  • Legitimizing user knowledge in mental health services : Epistemic (in)justice and barriers to knowledge integration

    • Including the voices and knowledge of service users is essential for developing recovery-oriented and evidence-based mental health services. Recent studies have however, suggested that challenges remain to the legitimization of user knowledge in practice. To further explore such challenges, a co-production study was conducted by a team of researchers and representatives from user organizations in Sweden. The aim of the study was to explore the barriers and facilitators to the legitimacy of user knowledge, as a central factor in sustainably implementing user influence in mental health practice. A series of workshops, with representatives of mental health services and user organizations were conducted by the research team to explore these issues. The analysis built on the theoretical framework of epistemic injustice, and the underlying aspects, testimonial, hermeneutic and participation-based injustice, were utilized as a framework for a deductive analysis. Results suggest that this is a useful model for exploring the complex dynamics related to the legitimacy of user knowledge in mental health systems. The analysis suggests that the legitimacy of user knowledge is related to the representativeness of the knowledge base, the systematic formulation of this knowledge inapplicable methods, access to resources and positions within the mental health system and participation in the process of integrating this knowledge-base in mental health contexts. Legitimizing user knowledge in practice additionally challenges mental health systems to support readiness for change in working environments and to address the power and role issues that these changes involve.   • Is an 'other' city possible? : Using feminist utopias in creating a more inclusive vision of the future city

    • Visions of the good future city are important in futures studies and urban planning. However, these visions have been criticised for reflecting Western, masculine, heteronormative values rather than diversity, and only allowing some voices to speak and be heard. This highlights the need to develop methods for bringing in 'other' voices and enabling alternative visions to be articulated that contest the 'straightjacket' of accepted meanings and ways of being in the city. Here, we present one such attempt using transgressing alternatives of imagining the city drawn from feminist science fiction and utopian writings. These were presented to focus groups of women from different backgrounds in two Swedish cities. The aim was to create a welcoming and safe space for meaning-making that encourage the women to re-imagine their subject positions, challenge the accepted ways of being in the city and picture an 'other' future city. This was partially successful in that, in their discussions, the women both accepted and contested the city's gendered norms and power relations. This reflects the difficulties involved in questioning the existing power relations and norms from a subordinate position, emphasizing the importance of further developing this type of approach in efforts to foster more inclusive cities.

  • Health problems during childhood and school achievement : Exploring associations between hospitalization exposures, gender, timing, and compulsory school grades

    • Aims To investigate while accounting for health at birth 1) associations between health problems during childhood, measured as hospitalizations, and school achievement in the final year of compulsory school, measured as overall grade points and eligibility for upper secondary education, 2) if and how gender moderates the association between health problems and school achievement, 3) if and how the timing of a health problem during childhood is associated with later school achievement. Methods Analyzes were performed on a population-based cohort (n = 115 196) born in 1990 in Sweden (51.3% boys, 48.7% girls) using data from several national registries. Multiple linear regression and logistic regression were used to analyze associations between study variables. Results Overall grade points and eligibility for continuation to upper secondary school were lower for individuals exposed to hospitalizations. Only the association between hospitalizations and overall grade points was moderated by gender and only for ages 13-16 years. Exposure close to actual grading had worst outcomes. Conclusions Health problems, measured through hospitalizations, was significantly associated with lower school achievements among Swedish children. Girls exposed to health problems requiring hospitalizations had relatively poorer school achievements as compared to boys. Health problems requiring hospitalization during junior high school had the greatest negative association with final achievement at compulsory school.

  • Youth unemployment and functional somatic symptoms in adulthood : Results from the Northern Swedish cohort

    • Background: Little is known about the possible long-term health consequences of youth unemployment. Research indicates that unemployment may lead to socioeconomic downward mobility and mental health problems, but we still lack knowledge of the long-term health consequences of youth unemployment. This article examines the potential long-term association between youth unemployment and functional somatic symptoms in adulthood. Methods: The ‘Northern Swedish cohort’ was used with data from five data collections, from 1981 (age 16) until 2007 (age 42). Youth unemployment was measured as months in unemployment between age 16 and 21, and health outcome as functional somatic symptoms (an index of 10 items of self-reported symptoms). Linear regression was used to analyse the relationship between months in youth unemployment and functional somatic symptoms at age 21 and age 42, stratified for women and men and adjusted for potential confounders, such as time spent in education at age 21 and later unemployment between age 21 and 42. Results: Youth unemployment was significantly related to functional somatic symptoms at age 21 for men after controlling for confounders, but not for women. Among men, the association remained for functional somatic symptoms at age 42, after controlling for confounders. Conclusions: Adolescence seems to be a sensitive period during which unemployment could have remaining health effects in adulthood, at least for men, though assumptions of causality are tentative and more research is needed.

  • Flotation REST as a Stress Reduction Method : The Effects on Anxiety, Muscle Tension, and Performance

    • The purpose of the study was to investigate the influence of flotation REST upon skilled and less skilled golfers’ anxiety in terms of physiological indicators of stress, self-rated anxiety scores, muscle tension, and the effect on golf putting. Prior to performing the putting task participants underwent a treatment of flotation REST or a period of resting in an armchair. Participants completed both treatments in a randomized order with a two-week interval. The results showed that both flotation REST and the armchair treatment reduced systolic blood pressure and heart rate, with no differences between treatments or athlete skill levels. No significant differences between treatments were revealed regarding self-ratings, level of muscle tension or putting precision. The results indicate that flotation REST may be useful for reducing negative symptoms related to stress and anxiety in general; however, no support for direct positive effects on golf performance were found.

    • Adolescent girls' subjective health, or well-being, is of international concern as the frequency of psychological and psychosomatic complaints is continuously increasing in several countries world-wide. The causes of this development are still obscure. The aim of this study was to explore well-being and strategies for increased well-being among adolescent girls. Grounded Theory method was used, in which in-depth interviews were held with 18 adolescent girls, 17-18 years of age. Results showed that striving for acceptance and avoiding rejection were central for their well-being. When rejection was experienced, emotions of stress-shame were recognized, a phenomena we call the stress-shame cycle. In the struggle to prevent rejection and to become accepted, the girls strived to boost their social attractiveness by impression management.

    • Even though it has been demonstrated that peer support contributes to positive outcomes for service users, organizational implementation issues remain. The aim of the current study was to examine an implementation of peer support from the perspective of managers in order to develop knowledge of factors influencing sustainability of this initiative. Eighteen managers were interviewed in connection with the introduction of peer support in sixteen mental health settings. Interviews were analyzed utilizing inductive and deductive approaches. The results suggested that managers were predominantly positive in their evaluation of peer support as a recovery-oriented addition to their services, but noted developmental issues regarding role, professional identity, supervision and financing in relation to other traditional personnel. The involvement of the user movement, especially with regard to training and supervision helped prepare staff and support peer workers, yet there was some apprehension attached to the critical scrutiny that this `outsider’ perspective might imply. The results confirm previously noted uncertainties regarding peer support as an integrated component of mental health systems and illuminate a number of culturally conditioned challenges that may hamper peer support from being implemented with the same approach as other interventions. In response, the present study suggests a number of focus areas that should be attended to in future implementation efforts, including issues related to staff roles, power dynamics, connection to the user movement and reconsideration of the value of experience-based knowledge. In the Swedish context, a government level commitment was identified as critical to ensure stable funding.

    • Introduction: Travel behavior research has only started to address how travel affects emotional wellbeing. The development of measurement methods is an important goal of this research.Methods: A review and assessment of methods of measuring travel-related emotional wellbeing is presented guided by a conceptual framework specifying what is measured (cognitive evaluations, emotional responses, or moods), the way it is measured (proactively, instantaneously, or retrospectively), and when it is measured (before, during, or after travel). Anticipated, current, residual and recalled moods are the objects of the measurement. Only studies of commuting or other types of daily travel are addressed.Results: We find that no research has measured anticipated moods, some research has measured current moods before, after and during travel, and most research has measured recalled moods.Conclusions: The most valid and reliable method is to measure current mood instantaneously at several points in time, before, during, and after travel. A measure of emotional wellbeing can then be obtained by objective aggregation. An approximate more feasible method is to retrospectively measure recalled moods for a given specified time period that may not only include travel. The available methods for measuring recalled moods have acceptable psychometric properties but research is needed to validate these methods by comparing the results to an objective aggregation of instantaneous measures of current mood at different points in time.
