    • The MRI examinations of seven patients with acute transverse myelopathy (ATM) were analysed. The patients were examined 2-5 times during the course of their disease with short and long TR/TE spin-echo sequences in the sagittal projection. A previous history of autoimmune disorder and/or signs of infection at the onset of ATM were present in all cases. Cerebrospinal fluid analysis showed local synthesis of immunoglobulin in the nervous system in three cases and signs of infectious myelitis in one. During the acute phase four patients had local enlargement of the cord and all had increased signal on long TR/TE sequences. The outcome was grave in the majority of patients and there seemed to be a correlation between the degree of cord enlargement, persistence of increased signal intensity and limited recovery. Atrophy and remaining high signal intensity were noted on late MRI in patients with poor outcome. In one patient with probable anterior spinal artery occlusion, cavitation of the cord was seen.

  • Differences in fracture pattern between an urban and a rural population: a comparative population-based study in southern Sweden

    • Differences in the incidence of hip fractures have been reported between urban and rural areas. In this population-based study the characteristics of fracture patterns between the city of Malmo and the nearby rural district of Sjobo were compared. A total of 782 individuals in Malmo and 486 in Sjobo were invited to participate. Fracture history for all invited was registered. The odds ratio for fracture was higher in Malmo, particularly for women over 70. More than half of the urban women aged 70 had a history of a fracture. A fourfold increase in fracture prevalence between the ages of 60 and 70 was observed in women in Malmo, whereas the prevalence doubled in Sjobo. The differences in fracture patterns between these two urban and rural communities may be explained by different lifestyles.

  • Function 10 years after hip fracture. 74 patients after internal fixation

    • We examined the physical and social function of patients surviving 10 years after internal fixation of a hip fracture. 74 out of 362 patients were alive 10 years after their fracture. In 47 survivors after cervical fracture, secondary hip arthroplasties had been performed in 25, and nails had been extracted in 13. The implants had been removed in 8 of 27 with trochanteric fracture. 58 survivors were interviewed; three fourths were still living in their own homes, with more than half needing no home assistance. However, limitation of activities, caused by the fracture, was experienced by more than one third of the survivors.

    • 40 patients with transversal or short oblique fractures of the proximal shaft of the fifth metatarsal bone (Jones fracture) were treated with full weight bearing with or without elastic bandage in 39 cases and plaster cast immobilization in one. 24 cases were acute traumatic fractures, 14 stress fractures and 2 refractures. After 17 (11-26) years, 33 of the fractures had healed primarily, evidence of refracture or delayed union was found in 7 and there were no non-unions. All but one of the patients were free of symptoms.

    • Twenty patients who had been treated with a silicone rubber trapezial implant for degenerative changes in the first carpometacarpal joint were evaluated radiologically and clinically. After a mean follow up of 4.5 years, 11 of the 20 required reoperation and removal of the prosthesis for persisting pain and weakness. In all hands in which the prosthesis remained radiographs showed bone resorption or carpal bone cysts typical of silicone particle synovitis. In the light of these results we now do not recommend this procedure for the treatment of arthritis of the first carpometacarpal joint.

    • Six girls aged 6--10 years meeting criteria for autistic disorder are presented. They were all high-functioning in that they had IQs ha the 60-100 range and spoke in complicated sentences. Their clinical picture had not led a variety of specialists to consider an autism diagnosis before age 6 years in spite of the fact that they had shown mild motor delays, uncharacteristic developmental deviance and social, communicative and imaginative deficits already before age 2 years in all cases. It is suggested that the autism phenotype might be different in girls as compared with boys. It is further speculated that if female cases such as those described - and others given a whole host of different diagnostic labels - were only considered for an autism diagnosis, the high male:female ratio traditionally encountered in autism might drop considerably.

    • Computer-assisted interpretation of resolution visual fields was performed in 147 normal subjects and 174 consecutive patients with known chiasmal or retrochiasmal lesion. The program identified 99% of normal subjects, and provided support for the correct diagnosis in 64% of patients with chiasmal and 83% of patients with retrochiasmal lesions. In conclusion, computer-assisted interpretation of resolution visual fields can provide considerable support to the final diagnostic decision in patients with lesions of the visual pathways.

    • 110 patients with non-operated on fracture of the lateral end of the clavicle were reviewed on an average 15 years after the injury. There were 73 undislocated Neer Type I fractures, 23 dislocated Type II fractures and 14 intraarticular Type III fractures. The patients' age at trauma was 36 (2-71) years. At follow-up 95 shoulders were asymptomatic. 15 shoulders had moderate pain and dysfunction and were rated fair; no patient had severe, residual shoulder disability. There were 10 nonunions, 8 of these asymptomatic. We conclude that fracture of the lateral end of the clavicle does not require operation.

  • Two major classes in the M protein family in group A streptococci

    • The M protein family of molecules in the group A streptococcus comprises a number of cell surface proteins that interact with the immune system of the host. One of the proteins in this family is the IgA receptor Arp4, which has C repeats similar to those that characterize the known M proteins. The streptococcal strain expressing Arp4 also expresses a second immunoglobulin-binding protein, Mrp4, which is shown here to be encoded by a gene located immediately upstream of the gene for Arp4. In addition to binding IgG, Mrp4 also binds fibrinogen, a property ascribed to M proteins. DNA sequence analysis demonstrated that the Mrp4 protein indeed is a member of the M protein family, but it was unexpectedly found to have a type of repeat that is identical to the A repeat described for FcRA76, a partially sequenced streptococcal Fc receptor. Purified FcRA76 was shown to bind fibrinogen and IgG, like Mrp4. These data show that the known molecules in the M protein family can be divided into two classes, A and C, according to the type of repeat region found. Hybridization studies with a panel of clinical isolates indicate that many streptococcal strains express class A and class C proteins, whereas some strains express only class C proteins. Class A molecules show amino-terminal sequence variation, like class C molecules, which suggests that proteins of both classes are targets for the immune response.

  • Why is congenital dislocation of the hip still missed? Analysis of 96,891 infants screened in Malmo 1956-1987

    • During 1956 through 1987, 96,891 children have been screened for neonatal hip instability according to the tests of Ortolani and Barlow. In 1956 through 1972 only 4/58, 759 (0.07 permille) were missed, whereas during 1980 through 1987, 12/19, 398 (0.6 permille) were missed. This increase is not caused by any formal alteration of the screening programme. The screening has prevented a late diagnosis in all children born in breech presentation and in all boys except one. General factors such as female sex and joint laxity imply an increased risk for being missed in the screening, whereas mechanical factors such as breech presentation and the primogeniture effect likely facilitate an early diagnosis in the screening. The time between birth and the first examination is also of some importance.
