  • Is apical periodontitis in root filled teeth associated with the type of restoration?

    • OBJECTIVE: To study the association between type of restoration and apical periodontitis (AP) in root filled teeth. MATERIALS AND METHODS: The present study used data from surveys conducted in 1983, 1993 and 2003. In 1983, 130 randomly selected subjects aged 3-80 years in the city of Jönköping, Sweden, were invited for a clinical and radiological examination. The study was repeated in 1993 and 2003. New participants were, thus, recruited with the same sampling criteria and sample size in the same geographical area in 1993 and 2003, respectively. In the present study, only dentate individuals aged 20-70 years with ≥1 root filled tooth were included, yielding a sample of 788 subjects with 2634 root filled teeth. Apical periodontitis on the tooth level was the dependent variable. Periapical status was assessed according to Periapical Index (PAI). Independent variables were root filling quality, recurrent caries, type of restoration, number of teeth with apical periodontitis, age and gender. Root fillings appearing homogenous and ending within 2 mm from radiographic apex were regarded as adequate, otherwise inadequate. All radiographs were re-studied by one observer regarding periapical status and root filling quality. Risk was analyzed by means of a GEE model. RESULTS: Type of restoration, root filling quality, number of teeth with apical periodontitis within the individual and age were found to be predictors of AP in root filled teeth. Presence of recurrent caries and gender were not found to be associated with AP. CONCLUSIONS: According to the present study, root filling quality and type of restoration may be predictive of AP in root filled teeth.

  • The meaning of oral health-related quality of life for elderly persons with dementia

    • Objective. Studies of oral health developments increasingly include self-reported assessments of how oral health affects quality of life (QoL), referred to as “oral health-related QoL”. People with dementia are often excluded in studies of oral health-related QoL and thus our aim in this study was to explore this area in elderly persons with dementia. Material and methods. Eighteen elderly individuals (aged between 78 and 94 years) with dementia of varying degrees of severity were interviewed with the aid of an interview guide; pictures and objects were used as stimulus material (triggers). The material was analyzed using grounded theory as point of departure, and a professional assessment of the oral health of the participants was used as reference. Results. Four categories were identified: the ability to chew and eat, independence, oral problems, and teeth are important. These factors are largely consistent with those that have emerged in earlier studies of the elderly, but in some cases less pronounced in persons with dementia. Conclusion. The use of triggers is a positive way to communicate oral health-related QoL among persons suffering from dementia, although the material used in this study needs further evaluation and development.

    • Bakgrund: Munhälsan har stor betydelse för den allmänna hälsan och välbefinnandet och påverkar därmed livskvaliteten .Syftet med studien var att beskriva munhälsorelaterad livskvalitet i en svensk vuxen population.Metod: Munhälsorelaterad livskvalitet mättes med frågeformuläret Oral Health Impact Profile (OHIP-14). Med OHIP-14 poängsätts munbesvär (0-56 p). Mer besvär ger högre poäng. Ett slumpmässigt urval av 910 individer, 20-80 år tillfrågades att deltaResultat: Totalt 516 individer deltog. 78% uppgav att de hade någon form av orala besvär relaterat till livskvaliteten.  20-åriga kvinnor var de som hade mest besvär. 20% av 30-åriga män och 59% av 70-åriga kvinnor uppgav att de haft besvär från munnen som inneburit att livet varit mindre tillfredställande. Andelen individer med svåra besvär, 17-41 poäng, var 9 %.Slutsatser: Den studerade populationen uppvisade i sin helhet en god munhälso-   relaterad livskvalitet. Det förekom ett relativt stort antal både unga och äldre individer med munbesvär som påverkade livskvaliteten.

    • Objectives. Oral health has a major impact on general quality of life. The aspects of oral health that are most important for each individual vary, and quality of life is a construct and not a measurable variable. The aim of this study was to describe the impact of oral health on quality of life in an adult Swedish population. Material and methods. The study comprised a stratified random sample of 519 individuals. The OHIP-14 questionnaire was used and the answers were rated on a scale of 0-4. Results. Twenty-one percent of the respondents stated that they had no oral problems related to well-being, and 79% had some form of problem related to quality of life. The mean value for the entire population was 6.4 (SD = 7.1); 5.9 (SD = 7.1) for men and 6.8 (SD = 7.2) for women. Of individuals who stated that they had oral problems, the majority were women aged 20 years. Nineteen percent of 30-year-old men and 53% of 70-year-old women stated that they had had problems that had made life less satisfactory. Individuals who frequently experienced problems related to oral health, with scores of 16-41 points, accounted for 10%. Conclusion. In this Swedish population, a number of individuals, young and old, experienced oral problems that had an impact on their well-being.

    • OBJECTIVE: The aim of the study was to test the hypothesis that root-filled teeth are at a higher caries risk than non-root-filled teeth. MATERIALS AND METHODS: Two sub-samples from epidemiologically obtained data collected in 1983, 1993 and 2003 in the city of Jönköping, Sweden, were analysed. All the participants were examined clinically and radiographically and the type of filling and the presence or absence of caries were recorded. Sub-sample 1 comprised 832 individuals (9202 teeth) aged 20-70 years with at least eight premolars/molars. In sub-sample 2, 163 subjects (577 teeth) with only one or two decayed tooth surfaces and at least one tooth with full crown coverage were analysed. RESULTS: Logistic regression analysis found that root-filled teeth were predictive of recurrent caries when controlling for the type of restoration in sub-sample 1 (OR = 1.68; 1.41-2.0; CI 95%) and sub-sample 2 (OR = 2.20; 1.07-4.52; CI 95%). CONCLUSION: In support of the suggested hypothesis, the data revealed a significant association between root-filled teeth and recurrent caries.

    • AIM: To study changes in technical quality of root fillings and periapical status in root filled teeth in random samples of 20 to 70 year-olds between 1973 and 2003. METHODOLOGY: Data from dentate subjects aged 20-70 years from examinations conducted in 1973 (n = 498), 1983 (n = 530), 1993 (n = 547) and 2003 (n = 491) were used for the analysis. Length of root fillings were measured on radiographs to the nearest 0.1 mm. Adequate seal was defined as a root filling without lateral and/or apical voids. Periapical status was assessed according to the periapical index. All observations were made by one calibrated observer registered from full mouth radiographic examinations. Teeth with root fillings ending within the canal without lateral or apical voids were considered adequate. The association between root filling quality and periapical status was analysed by means of the Chi-squared test and a multilevel logistic regression analysis. RESULTS: Adequately root filled teeth had a significantly lower frequency of apical periodontitis than inadequately root filled teeth (11.8% vs. 22.8%). The frequency of technically adequate root fillings increased statistically significant from 1973 (23.7%) to 2003 (36.4%) without a concomitant improvement of the periapical status in root filled teeth over time (24.5% vs. 24.6%). CONCLUSIONS: This study reports on an improving technical quality of root fillings over time, without a concomitant improvement of the periapical status in root filled teeth. A larger proportion of treated molars over time may be of importance for the result.

  • Dental caries and associated factors in a group of Swedish snus users

    • The aim of this study was to investigate the effects of the Swedish moist powder tobacco product known as "snus" on dental caries and to measure the pH fall in dental plaque. The subjects comprised male and female adults between 26 and 62 years of age (n=102), all habitual snus users for >= 10 years. The control group (n=101) consisted of similar individuals in terms of gender, age and educational level but with no tobacco use for >= 10 years. A clinical and radiographic examination and a questionnaire were completed.The pH fall after a sucrose rinse was estimated in situ in 10 randomly selected subjects per group. The salivary secretion rate was higher in snus users than non-users (2.5 vs 2.2 ml/min, p=0.005). There was no statistically significant difference regarding salivary buffer capacity. No differences were found between the two groups in terms of the plaque index, primary or secondary enamel and dentine caries, DFS and salivary counts of mutans streptococci or lactobacilli. The pH fall was somewhat more pronounced among non-users compared with snus users (NS). Snus users had a lower intake of snacks between meals and a less frequent intake of cookies (p=0.000). Furthermore, snus users had a mean gingival index (+/- SD) for the whole dentition of 20.4 +/- 18.2, while the index for non-users was14.4 +/- 13.9 (p=0.009); the corresponding values for teeth 13-23 were 14.9 +/- 20.6 and 7.7 +/- 11.9 respectively (p=0.003). To conclude, this clinical study revealed no statistically significant differences in caries prevalence between snus users and non-users and only minor differences regarding different cariesassociated factors.

    • Syftet med studien var att utvärdera pH-förändringar i plack in vivo vid användning av olika nikotinfritt och nikotinhaltigt snus. Kolhydratinnehållet i produkterna analyserades också. Tio deltagare, alla snusanvändare, deltog i en experimentell tvärsnittsstudie, vid elva tillfällen, med en veckas mellanrum. Sex nikotinfria och fyra nikotininnehållande produkter på den svenska marknaden ingick i studien och en sackaroslösning användes som kontroll. Deltagarna borstade inte tänderna under tre dagar innan besöket och avstod från att äta, dricka och snusa/röka två timmar före varje test. pH mättes i tre approximalrum upp till 45 minuter med snusprodukten placerad under överläppen. De nikotininnehållande produkterna innehöll endast spår av glukos, fruktos och sackaros (0.5-1%) och stärkelse (1.5%), medan de nikotinfria produkterna innehöll upp till 6.5 % lågmolekylära kolhydrater och 26.0 % stärkelse. Resultatet av pH-mätningarna i plack visade att alla fyra nikotininnehållande produkter höjde pH, till skillnad från tre av de sex nikotinfria produkterna, som sänkte pH betydligt. Dessa pH-förändringar i plack kan ha såväl positiv som negativ effekt på kariesrisken beroende på vilken produkt som används

    • The aim was to investigate pH changes in plaque in vivo during the use of nicotine- free and nicotine-containing snus. The carbohydrate content of the products was also analysed. Ten subjects, all regular snus users, participated in an experimental cross-over study, on eleven occasions with an interval of one week. Six nicotine-free and four nicotine-containing products, which are sold on the Swedish market, were included and a sucrose solution was used as a control. The subjects did not brush their teeth for three days before coming to the laboratory, without eating, drinking or using snus/smoking for two hours prior to the test. pH was measured at three approximal sites up to 45 min with the test product placed under the upper lip. The carbohydrate analysis showed that the nicotine-containing products contained only traces of glucose, fructose and sucrose (0.5-1%) and starch (1.5%). Some of the nicotine-free products contained up to 6.5% low-molecular-weight carbohydrates and 26.0% starch. The intraoral pH measurements showed that four nicotine-containing products increased the plaque pH, in contrast to three of the six nicotine-free products, which lowered the pH. These pH changes may have an effect on the caries risk, both positively and negatively, depending on which product is used.

    • OBJECTIVES: The aim of this study was to describe the use of tobacco and changes in its use over time among individuals living in Jönköping, Sweden, and to analyse tobacco habits in relation to socioeconomic conditions, personality aspects and dental care habits. METHODS: This study comprised three epidemiological cross-sectional studies, involving a random selection of individuals aged between 15 and 70 years, and was conducted in 1983, 1993 and 2003. The participants were asked to complete a questionnaire. RESULTS: The results revealed a statistically significant reduction from 34% tobacco users in 1983 to 27% in 1993 and 28% in 2003. The main decrease was seen among smokers. At the same time, the number of users of snuff increased in all the age groups between 20 and 60 years of age. The use of tobacco was therefore largely unchanged in 1993 and 2003. In 2003, there was a statistically significant difference between users and non-tobacco users when it came to the frequency of dental visits; more tobacco users than non-tobacco users did not visit a dentist at all or did not visit a dentist regularly. In 1993, non-tobacco users brushed their teeth more frequently than tobacco users and this difference was statistically significant. CONCLUSIONS: During the 20-year study, there was a reduction in the number of smokers and an increase in the number of snuff users. There was a difference between tobacco users and non-tobacco users when it came to the frequency of dental visits and oral hygiene habits.
