  • Inviting Community into the Development of Globally Sustainable Food Systems

    • Food systems and human diets contribute to unsustainable socioecological conditions, which in turn negatively affect human health. These driver-impact relationships span multiple scales, prompting international governance bodies, nations, and communities alike to grapple with solutions for a better food future. Collaborative action across scales and sectors is necessary; however, how communities can align contributions with efforts at broader scales is unclear.The aim of this research is to develop theoretical and procedural supports for community engagement in globally sustainable food systems (SFS), and to provide concrete results relevant to one case community.The community of nutrition and dietetics professionals was chosen as the case community given its history of engagement with SFS, its integration throughout food system sectors, and because dietary shifts have significant potential to contribute to SFS. Furthermore, the researcher’s position as a member of this community supported the case study work.The research uses transdisciplinary methods guided by the Framework for Strategic Sustainable Development (FSSD) and Community Development theory. The FSSD provides a concrete definition of sustainability and includes methodological supports for co-creation of sustainability transitions. Community Development theory supports participatory approaches and welcomes different knowledge cultures in such co-creation. The Delphi Inquiry method was used to facilitate data collection and community engagement. For measurement-specific elements of the research, causal loop diagrams (CLD) informed by the Cultural Adaptation Template (CAT) theory were used, and Critical Dietetics was used as a framework for dietetics-specific analysis.High level insights include that: (i) participatory and multidimensional approaches are important to facilitate community engagement in SFS development; (ii) objective parametres for defining sustainability are critical to guide concerted action and can provide an innovation space that invites creative and diverse solutions within; (iii) systems thinking and related tools help simplify the complexity of food systems without disregarding broader context, and support assessment in the absence of all data. Specifically in relation to the case community explored, insights include that, (i) integrating an SFS lens into existing roles and activities is important, because dietitians already work across sectors and scales, making them well positioned to contribute in diverse ways; (ii) a shared language based on transdisciplinary understandings of SFS is required; (iii) engaging in activities that facilitate SFS knowledge development within the profession, prior to integrating it into roles and activities, is an important first step; (iv) collaborative and reflexive approaches to continued knowledge development and practice are important, such that in the end sustainability becomes integrated into a cultural way of thinking about food.  Based on these insights, this dissertation outlines a procedure for collaborative community work for globally SFS. The procedure is adaptable to various community settings. The dissertation also provides specific guidance for how dietitians could utilise their strategic positions throughout food systems to contribute to SFS development.

  • Older people in persistent pain : nursing and paramedical staff perceptions and pain management

    • BACKGROUND: Persistent pain is a common problem for older people. Knowledge about how nursing and paramedical staff perceive these people and what they do to relieve the pain seems scarce. AIM: To explore nursing and paramedical staff perceptions of older people in persistent pain and their day-to-day management of pain. METHODS: Interviews in Swedish with 52 nursing auxiliaries, Registered Nurses, physiotherapists and occupational therapists were collected from February to May 2000. The analysis was based on their stories (n = 150) about older people in persistent pain who received help in their own homes or in special accommodation. A typology of staff perceptions of pain in older people was developed. Activities to manage pain were examined using content analysis. RESULTS: Respondents perceived the pain as real, exaggerated, trivial, care-related, endured, concealed, self-caused or inarticulate. Older people perceived as exaggerating the pain, those with care-related and self-caused pain evoked frustration in the staff, while those perceived as enduring their pain evoked satisfaction. Various strategies to manage pain were used: no activity, medication, mediating contacts, distracting activities, physical therapies, mobility, work in a gentle way, rest or relieving pressure on body part, and communication concerning pain. The activities differed between the types, as well as between staff with different professional backgrounds. CONCLUSION: Care and treatment provided by staff should be based on older people's needs rather than on staff attitudes and preferences. The typology revealed that staff perceived older people in pain as a heterogeneous group and that their perceptions affected the pain-relieving activities that were offered. It seems urgent to address how to handle pain in older people who never complain and those who complain a great deal, as well as how to handle pain in people with impaired communicative ability. Reflective discussions on feelings related to different individuals are needed.

    • AIMS AND OBJECTIVES: This study aimed at investigating life satisfaction and its relation to living conditions, overall health, self-care capacity, feeling lonely, physical activities and financial resources among people (65+) with reduced self-care capacity. BACKGROUND: Knowledge about factors related to low life satisfaction among older people with reduced self-care capacity is sparse, although this is important in health care and nursing so that the care is adapted to their needs and perspective. Previous research has mainly focused on isolated aspects such as pain in relation to life satisfaction among older people in general and less among so those with reduced self-care capacity in general. DESIGN AND METHOD: A subsample of 522 persons was selected from a randomly selected cross-sectional survey using a modified form of the Older Americans' Resources Schedule and Life Satisfaction Index Z. RESULTS: The mean age in the total sample was 77.9; women (79.5) were significantly older than men (77.0). Low life satisfaction was found among women, as well as those living in special accommodations. Life Satisfaction Index Z was 15.3 (SD 5.6) in the total sample. Gender and living conditions did not explain life satisfaction whilst poor overall self-reported health and poor financial resources in relation to needs had the strongest explanatory value. Also of significant importance were loneliness, the degree of reduced self-care capacity and feeling worried. CONCLUSION: Life satisfaction in older people with reduced self-care capacity is determined by several factors, with social, physical, mental and financial aspects probably interacting with each other; especially feeling lonely, degree of self-care capacity, poor overall health, feeling worried and poor financial resources in relation to needs. These factors need to be considered in the care of these people to preserve or improve their life satisfaction. RELEVANCE TO CLINICAL PRACTICE: Nursing interventions in terms of preventive home visits, rehabilitation, health education directed towards physical, psychological, social and economic aspects of importance may help to preserve or improve life satisfaction for those with reduced self-care capacity.

    • In Swedish municipalities, householders are addressed as important actors in the efforts to improve the quality of sewage sludge. Householders and sanitation professionals are regarded as integrated and active co-producers of the sanitation system, while limited by the functions offered by the technical and organizational structure. Through interviews with both householders and sanitation professionals, recognition of interdependence was identified in both groups although more communication with counterpart was asked for. If the aim is to increase sustainability in the sanitation system, regardless of technical changes, we argue that two-way communication between householders and sanitation professionals needs to increase and embrace not only advice about choice of products and ways to carry out water related activities, but also planning of activities, feedback on results and other aspects of interest.

    • Genus och miljö är två områden som sällan kopplas samman, men som båda ska integreras i olika verksamheter som undervisning, samhällsplanering och samhällsutveckling. Utgångspunkten för denna bok är att de båda områdena förutsätter varandra och måste hanteras samtidigt. Strävan efter jämställdhet måset ta hänsyn till de miljökonsekvenser som följer med förändringar som görs. Ansträngningar att minska belastningen på ekosystemen måste på motsvarande sätt belysa genusaspekter kring frågor som odling och vattenförsörjning eller energianvändning. Strävan efter jämställdhet leder till att vi inte alltid kan se tydligt hur framtidens miljöpolitik ska formas, och miljöproblem leder till att jämställdhetsarbetet måste omformuleras efter hand. Vägen in i framtiden är inte solklar och spikrak, och vi måste beträda den med varsamhet och lyhördhet för olika röster och nya insikter. Boken presenterar grundläggande genusvetenskapliga teorier kopplade till miljöfrågor, naturvetenskap och teknikvetenskap och vänder sig till studenter inom miljövetenskapliga utbildningar samt till studenter inom genusvetenskap med intresse för miljöfrågor.

    • Engineering is an important force in the building of society. Engineers are identified as problem solvers, but also criticized for causing new problems due to a narrow focus on technology. Feminist theorists as well as researchers within sustainable development have called for a more democratic and accountable practice among engineers, in order to cater for widespread societal and environmental risks as well as moral questions of technological innovations. The introduction of sustainable development has been introduced by the Swedish government into the school system at all levels and specifically in higher education of engineers. Engineering education has a potential to prepare future engineers for engagement in societal aspects to a higher degree, but this requires a more integrated education program where values and discussions are integrated in courses.

    • Studies have shown that societal change related to economic growth and development policies can affect the adaptive capacity of communities to a multitude of stressors including climate variability and change. Concerns have recently been raised about the consequences of climate mitigation and adaptation on vulnerable groups and the impacts of large-scale globalization processes on the adaptive capacities of local communities. This chapter addresses how side effects of technological and socioeconomic changes, which we refer to as spinoffs have potential to strengthen climate adaptation strategies. The spinoff examples fall under a two-dimensional framework according to whether they arise from orchestrated or opportunity-driven initiatives and technological or socio-economic changes. Three cases in developing countries undergoing rapid economic growth have been chosen as examples of different types of spinoffs and how they can positively influence climate adaptation and more particularly adaptive capacity. They are: information and communication technology (ICT) in South Africa, changing lifestyles in China and empowerment in India. The cases illustrate that new objects, inventions and trends constantly emerge which have potential to help people improve their livelihoods in ways that can be climate smart. People working as development workers and policy makers need to be observant and engage in open-minded dialogue with communities in order to recognize emergent technologies, lifestyles and trends to facilitate the use and development of on-going or potential spinoffs that positively affect adaptation to climate change.
