  • Dental implants from the patients perspective : transition from tooth loss, through amputation to implants – negative and positive trajectories

    • Aim The aim of this study was to explore patients' expectations on and experiences from dental implant treatment through deep-interview technique. Material & Methods A qualitative study design was chosen and 17 patients were interviewed by open-ended questions. All patients in the study had a previous history of periodontal disease with, in most cases, many years of treatment. The interviews were transcribed; a coding process was used according to qualitative conventional content analysis. Results In the analysis, a core category was identified as “Transition from tooth loss, to ‘Amputation’, and to implants – negative and positive trajectories”. When the patients faced the fact that it was not possible to keep the teeth any longer, a period of fear, shame and denial, which also affected their social life negatively followed. After they received their implants and the chewing ability and appearance became better, it also improved their quality of life. Conclusion Treatment with dental implants improved function, enhanced self-esteem, social life and, thus quality of life. In clinical practice, information about dental implants and motivational strategies are needed during the period before getting dental implants. Follow-up is important thereafter, capturing both the pros and cons with implants.

    • Findings from international research as well as fromSwedenshow that food distribution as a single inter­vention involves various professional groups on the various levels in organisation. The present research is one of several Action Research projects initiated as a result of a resolution on the highest political level aiming at achieving improvements within the care and service through collaboration between different levels in the organisations. The overall aim of this specific study is to describe the “top-down” perspective i.e. decision-makers involvement in identifying areas for the development of the food distribution chain. Data was collected in two steps, firstly by individual interviews with decision makers and secondly by group discussions. The group discussions were based on findings from individual interviews and results from two previous studies focused on interviews with various professionals and food distribution recipients. The findings were that food distribution was seen as a multifaceted system that included three interrelated issues: the administration of work, the professional’s competence in nutrition and the relationship between various professional groups. To achieve a change, the central component of complexity needs to be clarified. When the issues are multifaceted the solutions need to regard the whole picture rather than its parts. Keywords: action research, decision-makers, food distribution in Swedish context, “top-down” perspective The short paper and presentation would fit in the key theme Health Care Management of the congress.

  • Food distribution within a municipality from different perspectives – older people, decision makers and professionals : an action research study

    • The present research project is one of Action Research (AR) projects in a region in southern part of Sweden, Scania. All the AR projects originate as result of a decision on the highest political level. The decision was to do changes within the care service where collaboration between different care organisations should be advocated in aim to support and help care consumers. The care should be provided in more care consumer centred ways in which care consumers should be in the centre. These changes should emphasise public and care users’ involvement and participation in decisions around the treatment, service development and evaluation. The participatory service should be performed through a constructive dialogue between care users, health care professionals as a catalyst to common practice change.  The overall aim of the project was to explore FD to older people living alone in their own  homes from different perspectives: elderly as consumers, decision-makers and various professionals.  The specific aims were: 1)to explore various professionals’ experiences in relation to their involvement in food distribution in order to get a comprehensive understanding of the organisation, responsibilities and roles in the specific context (“bottom-up” perspective);2) to explore older people’s experiences of food distribution and their suggestions what they whish to change (“bottom-up” perspective); 3) to identify, suggest, implement and evaluate chosen actions issues together with various professionals and elderly people (“bottom-up” perspective); 4) the results from the studies 1-3 will be given as feedback to the decision-makers level and it will be start point for reflective reasoning by using Story Dialogue Method in aim to identify conditions for development of food distribution chain (“top-down” perspective). The short paper and presentation, on how to identify the practical issues involved and what can be done to create a receptive context for change, would fit in the fourth key theme of the congress.

    • The study was performed in accordance with action research approach and included four phases: look, think, act and evaluate.Look phase was performed in the three steps:To gain insight into and describe the experiences of elderly home-living people who receive meals from municipalityTo explore various professional’s experiences of involvement in the food distribution process in order to get a comprehensive understanding of the organization and responsibilities and rolesTo describe decision makers views on the food distribution serviceThink phase was performed in the three steps:To reflect on what was important for the elderly people relating to their receiving meals distributed by the municipalityTo reflect on what the various involved professionals identify as most important to develop in the food distribution chainTo reflect over suggested developmental areas and choose one of them and reflect over how to perform action and evaluationAct phase was performed in two steps:The knowledge in nutrition as developmental area was chosen to focus action onThe way how to develop nutrition knowledge, the information letters,  was chosenThe information letters were developed I collaboration with professionals and elderly peopleThe 1700 information letters were distributedThe evaluation phase was performed in three phases:The preliminary outcomes were given as feedback to involved representatives from various professionalsThe finally outcomes were given as feedback to decision makersFinal feedback resulted in three future actionsOutcomes from look and think phase are: The food distribution receivers expressed feelings of: to not have influence over food products the meals were made from, to feel isolated and confined at home due to difficulties getting out of the house and expressed sincere gratitude for the meal service from the municipality.The various professionals expressed that: food distribution is a fragmentary intervention where comprehensive perspective and roles of responsibility are lacking,  the fragmented organization seemed strictly divided and limited by constrains regarding time and money, the necessary observations and structured following up of nutrition status for food distribution receivers is missing,  and that knowledge in nutrition need to be continuously developed.The decision makers found areas to require facilitation for change related to food distribution: the monitoring of the food distribution receiver’s health and wellbeing, to focus on involved professional’s competence in nutrition and to create a forum for inter-professional communication.

  • Malnutrition prevalence and precision in nutritional care : an intervention study in one teaching hospital in Iceland

    • Aim. The aim of this study was to explore the point prevalence of malnutrition and the targeting of nutritional interventions in relation to undernutrition risk before and after an intervention. Background. Malnutrition risk and the precision in targeting nutritional treatment are indicators of quality of care. Knowledge regarding the in-hospital prevalence of malnutrition and nutritional treatment is meagre for Iceland. Design. Pre- and postintervention study. Methods. The study was performed during one day in 2006 (March) and one day in 2007 (April). In total, 95 (89%) and 92 (88%) patients agreed to participate. Moderate/high undernutrition risk was defined as the occurrence of at least two of the following: involuntary weight loss, body mass index below limit and eating difficulties according to Minimal Eating Observation Form - Version II. Being overweight was graded based on body mass index. Specific nutritional care actions were recorded. Intervention: A five-point programme for nutrition and eating was implemented. Results. Moderate/high risk for undernutrition was found in 25 and 17% in the two years (ns, not significant). A high body mass index was found in 53 and 54% (ns). The number of patients with a documented body mass index significantly increased between the two surveys (1 and 30%, p-value < 0 center dot 0005). The use of oral supplements increased from 11-40% (p < 0 center dot 0005) and especially among those at no/low undernutrition risk, with ingestion or deglutition difficulties (p < 0 center dot 0005 in both cases) but not among those with appetite and energy problems (ns). Conclusion. Implementing a nutritional programme does not necessarily affect the number of in-patients with malnutrition, but it is likely to increase the precision of nutritional care to some extent. Relevance to clinical practice. Greater efforts need to be taken to increase the precision of nutritional care among patients at moderate/high undernutrition risk and among those with appetite and energy problems.

  • A structured evidence-based approach to decrease cardiovascular complications among pregnant women with reumatic disease : a pilot randomized controlled trial

    • Background: Recent reports from the developing world have documented rheumatic fever (RE) incidence rates as high as 206/100 000 and Rheumatic Heart Disease (RHD) prevalence rates as high as 18.6/1000. The high frequency of RHD in the developing world necessitates aggressive prevention and control measures. Aim: The aim of the study was to examine the effect of intensive antenatal follow up program on decreasing cardiovascular complications among rheumatic pregnant woman. Design: Randomized controlled trial. Setting: This study was conducted at El Kasr-EL Aini, maternity hospital- Cairo- University Hospitals. Sample: A total of 100 rheumatic pregnant women that attended the antenatal clinic at El Kasr-EL Aini, maternity hospital who could read and write, had functional class I and II cardiac disease, primigravida, nullipara with single fetus were recruited to this study and assigned randomly into two groups. Procedure: Women who met the inclusion criteria were interviewed and baseline assessment was done. The intervention was an intensive antenatal follow up program for decreasing cardiovascular complications among rheumatic pregnant woman. Patients in the study group (SG) were offered an intervention at three levels, basic, intermediate and comprehensive. Evaluation of the program was carried out in term of assessment of maternal and neonatal outcome. Results: There were several significant differences between the study group (SG) and control group (CG). In the SG, antepartum complications was less common, gestational age at birth was higher, postnatal hospital stay shorter, birth weight higher, more had vaginal deliveries with episiotomy, fewer had caesarean section, forceps deliveries and termination of pregnancy, and cardiac class was better at follow up as compared to the CG. Conclusion: The difficult issues in pregnancy complications by cardiac disease are best managed through a team approach. Establishing a pre-pregnancy plan will foster a better therapeutic relationship between all parties and allow for better understanding of the patient’s beliefs regarding pregnancy, her disease and attitude towards medications.
