The Negotiation of Urgency: Economies of Attention in an Italian Emergency Room

- Urgency in a hospital Emergency Room (ER) is not a self-evident state. Urgency is made, by establishing priorities, distributing attention and material resources, and deciding who and what needs to be attended to first – and, simultaneously, who and what has to wait. The process of determining urgency is known as “triage” (from the French verb, trier, “to choose”). This thesis is about the vicissitudes of triage in an Italian ER. Based on one year of ethnographic fieldwork, the thesis explores what happens when urgency is at stake; when it is contested and caught up between different, and frequently conflicting, perspectives. It explores how urgency is determined in practice, and shows how triage always is a vulnerable process of negotiation guided by economies of attention. How is urgency actually shaped in interactions between patients, their families and friends, and the ER staff? The different chapters explore how time in the ER is created through shifting registers of attention, and how attention in the ER is affected by widespread economic and social precarity, and neoliberal national policies of governance. It discusses how triage increasingly is structured by attitudes of mistrust; and also by potential or real outbreaks of violence. Addressing the particular positioning of the ER as a thick space of conjunction between neoliberal state politics and people's increasing need for care and recognition, the thesis aims to contribute to medical anthropology literature by analyzing triage not as a neutral medical way of sorting, but as a practice that actively creates difference. It explores both the limits of triage, and how those limits can spark improvisation and creative reinvention.

- Mistrust is increasingly a daily reality of healthcare delivery worldwide. Yet it remains understudied as a form of relationship and a force in its own right. I address this gap through the ethnography of an Italian Emergency Department (ED), where conflicts have increased since the 2008 financial crisis. I show how mistrust does not result in a breakdown of healthcare interactions. Rather, mistrust is used in ambivalent care relationships to negotiate the roles, the risks, and the power that patients and staff are willing to entrust to others. Mistrust manifests in risk management strategies within relationships of “mistrustful dependency.”.

Därför blir det allt viktigare att ta tillvara på den kunskap som skapas inom dessa verksamheter och att möjliggöra ett utbyte av erfarenheter. "Kultur och hälsa i prakten" presenterar i fjorton kapitel hur man kan arbeta med kultur och hälsa inom olika verksamheter som exempel på gränsöverskridande samarbeten mellan praktik och forskning. Kapitalförfattarna är både förankrade i ett konkret verksamhetsområde och i akademisk forskning. "Kultur och hälsa i praktiken" ger en beskrivning av praktiska verksamheter utifrån aktuell forskning. Den syftar till inspiration såväl som fördjupad kunskap för praktiker, forskare, beslutsfattare, kulturutövare, journalister och hälso- och sjukvårdspersonal.

- While violence against health care workers is being progressively recognised as a serious problem in the healthcare industry, it remains an under-studied area of enquiry in global public health. Anthropologists have long observed that violence toward patients is tied to institutional care practises in multiple ways, including repression, misrecognition and silencing. But research on health care staff’s experience of violence is still lacking. This article aims to address this literature gap by providing research on the daily experience of vulnerability to violence that health care providers face during their work. To do so, the paper ethnographically explores the effects and perception of violence against health care workers in an emergency department (ED) in northern Italy, a place with a dramatic escalation of violent incidents. The article illustrates how the ED staff attended to the experience of suffering of potentially violent patients. In so doing, ED professionals shifted the responsibility of violence against them from violent individuals to violent structures shaping health inequities. The paper thus argues that ED professionals display a structural competence perspective when dealing with violence.

- This book advances critical discussions about what coloniality, decoloniality and decolonization mean and imply in the Nordic region. It brings together analysis of complex realities from the perspectives of the Nordic peoples, a region that are often overlooked in current research, and explores the processes of decolonization that are taking place in this region. The book offers a variety of perspectives that engage with issues such as Islamic feminism and the progressive left; racialization and agency among Muslim youths; indigenizing distance language education for Sami; extractivism and resistance among the Sami; the Nordic international development endeavour through education; Swedish TV-reporting on Venezuela; creolizing subjectivities across Roma and non-Roma worlds and hierarchies; and the whitewashing and sanitization of decoloniality in the Nordic region. As such, this book extends much of the productive dialogue that has recently occurred internationally in decolonial thinking but also in the areas of critical race theory, whiteness studies, and postcolonial studies to concrete and critical problems in the Nordic region. This should make the book of considerable interest to scholars of history of ideas, anthropology, sociology, cultural studies, postcolonial studies, international development studies, legal sociology and (intercultural) philosophy with an interest in coloniality and decolonial social change.

- Sweden is rapidly becoming an increasingly multicultural and digitalized society. Encounters between pediatric nurses and migrant mothers, who are often primary caregivers, are impeded by language problems and cultural differences. To support mothers, doulas, who are women having the same linguistic and cultural backgrounds, serve as cultural bridges in interactions with health care professionals. In addition, information and communication technology (ICT) can potentially be used to manage interactions owing to its accessibility.The objective of this study was to investigate the role of ICT in managing communicative challenges related to language problems and cultural differences in encounters with migrant mothers from the perspectives of Swedish pediatric nurses and doulas.Deep semistructured interviews with five pediatric nurses and four doulas from a migrant-dense urban area in western Sweden were audio recorded, transcribed, and analyzed using thematic content analysis.The results showed that ICT contributes to mitigating communicative challenges in interactions by providing opportunities for nurses and migrant mothers to receive distance interpreting via telephones and to themselves interpret using language translation apps. Using images and films from the internet is especially beneficial while discussing complex and culturally sensitive issues to complement or substitute verbal messages. These findings suggest that ICT helps enable migrant mothers to play a more active role in interactions with health care professionals. This has important implications for their involvement in other areas, such as child care, language learning, and integration in Sweden.The findings of this study suggest that ICT can be a bridging tool between health care professionals and migrants. The advantages and disadvantages of translation tools should be discussed to ensure that quality communication occurs in health care interactions and that health information is accessible. This study also suggests the development of targeted multimodal digital support, including pictorial and video resources, for pediatric care services.

Passive and active suicidal ideation in Swedish 85-year-olds: Time trends 1986-2015

- Abstract Background Older adults have high suicide rates. We investigated potential time trends in the prevalence of passive and active suicidal ideation in 85-year-olds. Further, we examined factors associated with such ideation in this age group. Methods Population-based samples of 85-year-olds were interviewed in 1986 (N=347), 2008 (N=426) and 2015 (N=320). Past-month passive/active suicidal ideation was evaluated with the Paykel questions. Results Reporting any type of passive or active suicidal ideation was less common in 2008 (7.3%, p<0.001) and 2015 (7.2%, p<0.001) compared to 1986 (16.4%). The change was driven by decreases in passive ideation. Passive/active suicidal ideation was associated with higher MADRS score (OR: 1.2, 95% CI: 1.1-1.2, p<0.001), institution residence (OR: 3.9, 95% CI: 1.7-8.9, p=0.001) and feelings of loneliness (OR: 2.7, 95% CI: 1.4-5.2, p=0.003). When stratified by sex, it was associated with institution residence (OR: 3.7, 95% CI: 1.4-9.9, p=0.008) and feelings of loneliness (OR: 3.0, 95% CI: 1.4-6.3, p=0.005) in women. In men, we observed a tenfold higher risk in those without partners (OR: 9.8, 95% CI: 2.9-33.5, p<0.001). Limitations While differential three-year mortality was not observed in 1986, mortality was higher among non-participants in 2008 and 2015. This might have inflated cohort differences in passive/active suicidal ideation. Conclusion An initial decrease in the prevalence of passive/active suicidal ideation in 85-year-olds was observed but this positive trend did not persist. Results underline that preventive strategies targeting loneliness and focusing on institutional settings are needed, as are interventions for men without partners.

- IntroductionModern sports equipment is nowadays manufactured industrially according to globally accepted and standardized models, but traditionally tools for play and games were prepared from materials found in the local environment. The objective of this article is to investigate various aspects of Sami local knowledge about organisms used for their material culture of traditional sports and games in northern Fennoscandia (Sapmi). What functions did the surrounding biota have in the production of equipment used in sports and games?MethodsA qualitative method was used; the ethnographic literature and travel narratives have been analyzed particularly for descriptions and notes on traditional games, toys, and sports.ResultsBefore the turn of the twentieth century, bats, balls, and skis were seldom produced in factories, but by children and adults who utilized available materials from the surrounding environment. The manufacture of tools for play and games was characterized by a rich creativity in the use of various biological and natural resources. A wide range of such resources is presented in this article, among them the bracket fungus Fomitopsis betulina, used for making balls, reindeer antlers utilized for lassoing contests, and pine bark painted with reindeer blood, prepared for playing cards. We also highlight how tools usually associated with means of transport could switch functions and serve playful and competitive purposes, such as skis made of compression pine or walking sticks of birch: The former were used in skiing races, and the latter appeared in fencing competitions.ConclusionThe industrialization of the material culture of sports has been contributed to a loss of local knowledge and familiarity with locally available organic stuffs for producing equipment for play and games. By reconnecting with previous knowledge of traditional games, we discover a potentially new direction for modern sports and games, shifting from globalization to environmentalization. Such an environmentalization could permit the local environmental context define the content, meaning and structure of sports, and simultaneously enrich both sports and outdoor life.
