  • An upper body garment with integrated sensors for people with neurological disorders – early development and evaluation

    • Background: To develop a novel wearable garment with integrated sensors for continuous monitoring of physiological and movement related variables to evaluate progression, tailor treatments and improve diagnosis in epilepsy, Parkinson’s disease and stroke. Methods: An iterative development process and evaluation of an upper body garment with integrated sensors included: identification of user needs, specification of technical and garment requirements, garment development and production as well as evaluation of garment design, functionality and usability. The project is a multidisciplinary collaboration with experts from medical, engineering, textile, and material science within the wearITmed consortium. The work was organized in regular meetings, task groups and hands-on workshops. User needs were identified using results from a mixed-methods systematic review, a focus group study and expert groups. Usability was evaluated in 19 individuals (13 controls, 6 patients with Parkinson’s disease) using semi-structured interviews and qualitative content analysis. Results: A prototype designed to monitor movements and heart rate was developed. The garment was well accepted by the users regarding design and comfort, although the users were cautious about the technology and suggested improvements. All electronic components passed a washability test. The most robust data was obtained from accelerometer and gyroscope sensors while the electrodes for heart rate registration were sensitive to motion. artefacts. The algorithm development within the wearITmed consortium has shown promising results. Conclusions: The prototype was accepted by the users. Technical improvements are needed, but preliminary data indicate that the garment has potential to be used as a tool for diagnosis and treatment selection and could provide added value for monitoring seizures in epilepsy, fluctuations in PD and activity levels in stroke. Future work aims to improve the prototype further, develop algorithms, and evaluate the functionality and usability in targeted patient groups. The potential of incorporating blood pressure and heart-rate variability monitoring will also be explored.

  • Sleepy driving on the real road and in the simulator - A comparison

    • Sleepiness has been identified as one of the most important factors contributing to road crashes. However, almost all work on the detailed changes in behavior and physiology leading up to sleep related crashes has been carried out in driving simulators. It is not clear, however, to what extent simulator results can be generalized to real driving. This study compared real driving with driving in a high fidelity, moving base, driving simulator with respect to driving performance, sleep related physiology (using electroencephalography and electrooculography) and subjective sleepiness during night and day driving for 10 participants. The real road was emulated in the simulator. The results show that the simulator was associated with higher levels of subjective and physiological sleepiness than real driving. However, both for real and simulated driving, the response to night driving appears to be rather similar for subjective sleepiness and sleep physiology. Lateral variability was more responsive to night driving in the simulator, while real driving at night involved a movement to the left in the lane and a reduction of speed, both of which effects were absent in the simulator. It was concluded that the relative validity of simulators is acceptable for many variables, but that in absolute terms simulators cause higher sleepiness levels than real driving. Thus, generalizations from simulators to real driving must be made with great caution.

  • Rollator related pedestrian single accidents and collision events in Sweden

    • Rollators (four-wheel walkers in USA; zimmer frames in UK) are commonly used as mobility aids for the elderly and people with a variety of disabilities. Pedestrian rollator users are a sub-group of Vulnerable Road Users (VRUs), although this group is rarely recognised in traffic safety contexts. The aim of this study is to extract and analyse rollator related pedestrian accident and injury data in Sweden. The results will provide valuable insight into the risks and obstacles rollator users are exposed to in the traffic environment and may in the long term contribute to improving the mobility of this group. The current study is based on data from 2,020 accidents involving 2,305 persons extracted from the Swedish Traffic Accident Data Acquisition (STRADA) database. For consistency reasons, a subset of data (N = 745) was analysed in order to investigate the development of accidents over a period of 10 years. Thereafter, each accident in the whole data set was registered as either single or collision. The results show that the number of rollator accidents in Sweden increased by approximately 80% during 2007–2016. Females dominate the injury statistics of single accidents, collisions, as well as fatal outcome, which may be due to exposure and/or differences in physical characteristics. Single accidents are much more common than collisions (n=1,668 and n=352, respectively) and the injury consequences are at least as serious. Data from the present study have revealed that the frequency of minor injuries (ISS 1–3) is 4.3 times, moderate (ISS 4–8) 6.0 times and serious injuries (ISS 9–15) 8.9 times higher in single accidents than in collisions. Fatal injuries are, however, more common in collision events (33 in comparison to 8). The vast majority of single accidents (99%) was due to falls. Many of the single accidents (29%) were caused by ground level differences (typically a curb) or due to surface conditions (19%). Collisions involved cars, trucks or buses in 91% of cases, occurring predominantly in car parks (31%) and on (zebra) crossings (30%). In 54% of cases the vehicles reversed into the rollator. Abbreviated Injury Scale (AIS) 3+ injuries were dominated by head (36%) and torso (33%) injuries in collision events, and hip fractures (71%) in single accidents. The present study shows that further research into rollator user related accidents, both single accidents and collision events, is required. In order to introduce appropriate measures, future work should follow up on accident and injury developments and further improve the quality of mobility aid related accident data in general. Improved stability and design, proper training programmes, effective maintenance services, development of a supporting infrastructure would contribute to increased safety for rollator users.

    • High-Performance Marine Craft (HPMC) occupants are currently being investigated for various psychophysical impairments degrading work performance postulating that these deteriorations are related to their occupational exposures. However, scientific evidence for this is lacking and the association of exposure conditions aboard HPMC with adverse health and performance effects is unknown. Therefore, the study estimates the prevalence of musculoskeletal pain (MSP) among HPMC occupants and the association of their work exposure with MSP and performance degradation. It also presents a criterion for evaluating the self-reported exposure severity aboard three different types of mono-hull HPMC; displacement, semi-displacement and planing, on a par with the available standard criteria for objectively measurable exposures. Furthermore, another criterion is proposed to assess the performance-degradation of HPMC occupants based on self-reported fatigue symptoms and MSP. Swedish Coast Guard HPMC occupants were surveyed for MSP, fatigue symptoms as well as for work-related and individual risk indicators using a validated web-based questionnaire. Prevalence of MSP and performance-degradation during the past 12 months were assessed and presented as a percentage of the sample. Associations of exposure conditions aboard HPMC with MSP and performance-capacity were systematically evaluated using multiple logistic regression models and expressed as odds ratio (OR). Prevalence of MSP was 72% among which lower back pain was the most prevalent (46%) followed by neck pain (29%) and shoulder pain (23%) while 29% with degraded performance. Exposure to severe conditions aboard semi-displacement craft was associated with lower back (OR = 2.3) and shoulder (OR = 2.6) pain while severe conditions aboard planing craft with neck pain (OR = 2.3) and performance-degradation (OR = 2.6). MSP is common among Swedish coast guards. Severe exposure conditions aboard HPMC are significantly associated with both MSP and performance-degradation. The spine and shoulders are the most susceptible to work-related MSP among HPMC occupants which should be targeted in work-related preventive and corrective measures.

    • The design of prosthetic controllers by means of neurophysiological signals still poses a crucial challenge to bioengineers. State of the art of electromyographic (EMG) continuous pattern recognition controllers rely on the questionable assumption that repeated muscular contractions produce repeatable patterns of steady-state EMG signals. Conversely, we propose an algorithm that decodes wrist and hand movements by processing the signals that immediately follow the onset of contraction (i.e., the \textit {transient} EMG). We collected EMG data from the forearms of 14 non-amputee and 5 transradial amputee participants while they performed wrist flexion/extension, pronation/supination, and four hand grasps (power, lateral, bi-digital, open). We firstly identified the combination of wrist and hand movements that yielded the best control performance for the same participant (intra-subject classification). Then, we assessed the ability of our algorithm to classify participant data that were not included in the training set (cross-subject classification). Our controller achieved a median accuracy of 96% with non-amputees, while it achieved heterogeneous outcomes with amputees, with a median accuracy of 89%. Importantly, for each amputee, it produced at least one \textit {acceptable} combination of wrist-hand movements (i.e., with accuracy >85%). Regarding the cross-subject classifier, while our algorithm obtained promising results with non-amputees (accuracy up to 80%), they were not as good with amputees (accuracy up to 35%), possibly suggesting further assessments with domain-adaptation strategies. In general, our offline outcomes, together with a preliminary online assessment, support the hypothesis that the transient EMG decoding could represent a viable pattern recognition strategy, encouraging further online assessments.

  • Pre-Crash and In-Crash Car Occupant Safety Assessment

    • Tens of millions are annually injured in Road Traffic Accidents (RTAs) worldwide, while the estimated number of RTA fatalities amounted to 1.35 million in 2016. In Europe, car occupants hold the largest share (48%) of fatalities among all road users. The high fatality and injury numbers motivate the work of enhancing road traffic safety. A holistic safety assessment approach, considering both the pre- and the in-crash phase of a crash, has the potential to enhance real-world occupant protection evaluation, as well as facilitate the development of effective countermeasures. In standardized car occupant safety assessments, occupant surrogates of standardized anthropometries are employed in standardized postures, with the seat adjusted to a single predefined position. The vehicle is then subjected to predefined crash configurations with meticulously described impact points and angles. In contrast, real-world traffic crashes involve occupants of different shapes and sizes, who adjust the position of the seat and their posture on the seat differently, and the vehicles are subjected to diverse crash configurations (multiple impact locations, impact directions, and speed combinations). The overall aim of this thesis is to develop and apply methods, spanning from the pre-crash to the in-crash phase, capable of evaluating and enhancing the real-world occupant protection of future vehicles. The introduction of crash-avoidance systems has the potential to alter the crash configurations that future vehicles will be exposed to. A method for predicting crash configurations has been developed in this thesis and applied to highway driving, and urban intersection crashes. Performing counterfactual simulations of digitized real-world crashes, with and without the addition of a conceptual Automatic Emergency Braking system, provides a prediction of the remaining crashes. The use of a novel crash configuration definition, along with a purpose-designed clustering method, facilitates the reduction of the number of predicted crash configurations without sacrificing coverage of the diverse real-world situations. Three predicted crash configurations, representative of urban intersection crashes, were further analyzed during the in-crash phase. A Human Body Model was positioned in a wide range of occupant postures identified from the literature. The findings suggest that the lower extremity postures had the largest overall influence on the lower extremities, pelvis, and whole-body responses for all crash configurations. In the evaluated side-impacts, leaning the torso in the coronal plane affected the torso and head kinematics by changing the interaction with the vehicle’s interior. Additionally, in far-side impacts supporting the occupant’s arm on the center console resulted in increased torso excursions. Moreover, the upper extremity responses were consistently sensitive to posture variations of all body regions.

    • Reporting incidents to an adult is the top recommendation for youth victims facing bullying. At the same time, victims need to develop strategies to use when they are facing their offenders: counter aggression and making a safety plan. This paper presents design considerations for a conversational system being a training center for a victim to learn and try such strategies. We first detail what is bullying and existing preventive measures. We then detail specific features of such a conversational system and we define a set of functionalities and dialog design considerations required to ensure compliance with the preceding features. We close with the presentation of a simple illustrative implementation.
