    • OBJECTIVE: Glucagon-like peptide-1 (GLP-1) has been proposed as a new treatment modality for type 2 diabetes. To circumvent the drawback of the short half-life of GLP-1, inhibitors of the GLP-1-degrading enzyme dipeptidyl peptidase IV (DPP IV) have been examined. Such inhibitors improve glucose tolerance in insulin-resistant rats and mice. In this study, we examined the 4-week effect of 1-[[[2-[(5-cyanopyridin-2-yl)amino]ethyl]amino]acetyl]-2-cyano-(S)-pyrrolidine (NVP DPP728), a selective, orally active inhibitor of DPP IV, in subjects with diet-controlled type 2 diabetes in a placebo-controlled double-blind multicenter study.RESEARCH DESIGN AND METHODS: A total of 93 patients (61 men and 32 women), aged 64 +/- 9 years (means +/- SD) and with BMI 27.3 +/- 2.7 kg/m(2), entered the study. Fasting blood glucose was 8.5 +/- 1.5 mmol/l, and HbA(1c) was 7.4 +/- 0.7%. Before and after treatment with NVP DPP728 at 100 mg x 3 (n = 31) or 150 mg x 5 (n = 32) or placebo (n = 30), subjects underwent a 24-h study with standardized meals (total 2,000 kcal).RESULTS: Compared with placebo, NVP DPP728 at 100 mg t.i.d. reduced fasting glucose by 1.0 mmol/l (mean), prandial glucose excursions by 1.2 mmol/l, and mean 24-h glucose levels by 1.0 mmol/l (all P < 0.001). Similar reductions were seen in the 150-mg b.i.d. treatment group. Mean 24-h insulin was reduced by 26 pmol/l in both groups (P = 0.017 and P = 0.023). Although not an efficacy parameter foreseen in the study protocol, HbA(1c) was reduced to 6.9 +/- 0.7% in the combined active treatment groups (P < 0.001). Laboratory safety and tolerability was good in all groups.CONCLUSIONS: We conclude that inhibition of DPP IV is a feasible approach to the treatment of type 2 diabetes in the early stage of the disease.

    • Objective: To investigate the effects of treatment with paraffin bath in patients with systemic sclerosis (scleroderma). Methods: In 17 patients with scleroderma one hand was treated daily with paraffin bath in combination with hand exercise. The other hand was treated with exercise only and was considered a control. Hand function was estimated before treatment and after 1 month of treatment, concerning hand mobility and grip force, and perceived pain, stiffness and skin elasticity. Results: At the follow-up, finger flexion and extension, thumb abduction, volar flexion in the wrist, and perceived stiffness and skin elasticity had improved significantly in the paraffin-treated hand compared with the baseline values. The improved hand function was independent of skin score and disease duration. Improvements in function were significantly greater in the hand which was treated with paraffin bath and exercise than in the hand treated with exercise only concerning extension deficit, perceived stiffness and skin elasticity. Conclusions: In this pilot study hand exercise in combination with paraffin bath seemed to improve mobility, perceived stiffness and skin elasticity. However, further studies with larger sample size are needed to attain more reliable results of the effect of paraffin bath treatment in patients with scleroderma.

  • Hand Mobility in Scleroderma (HAMIS) test: the reliability of a novel hand function test.

    • Objective Hand Mobility in Scleroderma (HAMIS) is a new hand function test developed for adults who have systemic sclerosis. HAMIS consists of 9 items designed to measure all movements assessed in an ordinary range of motion (ROM)-measured hand test. The aim of this study was to examine the reliability of the test. Methods Two observers (one occupational therapist and one physiotherapist) performed the assessments independently of each other on 30 adult subjects. There were 25 women and 5 men in the sample (mean age 53 years, average time since diagnosis 4 years). Subjects were tested twice by each rater. Results Internal consistency ranged from 0.80 to 0.85 (Cronbach's alpha). Agreement between the two observers was good for all items (estimated kappa 0.52 to 1.00). The agreement between the observers' first and second assessments was moderate to very good for most items (estimated kappa 0.48 to 1.00), but not for the assessment of supination (estimated kappa 0.25 to 0.59). Conclusion HAMIS is a reliable instrument for evaluation of hand function on scleroderma patients.

    • Goran Sonnevi's poetry represents a vast network of associations to other texts and other works of art. Consequently, this poetry often initiates a conversation on the present with partners from different times, Places and arts. "Burge, Oja; 1989" is an interesting example of such an intermedial conversation, set in the middle of a dramatic period of our time, the year 1989. The central theme is the problem of change. The scene is the island of Gotland, where old-fashioned houses and tools speak of the impact of the past. The speaker reflects on the political changes on the other side of the Baltic Sea, with references to artefacts in the landscape and in our cultural heritage. These intertextual and intermedial reflections function as a demonstration of an ethos: an openness to the Other, a willingness to respond to the world and to communicate with other voices in a boundless reciprocity, mediated by artefacts.
