon GV during phacoemulsification than in patients receiving Viscoat. The use of Provisc or Healon GV for IOL implantation did not affect the postoperative corneal thickness when Viscoat was used for phacoemulsification. The time course of the edema may be explained by a difference between the 2 agents in endothelial protection from ultrasonic, mechanical, or irrigation trauma.

  • Recent aging studies for the ATLAS transition radiation tracker

    • The transition radiation tracker (TRT) is one of the three subsystems of the inner detector of the ATLAS experiment. It is designed to operate for 10 yr at the LHC, with integrated charges of /spl sim/10 C/cm of wire and radiation doses of about 10 Mrad and 2/spl times/10/sup 14/ neutrons/cm/sup 2/. These doses translate into unprecedented ionization currents and integrated charges for a large-scale gaseous detector. This paper describes studies leading to the adoption of a new ionization gas regime for the ATLAS TRT. In this new regime, the primary gas mixture is 70%Xe-27%CO/sub 2/-3%O/sub 2/. It is planned to occasionally flush and operate the TRT detector with an Ar-based ternary mixture, containing a small percentage of CF/sub 4/, to remove, if needed, silicon pollution from the anode wires. This procedure has been validated in realistic conditions and would require a few days of dedicated operation. This paper covers both performance and aging studies with the new TRT gas mixture.

  • EUROQUEST - A questionnaire for solvent related symptoms: Factor structure, item analysis and predictive validity

    • The study evaluates the factor structure and predictive validity of the symptom questionnaire EUROQUEST (EQ) that had been developed with the goal of simplifying the evaluation of health effects associated with long-term solvent exposure. The EQ was added to the normal evaluation procedures for 118 male patients with suspected solvent-induced toxic encephalopathy (TE) referred to seven Swedish clinics of occupational medicine during an 18-month period. EQ was also completed by 239 males from a random sample of 400 Swedish males aged 25-64 years selected from the general population and a sample of 559 occupationally active male spray painters aged 25-64 years. Factor and item analyses of EQ responses were performed. Ordinary least square regression analysis was used to evaluate sensitivity and correlation to evaluate the specificity of EQ and the separate components. Questions concerning memory and concentration symptoms alone showed better sensitivity than the other five EQ dimensions singly or combined for the entire EQ and for a subset of questions approximating Q16, a widely used organic solvent symptom screening questionnaire. However, the diagnosis of TE required information in addition to exposure and responses to EQ and Q16-like questions. The results indicate that the subset of EQ questions concerning memory and concentration might replace the more cumbersome EQ and less sensitive Q16 in screening for TE, although none of the screening instruments alone replaces current clinical diagnostic procedures.     • This thesis analyses police officers' work with internal control of foreigners through a discussion of, among other things, the nation-state, rule of law ideals as well as the police profession and the conditions surrounding that profession. Internal control of foreigners, practised by police officers has two main aims: to control migration and to fight crime amongst people unauthorised to reside in the country. There has been a demand for increased efficiency in the internal control of foreigners, since Sweden joined the Schengen agreement. Police officers are now expected to be observant in all their work carried out, of the possibility that people they encounter are in the country without permission.The authors have accompanied police officers in Malmö and Stockholm in their work. They have also studied cases in the legal system and of the special board that handles cases of offences made by police officers. The authors discuss the importance of different factors to the police work on internal control of foreigners. Certain factors are more important than others and there is an interplay and reinforcement of some of these: the political and historical context, the discretion of the police, grey areas in the rules, the work tools of the police as well as inspection possibilities.A state of tension can be identified between the ideals of the nation-state and rule of law ideals. Several factors interplay to create a potential for and an apparent risk of ethnic discrimination in the police work on the internal control of foreigners. This study shows that the prerequisites necessary to perform the control efficiently, unerringly and with proper discretion are lacking. Police officers are put to the almost impossible task of determining who is in the country without permission.

  • Intracameral mydriatics in phacoemulsification cataract surgery.

    • PURPOSE: To evaluate intracameral injection of mydriatics in phacoemulsification cataract surgery and compare the results with those of conventional topical mydriatics. SETTING: Department of Clinical Science/Ophthalmology, Umeå University Hospital, Umeå, Sweden. METHODS: This prospective randomized double-blind study included 60 patients who were given topical (topical group) or intracameral (intracameral group) mydriatics. The topical mydriatics comprised 3 drops of cyclopentolate 1% and phenylephrine 10% given 15 minutes apart and 150 microL intracameral lidocaine hydrochloride 1% (Xylocaine) and the intracameral mydriatics, placebo eyedrops and 150 microL intracameral cyclopentolate 0.1%, phenylephrine 1.5%, and Xylocaine 1%. The pupil size was recorded preoperatively, throughout surgery, and 1 day and 1 month postoperatively. Preoperative and postoperative corneal endothelial morphology, corneal thickness, intraocular pressure, visual acuity, aqueous cells and flare, phacoemulsification energy, duration of surgery, pulse, blood pressure, and intraoperative sensation of pain and glare were also recorded. RESULTS: With intracameral mydriatics, mydriasis reached 95% +/- 3% (SD) of its final value within 20 seconds. In the intracameral group, the pupils were smaller than in the topical group (mean 6.7 +/- 1.0 mm versus 7.7 +/- 1.0 mm, P<.001) but did not contract intraoperatively. The pupils in the topical group tended to contract, and the difference between groups was significant (P =.0020). The intracameral group reported less glare during the procedure (P<.001). There was no difference in endothelial cell loss, inflammatory reaction, postoperative corneal swelling, or surgical performance between the groups. CONCLUSIONS: Intracameral mydriatics were a rapid, effective, and safe alternative to topical mydriatics in phacoemulsification. Their use can simplify preoperative routines and in certain high-risk groups, may reduce the risk for cardiovascular side effects.

    • The aim of a study comprising 607 women working as homecare personnel was to investigate general spinal, joint and segmental mobility, different symptoms (pain and strain) and their relation to various aspects of disability. Joint mobility (mainly peripheral) was estimated using the 'Beighton' score and spinal posture and mobility were measured by kyphometer. Passive segmental mobility and pain provocation were estimated manually. Pain intensity and strain during work and leisure were estimated using visual analogue scales for defined anatomical regions. Disability was rated using defined items and two indices. The 7-day prevalence of low back pain was 48%. Peripheral joint mobility, spinal sagittal posture and thoracic sagittal mobility showed low correlations with disability. Lumbar sagittal hypomobility was associated with higher disability. Manually estimated segmental mobility and segmental pain provocation of L4-L5 and L5-S1 correlated with disability; hypo- and hypermobility or positive pain provocation tests at these levels showed higher disability than normal mobility and negative pain provocation tests, respectively. Cluster analysis revealed that the combination of positive pain provocation tests and low lumbar sagittal mobility was associated with particularly high disability levels. In conclusion, positive pain provocation tests were clearly associated with high disability levels.

  • Tender point scores and their relations to signs of mobility, symptoms, and disability in female home care personnel and the prevalence of fibromyalgia syndrome

    • OBJECTIVE: In this study of female home care personnel employed in a municipality (n = 643; participation rate 94%) we investigated (1) the prevalence of tender points and fibromyalgia (FM); (2) the relationships between tender point score and other signs and symptoms; (3) if subgroups based on the tender point score differed with respect to signs, symptoms, disability, and health related quality of life; and (4) signs that showed the strongest intercorrelations with disability and health.METHODS: The following variables were registered: (1) Signs: joint mobility, spinal posture and mobility, tender points, and segmental mobility and pain provocation at L4-S1 levels of the low back. (2) Symptoms: pain and pain intensity and other symptoms. (3) Disability (i.e., self-rated reduced capacity for everyday activities and employment) and health: 3 indices and sick leave.RESULTS: The tender point score correlated with the number of pain regions and the pain intensities, and the amount of other symptoms, sick leave, and disability. Tender point score was the strongest regressor of the investigated signs in regression of the 2 disability indices. Segmental pain showed the strongest correlation with tender point score. Three subgroups identified by tender point score showed significant differences in segmental pain, prevalence and intensity of different symptoms, disability, and health related quality of life. The prevalence of FM was 2.0%.CONCLUSION: Tender point score together with different symptoms showed relatively strong correlations with disability. A relatively high prevalence of FM was found in occupationally active female home care personnel.

  • Crystallography and porosity effects of CO conversion on mesoporous CeO2

    • Catalytic properties and thermal stability were studied for samples of mesoporous ceria with different BET specific surface area. The catalytic conversion of carbon monoxide to carbon dioxide and how thermal treatments of the catalysts influence the catalytic properties have been investigated. The materials were studied by transmission electron microscopy and by conversion profile measurements of CO versus temperature using a plug flow micro reactor made in quartz glass only. In order to compare the catalytic properties associated with a specific structure or morphology directly, aliquots of surface area (0.6 m(2)) of the catalyst was used. Scanning electron microscopy and X-ray energy-dispersive spectrometry (XEDS) were used for surface morphology studies and elemental analysis. It was found that the proportion of {100} surfaces determine the catalytic properties of the material and these surfaces become important at calcination temperatures between 773 and 973 K. The internal mesoporous structure is destroyed at calcination temperatures around 873 K. (C) 2004 Elsevier Inc. All rights reserved.
