  • Migration, Stress and Mental Ill Health : Post-migration Factors and Experiences in the Swedish Context

    • This predominantly empirical dissertation deals with how socio-economic living conditions and immigrant-specific factors can be linked to immigrants’ mental ill health. It is also explored how cultural representations can affect stress and whether mental ill health is expressed differently among immigrants from Iraq and Iran than among individuals of Nordic origin. Moreover, a conceptual analysis is conducted, where a phenomenological conceptualisation of stress is outlined with a special focus on how this stress approach can be related to culture and migration.The empirical material consists of eleven in-depth interviews with Iraqi and Iranian immigrant women and two population-based surveys.The main findings of this thesis suggest as follows: 1) Mental ill health is more common among foreign-born than among native-born Swedes and can to a great extent be attributed to their poorer socio-economic living conditions. 2) Immigrants’ mental health is independently associated with different types of factors, such as traumatic episodes, socio-cultural adaptation level and socioeconomic living conditions. 3) The self-reporting mental health instruments, HSCL-25 and WHO (ten) Wellbeing Index, produce scores that are comparable between Scandinavians and immigrants of Middle Eastern descent. 4) Nonuniversal representations that can be found in Iraq and Iran can amplify, or even be necessary ingredients in certain types of stressful experiences among immigrant women from these countries. 5) The distinctions between universal and non-universal stress, and between immigrant/minority and non-immigrant/nonminority stress appear to be crucial for an adequate comprehension of immigrants’ stressful experiences.

    • This thesis deals with the role of technology in elderly peoples everydaylife, to what extent technology can facilitate or prevent them from havinga social life. The intension is to describe the role of technology from olderpeoples own perspective. Four activities in everyday life in which technology is involved are emphasised: social contacts, security, information and distribution of services and goods. Technology related to these activities are telephones, televisions, safety alarms and homeshopping-terminals. The aim of the study is to describe how these fourtechnologies are used by the elderly and what function and meaning theygive to the technology; and to describe old peoples attitudes to newtechnology in general and to the home shopping-terminals specificallyand how this technology affects their everyday life contacts and routins.The main emphirical work consists of recurrent interviews andobservations of a group of old people living in a city (Malmo) and in a rural district (Kinda).The results show that the function of technologies such as the telephoneand the television becomes more specific and even more important to connect the home with the outside world. By this kind of technology the interviewees remain socially integrated but, which is one of their expressed preferences, with no demands on social participation. A growing need of privacy is succesfully combined with an increased need of security and help. Technology is not used to establish new contacts but relations to relatives and friends are maintained. The attitude and acceptance of new technology in general is positive due to their lifelong experiences of improvements. The difference is that they are not that interested anymore, referring to an increased pragmatic vue towards modernity, to a limited future-perspective and to their basic values.

  • Influence and Invisibility : Tenants in Housing Provision in Mwanza City, Tanzania

    • A high proportion of urban residents in Tanzanian cities are tenants who rent rooms in privately owned houses in unplanned settlements. However, in housing policy and in urban planning rental tenure gets very little attention. This study focuses on the reasons for and consequences of this discrepancy between policy and practice. Perspectives and actions of different actors involved in the housing provision process in Mwanza City, Tanzania, have been central to the research. The examined actors are residents in various housing tenure forms as well as government officials and representatives at different levels, from the neighbourhood level to UN-Habitat. The main methods have been interviews and discussions with actors as well as studies of policy documents, laws and plans. Among government actors, private rental tenure is largely seen as an issue between landlords and tenants. Tanzanian housing policy focuses more on land for housing than on shelter. This means that house-owners who control land have a more important role in urban planning and policies than tenants have. In Tanzania in general and in Mwanza in particular, housing policy focuses on residents’ involvement in upgrading unplanned areas by organising in Community Based Organisations. This means that owners who live for a longer period in an area benefit more from settlement improvements than tenants. Tenants are relatively mobile and do not take for granted that they will stay in the same house for long. This raises the question of tenants’ possibilities to influence as well as their rights as citizens as compared to that of owners. The question of citizens’ rights for dwellers in informal settlements has received increased attention during the last years in international housing policy discussions. There is an evident need to intensify and diversify this discussion.

  • Integrated teaching for expanded vocational knowing : Studies in the Swedish upper secondary Health and social care program

    • Integrated teaching is emphasized in Swedish upper secondary vocational education and training, for managing sociocultural and historical changes related to: a) increased demands on future competent health care workers, b) educational reforms, c) altered conditions for vocational teachers’ work, and d) vocational contextualization of teaching and learning content. However, national curricula from 1970, 1994, and 2011 recommend integrated teaching as a solution without any specific concretization of what integration could or should contribute with. Thus, the aim of this thesis was to explore the realization of integrated teaching and the vocational knowing made available by integration for students at the Swedish upper secondary Health and social care program, and partly for nursing students in higher education and training. The research questions attended to how integrated teaching is realized, and what vocational knowing is made available by integration.Theoretical point of departure was Cultural historical activity theory (CHAT), complemented with New literacy studies (NLS). The methodological framework included ethnographically inspired case studies, interviews, specifically semi-structured interviews and life-history interviews, and a systematic review. Research material was collected in 2012 and 2018. Analysis was performed with selected concepts from CHAT, and in one study also with concepts from NLS. In one study, i.e. the systematic review, GRADE CERQual was used for an assessment of confidence in the review findings.Study results showed that integrated teaching, regardless of composition and format, made available a vocational, a general, and an expanded vocational knowing. Also, vocational contextualization of school subjects was shown to be significant as an additional teaching and learning content and as mediational means between school and workplace.In conclusion, integrated teaching was shown to respond to the sociocultural and historical developments by making available for students an expanded vocational knowing. Also, vocational contextualization was shown to make possible for students learning knowing relevant for their future profession.

  • "Tarzan och Jane" : Hur män som sjuksköterskor formar sin identitet

    • The study focuses on locally situated interactions between men and women and among men. The main focus is on how men in nursing practice constitute their identities. The aim of the disserta-tion is to understand the meaning of gender, particularly the constitution of masculinities, in the formation of identity for men in nursing.Theoretical points of departure were post-structuralist and masculinity theories. Within the theoretical framework, processes of gender should be seen as activities that are relational and integrated in ongoing organisational life, in this case, in the nursing context.My methodological approach was qualitative, based on ethnography. Techniques used for data collection were following observations and interviews. I followed seven men in their daily work in two hospital environments, one emergency department and a department in elder care (sheltered housing).The gender order was maintained by rewards to medical and technical knowledge and skills. The phenomenon of”gender dizziness” was manifested through interaction and became visible through the men’s practice. Different positions of masculinities co-operated and the physicality of the body was important in performing masculinities. Hegemonic positions of masculinities are maintained and other positions are subordinated. Homosociality creates influence and power in social relations, and it was obvious that the informants in these organisations found ways to keep together, in spite of their different positions in the organisation.Some of the informants cross over the border and perform ideals that are not traditional for men in nursing. In the nursing environment these men’s identities show a caring attitude. The stereotype, connected to heteronormative ways of thinking, plays an important role for men in constructing their identities in the nursing context. A central conclusion in this study is that sexuality order put strong pressure on identity formation and the construction of masculinities for men in nursing.

  • Lifelong learning : The social impact of digital villages as community resource centres on disadvantaged women

    • The overall aim of this research was to enhance the understanding of what affects the social impact of ICT in lifelong learning on disadvantaged women.In contributing to the field of social informatics, this research employs behavioural theories as strategy and analytic possibilities. This research mainly used the Kenyan digital villages as CRCs as settings but did also look beyond such establishments to provide a more solid picture. The studies were located in Kenya with complementary studies in Bolivia, Cameroon, Sri Lanka, and Sweden. The main strategies and methods used were case study, comparative education approaches, and observations and interviewing techniques.The findings suggest that ICT and CRCs have the potential to support disadvantaged women and their lifelong learning. However, the positive social impacts are limited because the arrangement of them generally does not favour vernacular languages, illiterate users, female owners and users, or non-students. In general, the use of ICT was sometimes perceived as forced, which is both a barrier and a stressor in the use of ICT in lifelong learning. It also emerged from the comparative studies that discussions among the participants in the CRCs largely covered issues in respect to 1) family and reproduction and 2) self-esteem, i.e. what settles the matter of the social impact of ICT in lifelong learning depends on change attitude among men and women. With minimal if not zero self-esteem a change that would make the difference or break a woman’s “legendary status quo” in order for a woman to feel that she can reach her goal or ambitions in lifelong learning would be difficult. Hence the lack of self-esteem is a stressor in itself.This research is valuable for stakeholders delving into issues of development and learning using ICTs, not only in Kenya but in a broader, global perspective.

  • Reframed Identity : Red Cross nurses' identity formation between 1945 and 1977

    • The overall aim is to explore the construction of nurse identity at the Red Cross Nursing School (RCNS) in Sweden, between 1945 and 1977, when nursing became part of the system of higher education in Sweden. Occupational identity is understood as a construct both of social learning and of a self-image. The empirical data consist of documents from the Red Cross Archive concerning the training of nurses and of interviews with nurses trained during the studied period. A hermeneutical interpretative method has been used. The result showed that the period studied was divided into two different phases. The first phase between 1945 and the end of the 1960s, is called the post war phase. The following phase, from the 1960s until 1977, is called the reform friendly phase. RCNS was in the post war phase a kind of total institution, based on a particular belonging to a very special and highly respected social community, reinforced by the student nurses being dressed in the same uniform and living under the same strong authority in a quite homogeneous group. The training was an introduction to a life-long membership in the total Red Cross Nurse community. This community was in the reform friendly phase challenged by diverse external changes in society. Within a period of approximately ten years, much came to be questioned, and many previously important customs and habits were abandoned. The total community started to dilute, and the lives of students changed gradually passing to a social arrangement where they slept, lived and worked in different places with different people under different authorities. The total community was impossible to modernise, built as it was on pre-modern values and hierarchies. This led to a breakdown of the traditional Red Cross nurse identity based on honour, where they were first of all Red Cross sisters. While the uniform occupational identity was challenged, more and more nurses combined family and work. This meant that their self-identities became more multidimensional. The key to our understanding of the Red Cross nursing education and the shift that took place in the late 1960s is thus to be found in the transformation of the internal collective, where nurses no longer lived and 2 learnt the traditional Red Cross codes of honour and conduct. Gradually, nurse training was integrated into higher education. This was a major reframing of the occupational identity formation, from specially selected Red Cross nurse students being trained into a total community of experienced Red Cross nurses, to an academic education of professional nurses.

    • Profession and Existence. A Hermeneutical Study of Asymmetry and Reciprocity in Nurses Encounters with Severely Ill PatientsThe purpose of this study is to investigate how nurses constitute themselves in encounters with severely ill suffering patients. The self is understood in existential meaning and it is described and analysed in the context of suffering by means of four research questions: How is the professional self constituted in 1) nurses concepts of being professional 2) nurses concepts of the other, the patient 3) nurses concepts of friendship and love in professional encounters 4) nurses concepts of the body? 14 registered nurses have been interviewed and the texts have been interpreted within a hermeneutic and existential framework, mainly influenced by Paul Ricoeur. When it comes to nurses’ concepts of being professional, the study indicates that there is a tension between the professional, the personal and the private fields of their lives. With the second research question the attention is focused on nurses’ images of patients. The interpretations here evolve in the tension found between nurses’ common concepts of professionalism and of being a patient, and their narrations of actual encounters with patients. The main themes when it comes to love, friendship and profession are proximity and distance and the tension between them. The meaning of the lived body in nurses’ narratives is captured in its most concrete appearance, as a touch.The interpretations of the study are further discussed by means of three models for how nurses constitute themselves in the encounter with patients. The asymmetric relation between a nurse and a patient characterizes these models. The dissertation argues for a fourth model that emanates from the existential framework of the study and where the asymmetric character of the relation fades out and gives space for reciprocity in the professional encounter.

  • Presenting the Absent : An Account of Undocumentedness in Sweden

    • This thesis provides an ethnography and critical phenomenology of undocumentedness in the Swedish context. By attending to the forces and processes that circumscribe the life-worlds of undocumented persons, as well as the phenomenology and essential experiences of their condition, a complex and multi-layered illustration of what undocumentedness is and means is successively presented. Employing a dual conceptualization of the state, as a juridico-political construct as well as a practiced and embodied set of institutions, the undocumented position emerges as a legal category defined only through omission, produced and reproduced through administrative routine and practice. The health care sector provides empirical examples of state-undocumented interaction where the physical and corporeal presence of the officially absent becomes irrefutable. This research suggests that the Swedish welfare state – universalistic, comprehensive and with digitized administrative routines – becomes a particularly austere environment in which to be undocumented. Drawing on interviews with regional and local health care administrators, NGO-clinics’ representatives and health professionals, as well as extensive participatory observation and interviews with undocumented persons, I argue that the undocumented condition is characterized by simultaneous absence and presence, and a correspondingly paradoxical spatiality. I suggest that the official absence and deportability of undocumented persons deprives them of the capacity to define space and, in an Arendtian sense, appear as themselves to others. There are, however, some opportunities for embodied political protest and dissensus. The paradoxical qualities of the absent-present condition manipulate the undocumented mode of being-in-the-world and I argue that alienation and disorientation are essential experiences of the undocumented situation.
