  • An agent-based simulation model for assessment of prehospital triage policies concerning destination of stroke patients

    • Stroke is a life threatening medical condition that is caused either by a blood clot or a bleeding inside the brain. It is generally agreed that immediate treatment of stroke patients is crucial for their ability to recover; however, before treatment can be initiated, the stroke patient has to be diagnosed using, for example, a computed tomography scan of the brain. This, in turn, requires that the patient is transported to a hospital for diagnosis, which is a procedure that consumes valuable time and resources. In the current paper, we present an agent-based simulation model for assessment of logistical stroke patient triage policies. Using a population of stroke patients as input, the model enables to simulate the activities and the main decisions involved in the logistical operations of stroke patients. In an experimental study covering the southernmost part of Sweden, we used the simulation model in order to assess two different policies regarding where to send patients for diagnosis; the nearest hospital policy and the nearest hospital towards the stroke center policy. Our simulation results show that the patients in need of special treatment favor from being transported in direction towards the stroke center, whereas the patients who are not in need of specialist treatment favor from being sent to the closest hospital.

  • Infraposition of anterior maxillary implant-supported single-tooth restorations in adolescent and adult patients-A prospective follow-up study up to 6 years

    • Background In the maxillary incisor area infraposition of implant restorations can be an esthetic problem. Purpose The aim was to analyze the prevalence of infraposition of single-tooth implant restorations in the anterior maxilla in adolescent and adult patients. Material and Methods The study comprised of 31 patients, 18 women and 13 men, with 39 restorations and with a mean and median age of 23.8 and 18.8 years, range 17.8 to 52.8 years, at the time of implant crown connection. The mean and median time in function for the restorations was 4.5 and 4.3 years respectively with a range of 3.3 to 6.6 years. Results Twenty restorations 51%, showed no signs of infraposition. A slight infraposition (<0.5 mm) was seen in 36% of the restorations. Only in one patient, the observed infraposition of the implant crown was more than 1 mm. We found no significant difference of the observed level of vertical infraposition, Score A (0 mm) and Score B (<0.5 mm), in subjects where the restoration was placed before the age of 20 years compared to the older patients. There were no significant differences in changes of the vertical infraposition when comparing different parameters such as age, gender, jaw relation (Angle-classes I-III), overbite/overjet, occlusal contact on the implant crown, bone augmentation before or at implant installation or if orthodontic treatment of the neighboring teeth had been made before or after the implant installation. Conclusions In the present study, there was a significant yet low correlation between the observed vertical infraposition of the single-tooth implant restoration and the subject's evaluation of the esthetics of the restoration (VAS). Fifty-one percent of the implant-supported crowns, showed no signs of vertical infraposition. A slight infraposition (< 0.5 mm) was however seen in 36% of the restorations. Only one crown showed a vertical infraposition of more than 1 mm.

  • Skin hydration dynamics investigated by electrical impedance techniques in vivo and in vitro

    • Skin is easily accessible for transdermal drug delivery and also attractive for biomarker sampling. These applications are strongly influenced by hydration where elevated hydration generally leads to increased skin permeability. Thus, favorable transdermal delivery and extraction conditions can be easily obtained by exploiting elevated skin hydration. Here, we provide a detailed in vivo and in vitro investigation of the skin hydration dynamics using three techniques based on electrical impedance spectroscopy. Good correlation between in vivo and in vitro results is demonstrated, which implies that simple but realistic in vitro models can be used for further studies related to skin hydration (e.g., cosmetic testing). Importantly, the results show that hydration proceeds in two stages. Firstly, hydration between 5 and 10 min results in a drastic skin impedance change, which is interpreted as filling of superficial voids in skin with conducting electrolyte solution. Secondly, a subtle impedance change is observed over time, which is interpreted as leveling of the water gradient across skin leading to structural relaxation/changes of the macromolecular skin barrier components. With respect to transdermal drug delivery and extraction of biomarkers; 1 h of hydration is suggested to result in beneficial and stable conditions in terms of high skin permeability and extraction efficiency.

  • Telemedicine for Remote Surgical Guidance in Endoscopic Retrograde Cholangiopancreatography : Mixed Methods Study of Practitioner Attitudes

    • BACKGROUND: Telemedicine innovations are rarely adopted into routine health care, the reasons for which are not well understood. Teleguidance, a promising service for remote surgical guidance during endoscopic retrograde cholangiopancreatography (ERCP) was due to be scaled up, but there were concerns that user attitudes might influence adoption.OBJECTIVE: Our objective was to gain a deeper understanding of ERCP practitioners' attitudes toward teleguidance. These findings could inform the implementation process and future evaluations.METHODS: We conducted semistructured interviews with ERCP staff about challenges during work and beliefs about teleguidance. Theoretical constructs from the technology acceptance model (TAM) guided the thematic analysis. Our findings became input to a 16-item questionnaire, investigating surgeons' beliefs about teleguidance's contribution to performance and factors that might interact with implementation.RESULTS: Results from 20 interviews with ERCP staff from 5 hospitals were used to adapt a TAM questionnaire, exchanging the standard "Ease of Use" items for "Compatibility and Implementation Climate." In total, 23 ERCP specialists from 15 ERCP clinics responded to the questionnaire: 9 novices (<500 ERCP procedures) and 14 experts (>500 ERCP procedures). The average agreement ratings for usefulness items were 64% (~9/14) among experts and 75% (~7/9) among novices. The average agreement ratings for compatibility items were somewhat lower (experts 64% [~9/14], novices 69% [~6/9]). The averages have been calculated from the sum of several items and therefore, they only approximate the actual values. While 11 of the 14 experts (79%) and 8 of the 9 novices (89%) agreed that teleguidance could improve overall quality and patient safety during ERCP procedures, only 8 of the 14 experts (57%) and 6 of the 9 novices (67%) agreed that teleguidance would not create new patient safety risks. Only 5 of the 14 experts (36%) and 3 of the 9 novices (33%) were convinced that video and image transmission would function well. Similarly, only 6 of the 14 experts (43%) and 6 of the 9 novices (67%) agreed that administration would work smoothly. There were no statistically significant differences between the experts and novices on any of the 16 items (P<.05).CONCLUSIONS: Both novices and experts in ERCP procedures had concerns that teleguidance might disrupt existing work practices. However, novices were generally more positive toward teleguidance than experts, especially with regard to the possibility of developing technical skills and work practices. While newly trained specialists were the main target for teleguidance, the experts were also intended users. As experts are more likely to be key decision makers, their attitudes may have a greater relative impact on adoption. We present suggestions to address these concerns. We conclude that using the TAM as a conceptual framework can support user-centered inquiry into telemedicine design and implementation by connecting qualitative findings to well-known analytical themes.

  • Computational phenotyping of obstructive airway diseases: protocol for a systematic review

    • Background: Over the last decade, computational sciences have contributed immensely to characterization of phenotypes of airway diseases, but it is difficult to compare derived phenotypes across studies, perhaps as a result of the different decisions that fed into these phenotyping exercises. We aim to perform a systematic review of studies using computational approaches to phenotype obstructive airway diseases in children and adults.Methods and analysis: We will search PubMed, Embase, Scopus, Web of Science, and Google Scholar for papers published between 2010 and 2020. Conferences proceedings, reference list of included papers, and experts will form additional sources of literature. We will include observational epidemiological studies that used a computational approach to derive phenotypes of chronic airway diseases, whether in a general population or in a clinical setting. Two reviewers will independently screen the retrieved studies for eligibility, extract relevant data, and perform quality appraisal of included studies. A third reviewer will arbitrate any disagreements in these processes. Quality appraisal of the studies will be undertaken using the Effective Public Health Practice Project quality assessment tool. We will use summary tables to describe the included studies. We will narratively synthesize the generated evidence, providing critical assessment of the populations, variables, and computational approaches used in deriving the phenotypes across studiesConclusion: As progress continues to be made in the area of computational phenotyping of chronic obstructive airway diseases, this systematic review, the first on this topic, will provide the state of the art on the field and highlight important perspectives for future works.Ethics and dissemination: No ethical approval is needed for this work is based only on the published literature and does not involve collection of any primary or human data.

  • Views of Swedish Elder Care Personnel on Ongoing Digital Transformation : Cross-Sectional Study

    • Background: Swedish municipalities are facing demographic challenges due to the growing number of older people and the resulting increased need for health care services. Welfare technologies are being launched as possible solutions for meeting some of these challenges.Objective: The aim of this study was to explore the perception, experimentation, evaluation, and procurement of welfare technology practices among professionals working in municipal elder care in relation to their gender, age, and profession.Methods: Data for this explorative cross-sectional study were collected from 393 responses to a web-based survey on municipal elder care in Sweden. Chi square tests were performed to determine the associations.Results: The results revealed gender, age, and professional differences in perspectives of municipal elder care workers. Differences were particularly evident in attitudes toward technology, both the use of technology in general and in the workplace, and involvement and participation in decision making regarding the procurement of new welfare technologies. Men (37/53, 70%) expressed a more positive attitude toward and curiosity regarding new technologies than women (157/336, 46.7%) (P=.03). Regarding age, the younger respondents (18-24 years old) perceived the digital transformation in the workplace as "too slow" (4/4, 100%), whereas the majority of older respondents (65-74 years old) perceived it as happening at the "right pace" (4/7, 57%). The elder care personnel felt encouraged by management to explore and experiment with new welfare technologies, but never did so either for management or with patients. Even though the majority of the respondents were women, more men (4/7, 57%) were involved in the procurement process for welfare technology devices and solutions than women (98/336, 29.2%) (P<.001).Conclusions: Personnel working within municipal elder care were generally very positive toward new technologies. However, both gender and age differences may influence these perspectives such as the personnel's resistance to welfare technology and patients' participation in welfare technology usage and deployment. Different levels of participation in the decision-making process regarding new technology deployment may negatively affect the overall digital transformation within municipal elder care.

    • The pedagogical use of multisensory environments (MSE/Snoezelen) addresses the fundamentals of engagement in the world through rich, wondrous and sensuous experiences. Despite the diversity of artefacts and materials used in these practices, interactive designs are few, screen-centric or limited to simplistic behaviour.Twenty-four children with profound developmental disabilities from three MSE institutions have together with us and pedagogues explored potentials in interactivity for MSE. From a suite of 17 interactive designs, we will describe the three we will demo.

    • The Timed-Up and Go test is a simple yet effective test used to evaluate balance and mobility in conditions that affect movement, such as Parkinson’s disease. This test can inform clinicians about the monitoring and progression of the disease by measuring the time taken to complete the test. We used a smartphone app to obtain the phone’s inertial data and implemented an algorithm to automatically extract the time taken to complete the test. We considered data collected from six healthy participants performing tests at different speeds. The proposed method was further tested on twelve participants with Parkinson’s disease based on a reference measurement in clinic. We show that, for both groups, we obtain good accuracy (RMSE = 3.42 and 1.95 s) and a strong positive correlation (r = 0.85 and 0.83) between estimated duration and ground truth. We highlight limitations in our approach when the test is performed at very low speed or without a clear pause between the test and the user interaction with the phone.

    • In this paper we present a feature descriptor, based on a Markov random field (MRF) texture model, for radio-frequency (RF) ultrasound data. The proposed approach combines global data statistics in terms of a maximum-likelihood-estimated (MLE) distribution with local pattern characteristics employing MRF interaction parameters. This combining approach facilitates the encoding of the underlying nature of the ultrasound envelope data and therefore represents a powerful feature descriptor. Applicability and performance is showcased on RF data from a human neck.

    • Double-sided three-dimensional porous silicon chips, 6 mm x 6 mm, covered with a 40 nm gold (nano)layer, were fabricated from a porous silicon wafer. Scanning electron microscopy along with electrochemical characterisation showed sample conductivity, mechanical stability, and high surface area of the thus fabricated devices, viz. 10 times higher electrochemically active surface area compared to the geometric area. The three-dimensional gold coated silicon chips were further modified with thiol layers, followed by immobilisation of a simple copper-containing redox protein, azurin, or a complex multicopper redox enzyme, laccase. The bioelectrochemical studies showed very high surface concentrations of azurin and laccase, i.e. close to the theoretical monolayer coverage. However, direct electron transfer reactions between the biomolecules and gold surfaces were observed only for a small percentage of the immobilised redox protein and enzyme, respectively. Thus, highly efficient oxygen-bioelectroreduction on laccase-modified 3D thiol-gold-porous silicon chips (as compared to planar laccase-modified gold electrodes, 42 mu A/cm(2) vs. 7 mu A/cm(2), respectively) was obtained only in the presence of an efficient soluble redox mediator.
