  • Real-Time and Offline Evaluation of Myoelectric Pattern Recognition for the Decoding of Hand Movements

    • Pattern recognition algorithms have been widely used to map surface electromyographic signals to target movements as a source for prosthetic control. However, most investigations have been conducted offline by performing the analysis on pre-recorded datasets. While real-time data analysis (i.e., classification when new data becomes available, with limits on latency under 200-300 milliseconds) plays an important role in the control of prosthetics, less knowledge has been gained with respect to real-time performance. Recent literature has underscored the differences between offline classification accuracy, the most common performance metric, and the usability of upper limb prostheses. Therefore, a comparative offline and real-time performance analysis between common algorithms had yet to be performed. In this study, we investigated the offline and real-time performance of nine different classification algorithms, decoding ten individual hand and wrist movements. Surface myoelectric signals were recorded from fifteen able-bodied subjects while performing the ten movements. The offline decoding demonstrated that linear discriminant analysis (LDA) and maximum likelihood estimation (MLE) significantly (p < 0.05) outperformed other classifiers, with an average classification accuracy of above 97%. On the other hand, the real-time investigation revealed that, in addition to the LDA and MLE, multilayer perceptron also outperformed the other algorithms and achieved a classification accuracy and completion rate of above 68% and 69%, respectively.

    • Eight ECG beat detection algorithms, from the PhysioNet's WFDB and Cardiovascular Signal toolboxes, were tested on twenty measurements, obtained by the Savvy patch ECG device, for their accuracy in beat detection. On each subject, one measurement is obtained while sitting and one while running. Each measurement lasted from thirty seconds to one minute. The measurements obtained while running were more challenging for all the algorithms, as most of them almost perfectly detected all the beats on the measurements obtained in sitting position. However, when applied on the measurements obtained while running, all the algorithms have performed with decreased accuracy. Considering overall percentage of the faulty detected peaks, the four best algorithms were jqrs, from the Cardiovascular Signal Toolbox, and ecgpuwave, gqrs, and wqrs, from the WFDB Toolbox, with percentages of faulty detected beats 1.7, 2.3, 2.9, and 3, respectively.

    • Remote patient monitoring should reduce mortality rates, improve care, and reduce costs. We present an overview of the available technologies for the remote monitoring of chronic obstructive pulmonary disease (COPD) patients, together with the most important medical information regarding COPD in a language that is adapted for engineers. Our aim is to bridge the gap between the technical and medical worlds and to facilitate and motivate future research in the field. We also present a justification, motivation, and explanation of how to monitor the most important parameters for COPD patients, together with pointers for the challenges that remain. Additionally, we propose and justify the importance of electrocardiograms (ECGs) and the arterial carbon dioxide partial pressure (PaCO2) as two crucial physiological parameters that have not been used so far to any great extent in the monitoring of COPD patients. We cover four possibilities for the remote monitoring of COPD patients: continuous monitoring during normal daily activities for the prediction and early detection of exacerbations and life-threatening events, monitoring during the home treatment of mild exacerbations, monitoring oxygen therapy applications, and monitoring exercise. We also present and discuss the current approaches to decision support at remote locations and list the normal and pathological values/ranges for all the relevant physiological parameters. The paper concludes with our insights into the future developments and remaining challenges for improvements to continuous remote monitoring systems. [Figure not available: see fulltext.]

    • The atrial irregular rhythm, often reflected in atrial fibrillation, undulation or flutter, is recognized as one of the major causes of brain stroke and entails an increased risk of thromboembolic events because it increases the likelihood of blood clots formation. Its early detection is becoming an increasingly important preventive measure. The paper presents a simple methodology for the detection of atrial irregular rhythm by ECG body gadget that can perform long-term measurements, e.g. several weeks or more. Multichannel ECG, on the body surface, gives a more detailed insight into the atrial activity in comparison to standard 12-lead ECG. The information from MECG is compared with single-channel patch ECG. The obtained results suggest that the proposed methodology could be useful in treatments of atrial irregular rhythm. One can obtain a reliable information about the time and duration of fibrillation events, or determine arrhythmic focuses and conductive pathways in heart atria, or study the effects of antiarrhythmic drugs on existing arrhythmias and on an eventual development of new types of arrhythmias.

  • Forth heart sound detection using backward time-growing neural network

    • This paper presents a novel method for processing heart sound signal for screening forth heart sound (S4). The proposed method is based on time growing neural network with a new scheme, which we call the Backward Time-Growing Neural Network (BTGNN). The BTGNN is trained for detecting S4 in recordings of heart sound signal. In total, 83 children patients, referred to a children University hospital, participated in the study. The collected signals are composed of the subjects with and without S4 for training and testing the method. Performance of the method is evaluated using the Leave-One-Out and the repeated random sub sampling methods. The accuracy/sensitivity of the method is estimated to be 88.3%/82.4% and the structural risk is calculated to be 18.3% using repeated random sub sampling and the A-Test methods, respectively.

    • This paper presents a novel method for detecting cardiac ejection murmurs from other pathological and physiological heart murmurs in children. The proposed method combines a hybrid model and a time growing neural network for an improved detection even in mild condition. Children with aortic stenosis and pulmonary stenosis comprised the patient category against the reference category containing mitral regurgitation, ventricular septal defect, innocent murmur and normal (no murmur) conditions. In total, 120 referrals to a children University hospital participated to the study after giving their informed consent. Confidence interval of the accuracy, sensitivity and specificity is estimated to be 87.2%-88.8%, 83.4%-86.9% and 88.3%-90.0%, respectively.

    • In recent years, pandemic outbreaks have raised concerns about the spread of respiratory infections and their impact on public health. Since the pathogen emission during human respiration is recognized as the primary source, characterizing the physical properties of exhaled particles and airflow has become a crucial focus of attention. This article critically reviews experimental studies in exhaled particles and airflow, examines the uncertainty introduced by different measurement methods, analyzes how it is reflected in measurement outcomes, and provides an in-depth understanding of particle size distribution and airflow behaviors of human respiration. The measurement techniques assessment highlights the variability among particle sizing techniques in detection size range, collection efficiency, hydration status of captured particles, and experimental protocols. A combination of sampling-based instruments and laser imaging systems is recommended for particle sizing to cover a wider detection range, with refined setups in thermal conditions, sampling distance, volume, and duration. Meanwhile, it identifies the complementary nature of qualitative and quantitative measurements of airflow characterization techniques. Image recording systems plus data reconstruction programs are suggested to capture dynamic airflow features while accuracy validation by other techniques is required at the same time. Subsequent analysis of the measurement data showed that the various experimental measurements provided substantial information, but they also revealed disagreements and challenges in quantification. The dominance of submicron aerosols in exhaled particles and jet-like transport in exhaled airflow is obvious. More efforts should be made to measure particles larger than 20 μm, capture airflow dynamics in a high temporal and spatial resolution, and quantify the impact of face coverings to improve the understanding of human respiratory emissions.

  • Clinical sensor-based fall risk assessment at an orthopedic clinic : A case study of the staff’s views on utility and effectiveness

    • In-hospital falls are a serious threat to patient security and fall risk assessment (FRA) is important to identify high-risk patients. Although sensor-based FRA (SFRA) can provide objective FRA, its clinical use is very limited and research to identify meaningful SFRA methods is required. This study aimed to investigate whether examples of SFRA methods might be relevant for FRA at an orthopedic clinic. Situations where SFRA might assist FRA were identified in a focus group interview with clinical staff. Thereafter, SFRA methods were identified in a literature review of SFRA methods developed for older adults. These were screened for potential relevance in the previously identified situations. Ten SFRA methods were considered potentially relevant in the identified FRA situations. The ten SFRA methods were presented to staff at the orthopedic clinic, and they provided their views on the SFRA methods by filling out a questionnaire. Clinical staff saw that several SFRA tasks could be clinically relevant and feasible, but also identified time constraints as a major barrier for clinical use of SFRA. The study indicates that SFRA methods developed for community-dwelling older adults may be relevant also for hospital inpatients and that effectiveness and efficiency are important for clinical use of SFRA.

  • Wireless Wearable Measurement System Based on Pedobarography for Monitoring of Health

    • Health care costs have increased over the last decades due to an ageing population. Therefore, research in personal health monitoring (PHM) has increased in response to this. PHM has advantages such as mobility (monitoring of health at work or at home), early detection of health problems enabling preventive health measures and a reduction of health care cost. Human motion analysis, using for example inertial measurement units and pedobarography, is an important subcategory of PHM. Pedobarography (PBG) is the study of pressure fields acting between the plantar surface of the foot and a supporting surface. Gait and posture analysis, prosthetics evaluation and monitoring of recovery from injury or disease are examples of PBG applications. Portable PBG can be performed using force sensing resistors built into the insole inside the shoe.In accordance with this, the research goal for this licentiate thesis is to design, build and evaluate a wireless wearable measurement system based on pedobarography for monitoring of health. In order to fulfil the objectives of the research, literature studies were done and problems with existing in-shoe system solutions were identified. Thus, it was found that further opportunities existed for new designs of PBG systems which take these problems into account. Cross-sectional test case studies were used for validation. The research area is multidisciplinary and encompasses biomedical measurements, electronics and computer science.The main research contributions include design and implementation of a PBG measurement system consisting of commercial off the shelf components, a novel method for selecting measurement samples for weight estimation of carried load during walk, and a novel method for analysing walking intensity using force-time integrals at the toe-off phase of the step. The research results suggest that the new PBG system, in combination with the two novel analysing methods, are suitable for use in wearable systems for monitoring of health. Personal health measurements are done to help decision making related to health. Thus, the future work will strive towards designing different decision support systems.

    • This study explores what role science and technology parks (STP) can play when aiming to increase knowledge regarding the implementation of products for the health, social welfare, and medical technology sector (health-tech), including Industry 4.0 technology-based products (products), on a national level in Sweden. Such products, have problems reaching the market due to increased complexity from a purchase perspective. In this study, an STP developed a guideline on implementation-advice based on user-needs, literature studies, and focus groups. The guideline was presented at a seminar, where representatives from all regions in Sweden attended. The STP initiated the project, developed the guideline, and disseminated it successfully at a national level. The guideline was evaluated six months after the seminar, indicating that it created value in practice. This indicates that STPs could take an active role supporting products in the health-tech sector. New seminars are planned, and further studies are suggested.
