- Tinnitus är en åkomma som i lindrigare eller allvarligare former drabbar 17% av västvärldens befolkning. Ca 85 000 svenskar har tinnitus på invalidiserande nivå. Förutom mänskligt lidande orsakar tinnitus samhällskostnader på ca 1.5 miljard kr årligen. Orsaken är till största delen okänd.Vissa tecken tyder på ett samband mellan tinnitus och funktionsstörning i en käkmuskel. Några olika icke–invasiva metoder för mätning av muskelstörning i M Pterygoideus Lateralis har utvärderas. Två av dessa är intressanta för fortsatta studier.Termografi användes för att diagnosticera muskelstörningar på ytligt liggande muskler. Vi såg åtskilliga varma områden på ytliga käk– och nackmuskler på de patienter som hade käkledsstörningar, samt möjligen tecken på onormal värme från M Pterygoideus Lateralis. Mätförhållandena var dock ej ideala.En metod att mäta EMG med adaptiv noise cancelling provades. EMG från en ryggmuskel, stört av en “EKG–signal” från hjärtat användes. Metoden fungerade bra. Fortsatt metodutveckling på t.ex. ryggmuskler borde göras.

- Screening for vitamin B12 deficiency and thyroid disease is cheap and enables early diagnosis to be made and treatment to be started while it is still simple and can prevent the development of such serious conditions as dementia, depression, or irreversible tissue damage. In 1995-6, 83% (126/151) of all 75-year-olds in Härryda, a district (population 28,500) to the east of Gothenburg, agreed to undergo a health control designed to detect hypo- or hyper-thyroidism or vitamin B12 deficiency among elderly residents without symptoms (or with atypical symptoms not easily recognised). Of the 126 participants, four (3%) had low plasma cobalamin (vitamin B12) levels (a figure similar to or lower than those reported by others), and were treated with vitamin B12 after further examination; eight (6%) had serum thyroid stimulating hormone (TSH) levels below the lower normal limit, though further examination showed all eight to be euthyroid; and two (1.6%) were diagnosed as being hypothyroid (a lower prevalence than figures reported elsewhere), and were treated with laevothyroxine. The findings suggest that such screening might be useful in primary care. However, the clinical diagnosis of vitamin B12 deficiency, and of hypo- or hyperthyroidism, is often difficult, especially in the elderly; and although a low serum TSH level is also considered to be a reliable marker of hyperthyroidism, like others this study showed that it may occur even in the absence of disease. Thus, serum TSH and plasma B12 levels are useful screening variables, but need to be complemented by other tests before diagnosis is set.

- Electroencephalography (EEG) and neuroradiology are both indispensible techniques in cases of suspected epileptic seizure, when the aim of investigation is to determine whether the seizure was of epileptic nature, and if so whether it was the result of specific provocative factors or an expression of epileptic disease. In the latter case, the epileptic condition should be classified and its aetiology determined, if possible. Routine or sleep EEG providing interictal epileptiform discharges is a useful aid to differential diagnosis. To obtain EEG recordings during actual seizures, long-term recordings, using either ambulatory equipment or an EEG-video procedure, are usually used. The combination of EEG and video recording, using surface or surgically implanted electrodes, is a procedure of major importance in the evaluation of patients refractory to medical treatment and possible candidates for epilepsy surgery. In cases of epilepsy suspected to be caused by tumour or cerebrovascular disease, neurological investigation does not differ from that routinely used in such conditions. MRI (magnetic resonance imaging) techniques have become important aids in the preoperative work-up in cases of chronic therapy-resistant partial epilepsy. MRI has also simplified the identification of minor morphological abnormalities causing partial epilepsy, and is the method of choice in such cases. The sensitivity of MRI is improved by its combination with volumetric measurements and spectroscopy. The use of functional neuroimaging with SPECT (single photon emission computed tomography) and PET (positron emission tomography) during seizures provides further information. A promising new development is the co-registration of MRI and functional imaging (dipolar reconstruction of EEG spikes and seizure patterns, SPECT, PET). MRI is a cornerstone of the preoperative work-up, but diagnosis and the choice of therapeutic approach is always based on the clinical picture, EEG, and functional and morphological imaging.
