    • The surgical procedure of using a titanium implant in combination with a mandibular bone graft for a single-tooth reconstruction is presented. The donor bone was taken from the vestibular base, close to the symphysis of the mandible. Patients who were missing a maxillary incisor or canine tooth, and exhibiting a loss of supporting bone in the area, were found suitable for the technique.

    • A method using cutting resistance measurements during low-speed threading for identification of various bone densities has been evaluated with regard to its precision and potential. Pig ribs were used as test samples. Differing hand pressure, minor deviation (5 degrees) from a vertical tapping direction and individual threading did not reveal any significant differences in cutting resistance values. After implants were inserted into the threaded canals, the total bone as well as trabecular and compact bone areas surrounding the implants were calculated via a computer program and using microradiographs of the bone test samples. The outcome of the cutting resistance measurements was compared with that of the microradiographic technique, and good agreement was observed between the two procedures in the ability to identify bone density. Therefore, cutting resistance measurements may in the future also be used to clinically identify bone qualities in jaws.

    • Ten autopsy jaw specimens (6 mandibles, 4 maxillae) were used for cutting resistance measurements during low-speed threading. Overall, 31 sites were analyzed where implants were inserted into threaded canals. Bone area measurements were performed around the implants as described previously. The cutting resistance values together with the total bone area values were found to be higher in mandibles than in maxillae, and a tendency towards higher values was seen in incisor regions compared with premolar regions. Furthermore, an intraindividual comparison between the true cutting resistance and the bone density values of prepared sites showed a statistically significant correlation. The method with cutting resistance measurements for evaluation of bone quality seems therefore to be reliable, at least when used in human autopsy jaw bone specimens.

  • MkII--a modified self-tapping Brånemark implant: 3-year results of a controlled prospective pilot study.

    • A modified self-tapping implant (MkII) with improved cutting characteristics has been designed for use in both maxillae and mandibles. Four sequential studies were conducted to evaluate the outcome of the MkII implant compared to the standard implant. The results presented here are from the extended pilot study that has been conducted as an intra-individual study of 103 patients; ie, each patient received both test (MkII) and control (standard Brånemark System) implants. Seventy patients were treated in the mandible and 33 in the maxilla. The cumulative survival rates and marginal bone resorption showed equivalent results for both test and control implants. Three-year cumulative survival rates were 87.9% and 86.8% for test and control implants in maxilla, respectively, and 99.5% and 100% in mandibles, respectively. The mean marginal bone resorption was approximately 0.5 (control) to (MK II) 0.6 mm after 3 years of function. A total of 288 test implants and 275 control implants were placed. All implants, both test and control, could be placed in an appropriate implant position, but 21.2% of the implants were not fully seated by machine power only; the use of a manual cylinder wrench for the final turns was necessary. During the last phase of the study, however, with an increase in twist-drill diameter from 3.0 to 3.15 mm and an increased motor torque, the prerequisites of successful implant placement into final position were met.

  • [Osseointegrated implants in adolescents. A three year study]

    • In 15 adolescents (age 13.2 to 19.4 years) in the late dental stage implants (n = 27) were chosen to replace missing teeth due to congenital absence or trauma. The patients were followed for at least 3 years. No fixture losses occurred. Only minor loss of bone support at the fixture was observed, while adjacent tooth surfaces showed some loss. Infra-occlusion of the implant restorations was noticed in patients with residual craniofacial growth. Thus, the dental and skeletal maturation, and not the chronological age, must be taken into account to avoid infra-occlusion. Furthermore it is important to gain enough space for the fixture in the mesio-distal direction in order to avoid the risk of marginal bone loss at teeth adjacent to the implants.
