    • The time trend monitoring of organochlorine pollution was carried out in Sweden since the late 1960s. This report presents data on concentrations of DDT, PCB, HCHs and HCB in biota samples collected and analysed annually. All the matrices and compounds studied show a significant decrease over time. The data cover severely polluted Swedish marine and fresh water in southern Sweden as well as locally unpolluted waters in remote northern Arctic regions of Sweden. A total of 13 time series representing different locations and species are presented for the different pollutants. The period studied covers the time when pollution was serious as well as the time of recovery. All monitoring activities were carried out at the same laboratories over the entire study period, which means that comparability over time is good in the sets of data presented. The various time trends show a convincing agreement with trends and annual change over time, although the concentrations differ between the species and locations investigated, the highest concentrations being in the south. Since the annual changes are normally similar regardless of locations and species, spatial variations in concentrations remain over time, although concentrations are lower today. The onset of changes in concentrations over time can be related to international measures or other circumstances that lowered releases into the environment. Similarities in the annual changes, as well as the time when changes began, are discussed with respect to suggested hypotheses on the fate of the investigated organochlorines. It was not possible to verify that the oxygenation of anoxic sediments mobilised old pollution in Baltic sediments. Neither was it possible to conclude that eutrophication has caused a measurable effect on the rate and timing of the decreases. Finally, long-range transport to Arctic regions seems to be due more to a one step transport than to the ‘Grass-hopper’ effect. The comprehensive database used, clearly shows how important it is to have datasets big enough to describe between-year variation before attempting to evaluate the time trend. In addition, if between-year variation is not known, it is then also difficult to evaluate spatial variation on the basis of single year observations.

  • Occupation and survival: a 25-year follow-up study of an aging population

    • OBJECTIVE: In this retrospective study of an elderly population cohort living in a defined Swedish rural area, the relationship between occupation and survival as a measure of objective health was investigated. The cohort has been followed for 25 years. METHOD: On the basis of the baseline socioeconomic interview from assessments performed when the participants were 67 years of age, an index of active participation in daily occupation was devised. The index was used to investigate the covariation between generic everyday occupation and long-time survival. RESULTS: For the female participants, Kaplan-Meier curves demonstrated differences in survival between the "less active" and "more active," and Cox regression survival analyses resulted in a significant covariance between occupation and survival. For the male participants, no such differences were found. CONCLUSION: The significant results for the women implied support for the core assumption of occupational therapy that a relationship exists between occupation and health. The lack of differences in survival among more active and less active men demonstrates the complexity of studying occupation.

  • Heparan/chondroitin/dermatan sulfate primer 2-(6-hydroxynaphthyl)-O-beta-D-xylopyranoside preferentially inhibits growth of transformed cells

    • Xylose forms the direct carbohydrate-protein link in extra- or pericellular proteoglycans (PGs) that are substituted with either chondroitin sulfate (CS)/dermatan sulfate (DS) and/or heparan sulfate (HS). Cell surface PGs carrying HS are important regulators of cell growth. Xylose coupled to an aromatic compound can enter cells and initiate either CS/DS synthesis or both HS and CS/DS synthesis, depending on the nature of the aromatic adduct. Here, we show that 2-(6-hydroxynaphthyl)-O-beta-D-xylopyranoside, which can prime both types of glycan chains, inhibits growth of a set of normal and transformed cells. Transformed cells are preferentially inhibited, and at a concentration of 0.15-0.20 mM xyloside, transformed cells are totally growth arrested, whereas normal cells are only < or = 50% inhibited. No inhibition of growth is observed with the stereoisomeric 2-(6-hydroxynaphthyl)-O-beta-L-xylopyranoside, which does not prime glycosaminoglycan synthesis at all; with the nonhydroxylated 2-naphthyl-O-beta-D-xylopyranoside, which only primes CS/DS synthesis under these conditions; or with p-nitrophenyl-O-beta-D-xylopyranoside, which is known to prime only CS/DS synthesis. We conclude that growth inhibition is due to priming of HS and/or CS/DS synthesis, which may either lead to the formation of specific antiproliferative glycans or glycan fragments or to interference with endogenous PG synthesis and turnover.

  • Development of multi-disciplinary team I-ADL assessment in community health care: an interrater reliability study of the measure of instrumental daily activity

    • This paper describes a development process concerning the active involvement of staff of different professions in developing and implementing methods for assessment of activities of daily living (ADL) in home-based geriatric rehabilitation. Although a variety of established ADL instruments exist, at the time for this study no I-ADL (Instrumental Activities of Daily Living) instrument suitable for communication among staff members of different professions was available. The specific aim was to test a new I-ADL instrument for interrater reliability. The developmental process resulting in the Measure of Instrumental Daily Activity (MIDA) is described. The instrument comprises 12 I-ADL items, defined on the basis of practical home rehabilitation experience. The study involved 36 clients with impairments, aged 65+ years. Multi-disciplinary interrater reliability was tested by 67 parallel independent assessments during a 3-month period, performed by pairs of raters of different professions. Overall agreement was very good (mean weighted kappa=0.89). The MIDA fulfils the basic requirements necessary for valid I-ADL assessment of elderly clients in community health care. An important quality is the active involvement of all staff in the assessment procedure, facilitating and stimulating the implementation of a general rehabilitative attitude in everyday practice.
