    • The combination of microwave-induced (2450 MHz) hyperthermia and ionizing radiation was used in 7 patients with superficial malignant tumours, which were considered refractory to other therapy. A newly developed heating system was used, allowing for a maintained temperature at the master probe of 42.5 degrees C +/- 0.5 degrees C during 45 min, but temperature measurements at multiple sites showed a marked variation. This preliminary series indicates that the combination of hyperthermia and ionizing radiation may be useful, the response rate (complete or partial) being 8 of 8 evaluable lesions. Even previously heavily irradiated sites responded. Technical improvements are highly needed to allow for controlled heating of any tissue volume.

    • In order to induce local hyperthermia in superficial tumours a computer system using a 2,450 MHz microwave generator connected to a circular (diameter 90 mm) direct contact applicator was constructed based on invasive temperature control. Small thermistor probes with a diameter of 0.6 mm are inserted in the tumour and surrounding tissues. Eight thermistors can be used simultaneously and one of them is chosen as the "master'. The automatic control system used a pulsed irradiation technique to avoid the problem with interactions between the metallic wires of the temperature probe and the electromagnetic field. With the system it was possible to control the temperature at the master thermistor within +/- 0.5 degrees C from the preset value.

    • Subcutaneous metastases from an oesophageal carcinoma were irradiated using different schedules. The results have to be evaluated with greatest caution but indicate that with the same CRE value, few fractions caused less skin reactions than several, and the size of the shoulder of the cell survival curve was of the order of 0.7 Gy.

    • We have evaluated if any differences in tumor radiosensitization exist between the two adenosine diphosphate ribosyl transferase (ADPRT) inhibitors nicotinamide and benzamide at fractionated low doses. A significant radiosensitizing effect with nicotinamide at a 10 mg/kg per day dose was found in the tumor model used. We found, however, no radiosensitizing effect with benzamide given according to this schedule.

    • The effect of hyperthermia and/or nicotinamide (200 mg/kg of body weight) on the tumour growth delay induced by radiation was evaluated in a C3H mouse mammary adenocarcinoma. The study showed a radiosensitizing effect of hyperthermia and of nicotinamide but the combination of all three modalitites showed no increased tumor growth delay compared to hyperthermia and radiation alone. The tumour growth delay induced by hyperthermia was not modified by nicotinamide.

    • Eighty-five evaluable superficial recurrent malignant tumours, mainly adenocarcinomas (78 per cent), in 38 patients were treated with either combined local hyperthermia (41-45 degrees C for four sessions) and low dose radiotherapy (30.0 Gy) or the same low dose radiotherapy alone. The treatment was given for two weeks. Hyperthermia was induced externally with 2450 MHz or 915 MHz microwaves. Totally 57 tumours were given combined treatment with a complete and partial response rate of 46 and 30 per cent, respectively (duration 1-38 months). In 18 patients with 2-10 superficial tumours each, 56 tumours were used in a comparative study, comparing the effect of combined hyperthermia and low dose radiotherapy versus the same low dose radiotherapy alone, the patients acting as their own controls. The total response rates were 89 and 50 per cent, respectively, in the two treatment modality groups. The difference in response rates is significant (p = 0.0039) in favour of the combined treatment, and this is also found when comparing complete remissions only (p = 0.0027). Local pain and normal tissue reactions presented problems during and after 2450 MHz microwave-induced hyperthermia treatment, performed without a coupling water bag system. Introduction of 915 MHz microwave-induced hyperthermia with a coupling deionized water bag system and refinement of microwave applicators, as well as the temperature control system considerably reduced these problems.

    • Sixty guinea pigs were exposed to a steady-state broadband noise with a falling frequency spectrum. The sound-pressure level was varied between 96 and 117 dB SPL, and the duration of the exposure was varied from 3 to 12 h. After 4-5 weeks, the auditory thresholds were determined by electrocochleography at 14 frequencies, and the results were compared with a control group. With increasing sound-pressure level, the thresholds became elevated at all frequencies. The maximum threshold elevation also exhibited a slight shift toward higher frequencies. With increasing exposure time, the threshold elevations increased and shifted into the high-frequency region, whereas the low-frequency region was less affected. Linear regression analysis showed that the average threshold elevation between 1 and 20 kHz did not deviate from that predicted by the equal-energy hypothesis. However, the high-frequency loss at 5-20 kHz was very dependent on the exposure time, whereas the 1- to 4-kHz loss was not. There was no sign of any critical intensity with sudden increments in threshold elevation.

    • A hyperthermia system which has been developed for clinical use is described. Different direct-contact applicators working at the ISM microwave frequency of 915 MHz are used for inducing heat in superficial tumours to a depth of about 3 cm. The system is a further development of an earlier prototype which worked at 2450 MHz. The microwave generator is controlled by a microcomputer governed by temperature measurements with thermistor probes inserted into the tissue. To minimise the influence of the interaction between the microwaves and the temperature probes, a pulsed irradiation technique is used. Up to July 1984, 80 superficial tumours had been treated with radiotherapy combined with local hyperthermia applied using this system.
