  • Prevalence of coeliac disease in Turner syndrome

    • This study was undertaken to investigate the prevalence of coeliac disease in children and adolescents with Turner syndrome. Eighty-seven children and adolescents with Turner syndrome were screened for IgA- antiendomysium antibodies (EMA) and IgA-antigliadin antibodies (AGA), 5% (4/87) being found to be EMA-positive, and 15% (13/87) to have AGA levels above normal. Of the 10 patients who were either AGA- or EMA-positive and further investigated with intestinal biopsy, four manifested villous atrophy (i.e. all three of the EMA-positive patients, but only one of the seven AGA- positive patients). The results suggest EMA-positivity to be a good immunological marker for use in screening for coeliac disease, and such screening to be justified in patients with Turner syndrome.

    • We have examined 645 recorded cases of childhood acute lymphatic leukaemia (ALL) in Sweden during 1973–89 to identify space–time clustering by using the close-pair method of Knox. The records included date of birth and of diagnosis as well as addresses at birth and at diagnosis. There was a significant excess of case pairs close in date of birth and place of birth in the 5- to 15-year age group.

  • Clinical course in synovial sarcoma : A Scandinavian sarcoma group study of 104 patients

    • We analyzed treatment and outcome in 104 Scandinavian patients with synovial sarcoma in the extremities or trunk wall, diagnosed between 1986 and 1994. Only surgically treated patients without metastases at diagnosis were included. Median follow-up of survivors was 6 (3-11) years. 34 patients developed metastases. The overall 5- and 7-year survival rates were 0.76 (95% Cl 0.66-0.83) and 0.69 (0.58-0.78), respectively. Large tumor size and amputation were significantly associated with impaired metastasis-free survival. Patients with local recurrence had a higher risk of metastases following the local event. Local excision with inadequate margin was associated with a higher risk of local recurrence.

  • Vaginal changes and sexuality in women with a history of cervical cancer.

    • BACKGROUND: In women with cervical cancer, treatment causes changes in vaginal anatomy and function. The effect of these changes on sexual function and the extent, if any, to which they distress women are not known.METHODS: In 1996 and 1997, we attempted to contact 332 women with a history of early-stage cervical cancer (age range, 26 to 80 years) who had been treated in 1991 and 1992 at the seven departments of gynecological oncology in Sweden and 489 women without a history of cancer (controls) to ask them to answer an anonymous questionnaire about vaginal changes and sexual function.RESULTS: We received completed questionnaires from 256 of the women with a history of cervical cancer and 350 of the controls. A total of 167 of 247 women with a history of cancer (68 percent) and 236 of 330 controls (72 percent) reported that they had regular vaginal intercourse. Twenty-six percent of the women who had cancer and 11 percent of the controls reported insufficient vaginal lubrication for sexual intercourse, 26 percent of the women who had cancer and 3 percent of the controls reported a short vagina, and 23 percent of the women who had cancer and 4 percent of the controls reported an insufficiently elastic vagina. Twenty-six percent of the women who had cancer reported moderate or much distress due to vaginal changes, as compared with 8 percent of the women in the control group. Dyspareunia was also more common among the women who had cervical cancer. The frequency of orgasms and orgasmic pleasure was similar in the two groups. Among the women who had cervical cancer, the type of treatment received had little if any effect on the prevalence of specific vaginal changes.CONCLUSIONS: Women who have been treated for cervical cancer have persistent vaginal changes that compromise sexual activity and result in considerable distress.

  • Increased accuracy of echocardiographic measurement of flow using automated spherical integration of multiple plane velocity vectors

    • The calculation of blood flow in the heart by surface integration of velocity vectors (SIVV) using Doppler ultrasound is independent of the angle. Flow is normally calculated from velocity in a spherical thick shell with its center located at the ultrasound transducer. In a numerical simulation, we have shown that the ratio between minor and major axes of an elliptic flow area substantially influences the accuracy of the estimation of flow in a single scan plane. The accuracy of flow measurements by SIVV can be improved by calculating the mean of the values from more than one scan plane. We have produced an automated computer program that includes an antialiasing procedure. We confirmed an improvement of flow measurements in a pulsatile hydraulic flow model, the 95% confidence interval for single estimations being reduced from 20% to 10% (p < 0.05) using the newly developed software. We think that the SIVV method has important implications for clinical transthoracic echocardiography.

    • Human papillomavirus (HPV) exists as more than 100 genotypes. It is not well-established whether the different HPV types interfere with infection or pathogenesis by each other. Possible interactions in cervical carcinogenesis between infection with the most common HPV types (6, 11, 16, 18 and 33) were studied in a seroepidemiological case- control study of 218 women with primary untreated cervical cancer and 219 healthy age-matched control women. As previously shown, HPV-16 seropositivity was associated with cervical cancer risk [odds ratio (OR), 2.39], but HPV-16 was not associated with cervical cancer risk among HPV-6 seropositive women (OR, 1.0). The relative excess risk due to interaction between HPV-6 and -16 was -2. 35 (95% confidence interval, -0.04 to -4.65), indicating significant antagonism. The results suggest that infection with HPV-6 may interfere with HPV-16-associated cervical carcinogenesis.

    • The objectives of this study were to measure interleukins 5 and 8 (IL-5 and IL-8) in relation to eosinophils and neutrophils, in nasal lavage fluids from 60 school children with allergic rhinitis, and to determine the influence of treatment with a topical steroid (budesonide) on the levels of the two cytokines. Highly sensitive enzyme immunoassays were used to analyze IL-5 and IL-8. IL-5 levels and relative eosinophil counts in nasal lavage fluid increased significantly in patients with allergic rhinitis during the pollen season, compared with values obtained before the start of the season, and decreased significantly after treatment with budesonide. By contrast, no significant changes in IL-8 or neutrophils were found during the pollen season, nor did they decrease following treatment. In the untreated patients, IL-5 levels correlated significantly with eosinophil counts but not with neutrophil counts, whereas IL-8 levels correlated with neutrophil counts but not with eosinophil counts. After budesonide treatment, the correlation between IL-8 and neutrophils remained, and a correlation between IL-8 and eosinophils emerged. These findings support the concepts that IL-5 has a key role in regulating eosinophils and that IL-8 is important for the regulation of neutrophils. Whereas IL-5 and relative eosinophil counts are profoundly affected by topical steroid treatment, IL-8 and neutrophils are not demonstrably affected by such treatment. It is possible that neutrophils, through the release of IL-8, could be chemotactic for eosinophils in steroid-treated patients.

  • BOF (Barnorienterad familjeterapi) : symtomens arena?

    • Barnorienterad familjeterapi - BOF, en terapiform där barn och föräldrar leker tillsammans i en sandlåda med ett material av dockor, djur, träd, staket, etc. I studien undersöker vi om man med ledning av det som sker i leken, samhandlingen, kan se och förstå barnets problem, dvs det symtom familjen sökte till BUP för.Vi valde att dela in symtomen i tre huvudgrupper; externaliserande, dvs. utagerande symtom, internaliserande symtom, dvs mer inåtvända symtom som oro, ledsenhet samt somatiserande, där symtomen tar sig kroppsliga uttryck.Nio familjer som inledde en BOF-terapi vid BUP, Vrinnevisjukhuset under 1997-98 kom att ingå i undersökningen.Innan terapin påbörjades fyllde föräldrarna i ett CBCL -formulär om barnets symtom. BOF-terapin utfördes på vanligt sätt med filmning av sessionerna och samtal i familjen om filmerna. Därefter fick vi tillgång till filmerna. Vi fick ej veta något om symtom eller problematik. I ett fall kände vi anmälningsorsaken. Familjesammansättningen var också okänd för oss.Vi har studerat den första terapiomgången med barn, föräldrar och terapeut vid sandlådan. Filmen har transkriberats vilket innebär att varje  sammanhållen handling och verbal tur, dvs. yttrande, har dikterats in på band. Ex, Pojke gör: Tar bilen, Mamma säger: "Kom hit" Transkriptionen har skrivits ut och analyserats. Vi har sorterat och räknat antal handlingar och yttranden och för varje aktör. För att få veta intensiteten i leken har vi sedan delat antalet med lekens längd. Vi antog att intensivare lek kan tyda på utagerande problematik.Vi har läst materialet och försökt beskriva terapiomgångarna globalt avseende innehåll och teman i leken, gränser och aktivitet i sandlådan samt terapeutens agerande.I utskrifterna har vi även försökt finna handlingar och yttranden som visar på gränsergränssättning, aggressivitet, omsorg, oro-ängslan, sjukdom-skada, ledsenhet, att bli sedd etc. Vi har även försökt se till samspelet ur olika aspekter.De olika kategorierna har markerats i skriften, förts in i ett protokoll och räknats samman. Vi har jämfört de olika resultaten och ställt en hypotes om barnets symtom.Våra antaganden om varje barns symtom har jämförts med resultatet av CBCL-skattningen. Vi fann, att vi vid åtta av nio barn hamnat inom samma huvudgrupp av symtom som CBCL. Pojken som avvek hade, liksom flertalet av barnen, en dubbel problematik med både internaliserande och externaliserande symtom. I gruppen ingick inget barn med starka somatiserande symtom varför vi ej kan uttala oss om giltigheten för den gruppen.Vår slutsats är att barnets symtom syns i samleken och att BOF-sandlådan verkligen är symtomets arena.

  • Assessing personality : The Temperament and Character Inventory in a cross-cultural comparison between Germany, Sweden, and the U.S.A.

    • In this study the American, Swedish, and German versions of the Temperament and Character Inventory were compared based on samples of 330 healthy volunteers each, which had been carefully matched for age and sex. The analyses indicate a high agreement for scores on the temperament and character dimensions and subscales across the samples. Exceptions include minor differences that appear to be due to cultural variations, differences in sampling methods, and of some minor difficulties with two subscales (Exploratory Excitability and Self-acceptance) as well as defining the Persistence factor as an independent dimension of Temperament. The subscales yielded similar internal consistencies, correlational structure, factor structures, and high factor congruence coefficients. The results indicate a cross-cultural transferability of the Temperament and Character dimensions of the inventory. Also, the validity and stability of the seven-factor model of personality, as suggested by Cloninger, is supported.
