  • Changes in outdoor mobility when becoming alone in the household in old age

    • The aim of this article is to analyze reported changes in outdoor mobility, increased/unchanged/decreased, for a sample of older people (>62 years) in two regions in Sweden, who have transitioned from a two-person to a single-person household during the two years since the study was conducted. The target group (N=162) consists of all people who had transitioned to a single-person household in a random sample of 2033 people. The predominant results reveal that the stressful life event of transitioning into a single-person household in old age means reduced outdoor mobility for certain sub-groups. All modes of transport are used similarly regardless of reported changes in mobility (except for walking). Our results suggest that society must put more effort into offering good walking conditions, since (a) walking seems to be the most important mode of transport for outdoor mobility and (b) walking is valuated almost as high as car after becoming alone in the household regardless if the population in our study reported unchanged, decreased or increased mobility. Further, illuminating another result, namely that special transport service 3 (STS) came out as especially important for people with increased activity, society also needs to invest in the provision of STS to keep the most vulnerable group of people mobile when other modes of transport are no longer a reality.

    • In Sweden, personal assistance is one form of support for persons with certain functional impairment. The aim of personal assistance is to give compensatory support to individuals so that they can enjoy the right to live a life like anyone else. This article explores what kind of support parents who have functional impairments and apply for personal assistance may need when they have underage children. The article is focused on the following: What support needs in parenting are expressed in personal assistance applications and by whom? How are the expressed needs in parenting met by the intervention personal assistance, i.e. how do the decisions correspond with the expressed needs? The article is based on qualitative and quantitative document-analyses of 100 randomly selected applications for the right to personal assistance submitted to the Swedish Social Insurance Agency by applicants who were parents with children 0–17 years of age, during the years 2014–2017. The central concepts resulting from the analyses of the empirical material were support and care. The results show that in many cases the children were invisible in the material studied. In several investigations, there was no information at all about the applicant’s children. Many applications for assistance were refused. Few parents received assistance in their parenting role. In the documents, we found the following themes in statements about needs for support related to the role as a parent: practical support, care and supervision, support with communication, emotional support and safety, and support to be involved in the children’s lives.

  • Lack of Coordination between Health Care and Social Care in Multi-Professional Teamwork : the Obstacle for Coherent Care of Older People Suffering from Multi-Morbidity

    • The lack of a cohesive health and social care is a well-known problem of significance for ageing people in general and frail older people in particular. Responsibility for organising and conducting social care and healthcare for the elderly rests on different principals in different countries but difficulties with organisational coordination and collaboration between professions and authorities in social care and healthcare is an extensive concern worldwide. Regardless of the distribution of responsibilities, collaboration and coordination structures are complex and often lead to problems. However, the gap in the coordination between different organisations and the collaboration between professions, implying that frail older people with major care needs still living in their own homes are pinched, has received hardly any recognition. By closely following an implementation project focused on teamwork in order to improve collaboration and coordination between social care and healthcare, the purpose of this article is to fill this gap with the help of an example from Sweden. Data consisted of event diaries, observations, focus groups, structured questionnaires and interviews. Findings showed that multi-professional teams certainly were established, but did not work or last. Among the obstacles found the most prominent features were the various professions’ own organisations, territorial thinking and rivalries. The whole idea of the initiative to achieve a cohesive healthcare and social care for ageing frail older people fell through. By letting this happen, not only did the project hinder the development of better practice in serving older adults, but also cemented the problematic structures it was intended to dissolve.

  • Family caregivers and decision-making for older people with dementia

    • This article addresses the dilemmas concerning legislation, individual autonomy and the reality of everyday life for people coping with dementia. We describe and analyse decision-making in relation to older people with dementia in Sweden, within the area of social work regulated by the Social Services Act and the Parental Act. Swedish legislation is based on the individual's autonomy and capacity to consent to services without anyone having legal authority to decide on behalf of the individual. Based on data from interviews with family caregivers living at home, decision-making through family caregivers is discussed and formal guardianship is also considered. Swedish legislation leaves individuals with dementia and family caregivers in a vacuum between self-determination and full autonomy with the ideal of citizenship emphasised and recognised in the Social Services Act on the one hand, and on the other, a strong need for support in everyday life and with decision-making.

  • Personlig assistans och föräldraskap : Barnperspektiv i personlig assistans

    • I rapporten redovisas en studie om personlig assistans och föräldraskap i situationen då föräldrar med minderåriga barn har funktionsnedsättningar. Studien har genomförts med forskningsmedel från Försäkringskassans öppna utlysning. Syftet med studien var att undersöka vilket stöd föräldrar som har personlig assistans behöver för att kunna utöva föräldraskap på lika villkor som andra. Mer specifikt var avsikten att undersöka hur stöd i föräldraskap som ges genom personlig assistans utformas. Rapporten bygger på två delstudier. Den ena delstudien bygger på dokumentanalyser av ett slumpmässigt urval av Försäkringskassans utredningar (N=100) av rätten till assistansersättning för föräldrar med barn under 18 år. Den andra delstudien är en intervjustudie som bygger på intervjuer med föräldrar (N=11) och personliga assistenter (N=9) om deras erfarenheter av personlig assistans i föräldraskap.Resultaten av dokumentstudien visar att det i 31 procent av akterna inte hade dokumenterats att den sökande hade minderåriga barn. Av de sökande beviljades 26 procent assistansersättning och av dessa beviljades 9 procent specifik assistanstid för att vara föräldrar. Det var en högre andel män än kvinnor som fick rätt till assistansersättning. Analyser av handläggningsprocessen visar att inte alla sökande hade fått möjlighet att vid ett möte med handläggare beskriva sina behov. En förklaring till att få sökande fick rätt till assistans i föräldraskap var att föräldraskap inte inkluderas i definitionen av grundläggande behov, utan betraktas som ett annat personligt behov, vilket kan bli aktuellt först om den sökande bedöms ha grundläggande behov om i genomsnitt mer än 20 timmar per vecka.Av ansökningarna framgick att det fanns dokumentation om upplevda behov av assistans i föräldraskap. Behoven beskrevs av föräldrarna själva och intygas av professionella inom vården. Likartade behov har beskrivits av föräldrar och personliga assistenter i intervjustudien. Det handlar om behov av assistans med praktisk hjälp såsom hushållsarbete, omsorg och tillsyn av yngre barn, samt assistans för att kunna vara delaktig i barnens vardagsliv och kunna ge dem stimulans och aktivitet. Dessa identifierade områden kan förstås som grundläggande för barns omsorg och överensstämmer med de behov som tas upp i Socialstyrelsens handläggningsstöd Barns behov icentrum (BBIC), vilket används vid socialtjänstens barnutredningar. Det var också flera av de intervjuade föräldrarna som tog upp att omsorg om barn borde ingå i definitionen av grundläggande behov. Studien har visat exempel på att när samhällets insatser brister, och det inte heller finns ett stödjande nätverk som kan träda in, händer det att såväl minderåriga barn som unga vuxna får ta på sig uppgiften att ge omsorg till sina föräldrar. Studien visar på behovet av implementering av barnperspektiv i LSS där hänsyn tas till denna grupp föräldrar och barn.

    • The austerity measures in social welfare implemented by street-level bureaucrats in their practice are changing the direction of social policy. This article examines the strategies used by public officials in implementing austerity measures in needs assessment for personal assistance in Sweden. The article is based on a document study with N = 100 records of needs assessment for personal assistance for persons with serious functional disabilities. Findings show that the public officials at the Swedish Social Insurance Agency either limit or extend their discretion as a strategy, as well as use weak warrants as strong warrants in recommending decisions. Other strategies involve the organisational logic, fragmentation of the content in the needs assessment and division of work among the public officials. Implementing the cost-cutting goals causes severe consequences for persons with disabilities who bear the brunt by being excluded from participation in society. The social policy values of fifty years, emphasising the right to equal participation in society, are traded for economic austerity goals.
