  • Socioeconomic characteristics, housing conditions and criminal offences among women with cervical neoplasia

    • Objective. To investigate the association between cervical neoplasia and socioeconomic factors, housing conditions and criminal offences. Design. Longitudinal observational study. Setting. Falun county hospital, Sweden. Population. A total of 1331 women diagnosed with cervical intraepithelial neoplasia I-III or cervical cancer between 1967 and 1978 were compared with 2604 age-matched controls from the same geographical area in Sweden. Methods. The Population and Housing Censuses were used for information about civil status, education, housing conditions, employment and socioeconomic status. The Swedish Register of Conviction Decisions was used to access information on criminal offences. Main outcome measures. Socioeconomic status, housing conditions, criminal offences. Results. Women with cervical neoplasia had a lower socioeconomic status and a lower educational level than their age-matched controls. They were more often divorced and did not own their home as often as controls. A significant association with criminal offences was observed, and it persisted after adjustment for socioeconomic status. Differences in socioeconomic factors between women with cervical neoplasia and their controls had not diminished in the younger, compared with the older, part of the study population. Conclusions. The results indicate that women with cervical neoplasia belong to a socioeconomically disadvantaged group. Furthermore, the study provides information about an association with criminal offences.

  • Reference values for F wave parameters in healthy 3-20 year old subjects

    • Objective: To create a reference value database for F wave parameters from healthy subjects aged 3-20 yr. Methods: We studied the following parameters: minimum F wave latency minus distal motor latency (FMINLAT), number of F waves/20 stimuli (FNUMBER) and F wave dispersion (FDISP). The median, ulnar, peroneal and tibial nerves were studied. All four nerves were not analyzed in all subjects, the number of subjects varied from 78 to 118 in each nerve. Results: Age explained 71-87% of the variability of FLATMIN while height explained 80-95% of the variability. The FMINLAT increases by 0.12 ms/cm of height in the upper limb nerves and by 0.28 ms/cm in the lower limb nerves. Gender did not influence the FMINLAT. FDISP was not related with age, height or gender. FNUMBER was not related with age or height, it was somewhat larger in males than females but the difference was not significant in all nerves. Conclusions: The best model for FMINLAT was a linear regression model with height as an independent variable. FDISP and FNUMBER are not related to age, height or gender between the ages of 3 and 20 yr. Significance: We have constructed clinically useful reference values for F wave parameters in healthy subjects aged 3-20 yr for the main motor nerves commonly studied.

  • Evaluation of prevalent and incident ovarian cancer co-morbidity

    • BACKGROUND: The peak in incidence of ovarian cancer occurs around 65 years and concurrent increasing risk by age for a number of diseases strongly influence treatment and prognosis. The aim was to explore prevalence and incidence of co-morbidity in ovarian cancer patients compared with the general population.METHODS: The study population was patients with ovarian cancer in Sweden 1993-2006 (n = 11 139) and five controls per case (n = 55 687). Co-morbidity from 1987 to 2006 was obtained from the Swedish Patient Register. Prevalent data were analysed with logistic regression and incident data with Cox proportional hazards models.RESULTS: Women developing ovarian cancer did not have higher overall morbidity than other women earlier than 3 months preceding cancer diagnosis. However, at time of diagnosis 11 of 13 prevalent diagnosis groups were more common among ovarian cancer patients compared with controls. The incidence of many common diagnoses was increased several years following the ovarian cancer and the most common diagnoses during the follow-up period were thromboembolism, haematologic and gastrointestinal complications.CONCLUSION: Women developing ovarian cancer do not have higher overall morbidity the years preceding cancer diagnosis. The incidence of many common diagnoses was increased several years following the ovarian cancer. It is crucial to consider time between co-morbidity and cancer diagnosis to understand and interpret associations.

    • Background Childhood brain tumors are associated with high mortality and morbidity but little is known about its causes About half of women use medicines when pregnant and some of the medicines commonly used might be carcinogenic Objective The aim with this population-based case-control study was to analyze associations between specific groups of medicines taken during pregnancy and the risk of brain tumor in the offspring Methods All children. up to 15 years of age, born in Sweden between 1975 and 1984 were eligible for the study. Cases (N = 512) were children diagnosed with brain tumor and controls (N = 525) were randomly selected from the Medical Birth Register Exposure data on medicines was extracted blindly from antenatal medical records and grouped according to Anatomical Therapeutic Chemical van code Information on maternal reproductive history was received from the Medical Birth Register We used logistic regression to estimate associations between fetal exposure to medicines and childhood brain tumor Results No significant changes in risk were noted after exposure to iron supplementation, antiemetics, analgesics, antibiotics or any other main ATC group A tendency of protective effect was seen for prenatal exposure to folic acid (adjusted OR 0 6, 95% Cl 03-1.1). Ten children with a diagnosis of brain tumor had been exposed to beta-blocking agents in fetal life as compared to two children without brain tumor (adjusted OR 5 3.95% Cl 1.2-24 8) Conclusions In this case-control study, an increased risk of brain tumor was seen in children exposed to beta-blocking agents during fetal life However, due to the low number of exposed the interpretation of this finding should be made with caution.

  • The influence of comorbidity on mortality in ovarian cancer patients

    • Background. Ovarian cancer is a severe disease with a peak incidence in the older age groups where concurrent morbidity is common and could potentially influence mortality rates. Objectives. The aim was to study the influence of common comorbidity diagnoses on mortality in ovarian cancer patients. Methods. The study population was patients with ovarian cancer in Sweden 1993-2006 (n = 11.139) identified in the national Cancer Register. Comorbidity data was obtained from the Patient Register and mortality from Cause of Death Register. Mortality was analyzed with Cox' proportional hazards models and subgroup analyses were performed by age and tumor histology. Results. Almost all of the assessed comprbidities increased mortality in ovarian cancer patients. Thromboembolism was the most hazardous comorbidity (HR = 1.95, <1 year after cancer diagnosis and HR = 7.83, 1-5 years after cancer diagnosis) followed by hematologic complications (HR = 1.84 and 7.11 respectively) and infectious disease (HR = 1.48 and 5.28 respectively). The occurrence of diabetes mellitus and hypertension had less impact on mortality. Conclusion. Thromboembolism, hematologic complications and infections had a pronounced effect on mortality rates in women with ovarian cancer. The impact of comorbidity was mainly apparent among those with a more prosperous prognosis, such as longer time since cancer diagnosis, less aggressive tumors and younger age. (C) 2014 Elsevier Inc.
