  • Dissemination of research results over the internet - a web-based anthology of Cardiovascular care

    • Background: The widespread use of web-based resources has dramatically increased access to scientifically based information of use to both health professionals and the general public. Scientific results are, however, often difficult for people outside the scientific community to understand. Moreover, access to large amounts of information of various kinds often places insurmountable demands on the readers to understand, assess and select what is relevant and trustworthy. The Swedish research institute, Va° rdal Institute took the initiative to set up a virtual room with the theme of cardiovascular care in Sweden, as part of a larger project encompassing nine virtual rooms with diverse health care themes. Methods: The aim of the project is to give an overview of the virtual thematic room and issues related to the dissemination of knowledge to patients, relatives, students, administrators and staff involved in cardiovascular care. Researchers and specialists within the heart field were invited to publish themselves on the website, more specifically in the space designated Thematic Room Heart. Results: Popular science articles are published in close cooperation between an editing committee and researchers, written by experts for the general public. The virtual room presents reviews of recent research organized into the following themes: Coronary artery disease, arrhythmia, valve failure, heart failure, health economics in heart disease, nutrition, physical activity and exercise, nutrition, geriatric and gender perspectives, outpatient clinics run by nurses e hypertension, heart failure, coronary artery prevention and pacemaker. Sixteen of these are currently available for download and there are more being published all the time. In addition to these articles, the room contains news bulletin, information from patient organization and reference lists for text books, dissertations, guidelines, legal issues and fiction. Fields of knowledge about prevention and from acute care to everyday life situations are presented. Health care teams, patients, their families and the public can easily access and use this information on this website. A syllabus and texts connected with each theme may be downloaded for use in local study circles. The idea is to stimulate the participants’ interest in learning in different contexts. The “current profile” and “good example” presentations are constantly updated. As this particular website is part of a larger virtual project, links and references to related issues such as “Living with chronic pain” and “Nutrition and eating” can easily be created. Conclusion: The virtual thematic room is a flexible, web-based anthology of interest and relevance to a wide range of readers interested in cardiovascular care. Professionals use the website material regularly for classes and informing patients. The general population uses it as well on a daily basis. It is hoped that an additional interactive forum to determine how the information is understood and used would provide important feedback on how the virtual Thematic Room Heart may be further improved.

  • Patients with worsening chronic heart failure require hospital care - listen to the patient

    • Background: Chronic heart failure (CHF) is a major public health problem characterised by progressive deterioration with disabling symptoms and frequent hospital admissions. To influence hospitalisation rates it is crucial to identify precipitating factors. Aim: To characterise patients with CHF that seek an emergency department (ED) because of their deteriorating condition and to explore the reasons why they are admitted to hospital. Method: Patients (n = 2648) seeking care for dyspnoea were identified at the ED. Of these 2648 patients, 1127 had a previous diagnosis of CHF, and of these, 785 were included in the present study with at least one sign and one symptom of worsening CHF. Results: Only 2% of the patients could be sent home directly from the ED. The remaining patients were admitted to hospital because of serious conditions, including pneumonia/respiratory disease, myocardial infarction, pulmonary oedema, anaemia, the need to monitor cardiac rhythm, pathological blood chemistry and difficulties to communicate. Conclusion: The vast majority of patients with worsening CHF seeking the ED required hospital care, predominantly because of comorbidities. Patients with severe chronic illness like CHF with symptomatic deterioration could be attended with focus on symptom relieve and without additional emergency room investigations before hospital admission.

  • Decreasing trends in the incidence of heart failure after acute myocardial infarction from 1993-2004: a study of 175,216 patients with a first acute myocardial infarction in Sweden

    • Aims To investigate temporal trends in the risk of heart failure (HF) complicating acute myocardial infarction (AMI) and to determine whether these trends differ by gender or age. Methods and results The national Swedish hospital discharge and death registries from 1993 to 2004 were used to calculate age- and gender-specific trends for a first episode of HF within 3 years in 175 216 patients aged 35–84 and hospitalized with a first AMI. Overall, 14.4% of patients aged 35–64 and 31.5% of those aged 65–84 with AMI in 1993–1995 had a hospital diagnosis of HF within 3 years (including the index admission). Corresponding figures for patients with AMI from 2002 to 2004 were 11.5 and 28.0%, respectively. In multivariable analyses, the risk of HF decreased by 4% per year. Having had a stroke before admission increased HF risk by 37%, diabetes increased the risk by 76% and atrial fibrillation by 80%. Patients with any kind of valvular disease had a more than doubled risk. Women had a 6% higher incidence of HF than men, whereas men with an index admission for AMI who did not develop HF had higher mortality than women. Conclusions In this national sample, we observed a steady decrease in the risk of being hospitalized with HF after an AMI. However, the 3-year risk of HF remains high, with nearly one-third of AMI patients aged 65–84 developing HF within 3 years.
