Technological and informational frames : explaining age-related variation in the use of patient accessible electronic health records as technology and information

Purpose Data from a national patient survey (N = 1,155) of the Swedish PAEHR "Journalen" users were analysed, and an extended version of the theory of technological frames was developed to explain the variation in the technological and informational framing of information technologies found in the data.Design/methodology/approach Patient Accessible Electronic Health Records (PAEHRs) are implemented globally to address challenges with an ageing population. However, firstly, little is known about age-related variation in PAEHR use, and secondly, user perceptions of the PAEHR technology and the health record information and how the technology and information-related perceptions are linked to each other. The purpose of this study is to investigate these two under-studied aspects of PAEHRs and propose a framework based on the theory of technological frames to support studying the second aspect, i.e. the interplay of information and technology-related perceptions.Findings The results suggest that younger respondents were more likely to be interested in PAEHR contents for general interest. However, they did not value online access to the information as high as older ones. Older respondents were instead inclined to use medical records information to understand their health condition, prepare for visits, become involved in their own healthcare and think that technology has a much potential. Moreover, the oldest respondents were more likely to consider the information in PAEHRs useful and aimed for them but to experience the technology as inherently difficult to use.Research limitations/implications The sample excludes non-users and is not a representative sample of the population of Sweden. However, although the data contain an unknown bias, there are no specific reasons to believe that it would differently affect the survey’s age groups.Practical implications Age should be taken into account as a key factor that influences perceptions of the usefulness of PAEHRs. It is also crucial to consider separately patients’ views of PAEHRs as a technology and of the information contained in the EHR when developing and evaluating existing and future systems and information provision for patients.Social implications This study contributes to bridging the gap between information behaviour and systems design research by showing how the theory of technological frames complemented with parallel informational frames to provide a potentially powerful framework for elucidating distinct conceptualisations of (information) technologies and the information they mediate. The empirical findings show how information and information technology needs relating to PAEHRs vary according to age. In contrast to the assumptions in much of the earlier work, they need to be addressed separately.Originality/value Few earlier studies focus on (1) age-related variation in PAEHR use and (2) user perceptions of the PAEHR technology and the health record information and how the technology and information-related perceptions are linked to each other.

Do you want to receive bad news through your patient accessible electronic health record? : A national survey on receiving bad news in an era of digital health

Despite the fact that patient accessible electronic health records (PAEHRs) have been around for many years in several countries, there is a lack of research investigating patient preferences for receiving bad news, including through PAEHRs. Little is also known about the characteristics of the patients who prefer to receive bad news through the PAEHR in terms of e.g. medical diagnosis, age and educational level. This study, based on a national patient survey in Sweden (N=2587), investigated this. Results show that, generally, receiving bad news by reading in the PAEHR is still among the least preferred options. Additionally, a higher proportion of men want to receive bad news in the PAEHR compared to women (p=0.001), and the same goes for those who are not working/have worked in healthcare (p=0.007). An effect of disease groups was also found, showing that diabetes patients in particular, want to receive bad news through the PAEHR.

Opinions and use of mobile information technology among older people in Northern Finland : Preliminary results of a population based study

Older peoples usage of mobile devices is increasing. This study is part of the GASEL project and seeks to increase the understanding of the use of mobile information technology and opinions towards it. Opinions are compared between genders and age groups. The population based data were collected by conducting a questionnaire survey. A random sample of 1,500 adults 65 years of age or older was obtained from the Finnish Population Register Centre. The number of respondents was 918 with a response rate of 61.2 %. About one fifth (n=169) had used a tablet computer and approximately one third (n=226) had used a mobile phone with a touch screen in the past 12 months without encountering major difficulties. The respondents had mostly thought that using the devices would not be easy. They had also thought that the usage would not be too expensive for them. Men and younger old people held more positive opinions towards these new technologies. Older people are slower in adapting and accepting new technologies and this should be taken into account when designing services, applications and content, as these technologies have the potential to enrichen the lives of this specific population group.

Differences in Health Information Literacy Competencies Among Older Adults, Elderly and Younger Citizens

To address the research gap on age-based differences in health information literacy (HIL), we investigated how younger (born 1960–) and older adults (1946–1960), and elderly citizens (–1945) differed from each other by their HIL competencies. Data were collected with an online survey of patients using the Swedish national electronic health record system. Altogether, 2,587 users responded. One-way ANOVA with post hoc tests revealed several differences between the groups: younger adults were less likely to value health information than older adults; older adults and elderly were least likely to compare information from multiple sources and had trouble in determining health information needs; older adults were most likely to have trouble understanding health terminology and the elderly to have difficulties in understanding medicinal package labels. The study shows that HIL is not necessarily improving or declining but adapting to challenges of advanced age. © 2019, Springer Nature Switzerland AG.

Improving the Usefulness of Research Data with Better Paradata

Considerable investments have been made in Europe and worldwide for developing research data infrastructures. Instead of a general lack of data about data, it has become apparent that a pivotal factor that drastically constrains data use is the absence of contextual knowledge about how data was created and how it has been curated and used. This applies especially to many branches of social science and humanities research, where data is highly heterogeneous, both by its kind (e.g. being qualitative, quantitative, naturalistic, purposefully created) and origins (e.g. being historical/contemporary, from different contexts and geographical places). The problem is that there may be enough metadata (data about data) but there is too little paradata (data on the processes of its creation, curation and use). The aim of this position paper is to draw attention 1) to the need for a better and more systematic understanding and documentation of the contexts of creation, curation and use of research data to make it useful and usable for researchers and other potential users in the future, and 2) to specific obstacles that make the capturing of this particular type of metadata, known as paradata, especially difficult. Failing to understand what information about the creation, curation and use of research data is needed and how to capture enough of that information risks that the currently collected vast amounts of research data become useless in the future.

There is a broad consensus that better models for assessing the impact of information efforts are needed to inform information and knowledge management and digital preservation. In contrast to measuring the quality of information, less attention has been directed to the assessment of knowledge and information processes as its constituent. Using archaeology and archaeological information as a sample context, the aim of this conceptual paper is to probe in to the evaluation of the impact and usefulness of information by taking into account the practices of how it is produced, managed and used. On a basis of a review and discussion of earlier literature on the impact of information and evaluation and management of information in archaeology, it is proposed that a better understanding of how the impact of information unfolds as a part of its production, management and use could contribute to the development of infrastructures, repositories and procedures for the management of the preservation and use of these resources.

Cancer patients' information seeking behaviour related to online electronic healthcare records

Patients' online access to their EHR together with the rapid proliferation of medical information on the Internet has changed the way patients use the information to learn about their health. It is well documented that patients often turn to the Internet to find information about their health and care. However, little is known about patients’ information seeking behaviour when using online EHRs. By using information horizons as an analytical tool this paper aims to investigate the information behaviour of cancer patients who have chosen to view their EHRs (readers) and to those who have not made that option (non-readers). Thirty interviews were conducted with patients. Based on information horizons, it seems that non-reading is associated with living in a narrower information world in comparison to readers. The findings do not suggest that the smallness would be a result of an active avoidance of information, or that it would be counterproductive for the patients.

This panel debates whether the ways in which social media are changing the nature, creation, seeking, use and sharing of information constitute a transformation or are primarily marked by continuity. Ubiquitous and everyday access to social media (for some) seems to be bringing about changes in social practice, including of information-related activities, such that conceptualisations of information itself are potentially reshaped. Discussants draw inspiration from the pervasive impact on information activities of the everyday adoption of social media. At a theoretical level they also draw inspiration from the analytic resources of contemporary practice theory and its emphasis on materiality and embodiment, routine and change, social expectations and social identity, and knowledge as a process. All the participants of the panel have conducted new empirical research on social media use with a focus on its deep as well as broad impact. The audience members are invited to discuss with the panelists questions such as how social media relate to routinised daily practices and institutionalised practices and hierarchies, how their use refashions social relationships, how they turn information seekers and users into information managers, producers and creators and shape perceptions of information authority and trustworthiness, and how a new theorisation can help librarians, information professionals and researchers understand change and assume a proactive role in it.

Information sources and perceived success in corporate finance

The work of corporate finance professionals is information-intensive, despite the fact that the practices and motivations of their information preferences have been researched very little. The present study investigates perceived success and how it is related to corporate finance professionals’ information source use behavior based on a Web survey of 92 Finnish corporate finance professionals. The principal finding of the statistical analysis of the data is that perceptions of work success and specific types of information sources contributing to the success are related. The correlations are complex, and very different types of information sources contribute to individual types of success, and vice versa. The findings indicate that information sources function as measures of success and serve an instrumental purpose. Besides functional relations, the correlation of the variables suggests more comprehensive success and information source use related dependencies and preferences. The findings imply that by studying existing perceptions of success, it is possible to make inferences about preferred information sources. The study also suggests that both personal and organizational perceptions of success should be taken into account when planning information services and information literacy education for corporate finance professionals to increase their effectiveness and relevance for the professionals.
