    • This paper addresses the under-researched issue of stakeholder identification and engagement in problem structuring interventions. A concise framework is proposed to aid critical reflection in the design and reporting of stakeholder identification and engagement. This is grounded in a critical-systemic epistemology, and is informed by social identity theory. We illustrate the utility of the framework with an example of a problem structuring workshop, which was part of a green innovation project on the development of a technology for the recovery of rare metals from steel slag. The workshop was initially going to be designed to surface stakeholder views on the technology itself. However, it became apparent that a range of other strategic issues concerning the future of the site were going to impact on decision making about the use of steel slag. It therefore became important to evolve the agenda for the problem structuring, and this is where the critical-systemic approach made a difference. It enabled the workshop to be reframed as a community-based event looking at how the former steelworks site could be developed for new purposes. Evaluation of this problem structuring intervention revealed significant stakeholder learning about the issues needing to be accounted for, and a range of possible options for the development of the steelworks site were explored. The paper ends with a discussion of the utility of social identity theory for understanding the processes and outcomes of the workshop, and reflections are provided on its implications for operational research practice more generally.

  • Legitimising a Feminist Agenda : The #metoo petitions in Sweden 2017–2018

    • During 2017 and 2018, the #metoo hashtag united a global movement against sexual abuse and harassment. In Sweden, a large portion of the attention was given to the voices of working women, who organised and wrote petitions that were published in news media. Previous research has found that media reports of sexual abuse often focus on singular stories, rather than describing the underlying structural problems, and that the problem is often framed as an individual rather than structural problem. This article accounts for a qualitative content analysis of the first 28 published #metoo petitions in Sweden, with the goal of understanding how these framed the issue. In contrast to previous research, this study shows how the petitions established a coherent feminist explanatory framework that placed the problems on a structural level by focusing on work environments and framing demands in terms of general and perfectly reasonable human rights.

  • How can evidence-based interventions give the best value for users in social services? Balance between adherence and adaptations : a study protocol

    • Background: Using evidence-based interventions (EBIs) is a basic premise of contemporary social services (e.g., child and family social services). However, EBIs seldom fit seamlessly into a specific setting but often need to be adapted. Although some adaptions might be necessary, they can cause interventions to be less effective or even unsafe. The challenge of balancing adherence and adaptations when using EBIs is often referred to as the adherence and adaptation dilemma. Although the current literature identifies professionals’ management of this dilemma as problematic, it offers little practical guidance for professionals. This research aims to investigate how the adherence and adaptation dilemma is handled in social services and to explore how structured decision support can impact the management of the dilemma.Methods: The design is a prospective, longitudinal intervention with a focus on the feasibility and usefulness of the structured decision support. The project is a collaboration between academic researchers, embedded researchers at three research and development units, and social service organizations. A multi-method data collection will be employed. Initially, a scoping review will be performed, and the results will be used in the development of a structured decision support. The decision support will be further developed and tested during a series of workshops with social service professionals. Different forms of data—focus group interviews, questionnaires, and documentation—will be used on several occasions to evaluate the impact of the structured decision support. Qualitative and quantitative analysis will be performed and usefulness for practice prioritized throughout the study.Discussion: The study will contribute with knowledge on how the adherence and adaption dilemma is handled and experienced by social service professionals. Most importantly, the study will generate rich empirical data on how a structured decision support impacts professionals’ management of adherence and adaptions. The goal is to produce more strategic and context-sensitive implementation of EBIs in social service, which will increase value for service users.

  • Hur utmanar styrningen det strategiska arbetet? : En studie av strategisk ledning och styrning i Region Stockholm åren 2017 – 2019

    • Denna studie utgör slutrapporten av ett treårigt forskningsprojekt som genomförts i Region Stockholm under åren 2017 – 2019. Projektet är finansierat inom ramen för ett strategiskt samarbete mellan Region Stockholm och Stockholms universitet. Syftet med studien är att bidra med ökad förståelse om strategisk ledning och styrning i offentlig verksamhet utifrån teorier om hur långsiktiga samhällsvärden skapas. För att undersöka detta empiriskt har följande forskningsfrågor varit vägledande:·       Hur styrs Region Stockholm?·       Hur utmanar styrningen det strategiska arbetet i Region Stockholm?Den första forskningsfrågan handlar om hur Region Stockholm styrs i praktiken och den andra de utmaningar som kan uppstå när styrning och strategiskt arbete möter varandra i praktiken. Vi vet från tidigare forskning att all styrning kommer med konsekvenser som på olika sätt utmanar det strategiska arbetet. Det är detta vi har intresserat oss för i denna rapport. Det empiriska materialet är omfattande och består av två huvudsakliga delar – en dokumentstudie och en intervjustudie. För att tolka och analysera de empiriska resultaten har vi gått igenom tidigare forskning om strategisk ledning och styrning i offentlig verksamhet med fokus på teorier om hur långsiktiga samhällsvärden skapas. Teorigenomgången avslutas med att ett analytiskt ramverk formuleras vilket tar avstamp i den strategiska triangeln som är uppbyggd utifrån tre noder – samhällsvärde och mål, den auktoriserande instansen och den operativa verksamheten. Dessa tre noder behöver förstås i relation till varandra för att den offentliga organisationen ska kunna uppnå en god strategisk position som bidrar till och skapar hållbara samhällsvärden. Detta betyder att om det finns en välfungerande strategi ska det finnas ett flöde mellan de tre noderna eftersom alla noder är ömsesidigt beroende av varandra. Våra resultat visar att den allt mer standardiserade styrningen i regionen, det ökade fokuset på kvantitativa resultatmått och den allt tydligare politiska styrningen utmanar flödet mellan de tre noderna och därmed också möjligheterna till att skapa långsiktiga samhällsvärden. Våra resultat visar därtill att styrningen på flera sätt utmanar det strategiska arbetet till exempel genom otydlighet i hur verksamheten är organiserad och otydlig ansvarsfördelning, men också genom den stora mängden av styrdokument och styrformer, inkonsekventa målformuleringar, avsaknaden av indikatorer och bristande tillit mellan tjänstemän på olika nivåer, samt mellan politiker och tjänstemän.Avslutningsvis presenterar vi några frågor som Region Stockholm, och andra offentliga verksamheter, kan behöva ställa sig i relation till strategisk ledning och styrning i offentliga verksamheter. Vi berör bland annat frågan om vilken nivå den strategiska ledningen och styrningen ska läggas på? Om "one size fits all” är rätt väg att gå? Om det finns ett ökat behov av att styra samverkan? Samt om långsiktighet i det strategiska arbetet är en (o)möjlighet i offentlig sektor?

  • Getting caught between discourse(s) : hybrid choices in technology use at work

    • Winner (1977, Autonomous Technology, Cambridge, MA: MIT Press, 77), in defense of technology determinism, cautioned against 'throwing out the baby with the methodological bathwater'. His concern was that in so doing STS research would underplay, or be unable to account for, the effects that technology change does have on society. We similarly now find that powerful explanatory concepts like 'structural-discourse' have been largely expunged from the contemporary STS analytical lexicon; with consequences, we believe, for our ability as researchers to interpret and explain the rapid change we see in contemporary work places. In this paper we make the case for the continued use of a strong structural-discourse theory alongside other emergent forms of discourse. We show how workers, responding to conflicting and different types of discourse, produce varying hybrid responses-actions that react to and combine elements of emergent and structural discourses. Our work considers the implications of this finding for contemporary STS theory.

    • Drawing on contemporary and historical discourse around UK business schools and insights from the sociology of scientific knowledge, we argue that business schools should be understood and judged, not as they typically have been, as engines of knowledge production, but as engines of credibility production. Credibility, we argue, is central to the attractiveness of business schools to students and other key stakeholders and therefore credibility, and the mechanisms through which credibility are maintained, should be at the center of strategic thinking within business schools. We argue that over-reliance on funding from corporate sources can have profound consequences for the ability of schools to continue to produce credibility. This article focuses primarily on the experiences of business schools in the UK.

  • Factors Impacting Clinicians' Adoption of a Clinical Photo Documentation App and its Implications for Clinical Workflows and Quality of Care : Qualitative Case Study

    • BACKGROUND: Mobile health (mHealth) tools have shown promise in clinical photo and wound documentation for their potential to improve workflows, expand access to care, and improve the quality of patient care. However, some barriers to adoption persist. OBJECTIVE: This study aims to understand the social, organizational, and technical factors affecting clinicians' adoption of a clinical photo documentation mHealth app and its implications for clinical workflows and quality of care. METHODS: A qualitative case study of a clinical photo and wound documentation app called imitoCam was conducted. The data were collected through 20 in-depth interviews with mHealth providers, clinicians, and medical informatics experts from 8 clinics and hospitals in Switzerland and Germany. RESULTS: According to the study participants, the use of mHealth in clinical photo and wound documentation provides numerous benefits such as time-saving and efficacy, better patient safety and quality of care, enhanced data security and validation, and better accessibility. The clinical workflow may also improve when the app is a good fit, resulting in better collaboration and transparency, streamlined daily work, clinician empowerment, and improved quality of care. The findings included important factors that may contribute to or hinder adoption. Factors may be related to the material nature of the tool, such as the perceived usefulness, ease of use, interoperability, cost, or security of the app, or social aspects such as personal experience, attitudes, awareness, or culture. Organizational and policy barriers include the available clinical practice infrastructure, workload and resources, the complexity of decision making, training, and ambiguity or lack of regulations. User engagement in the development and implementation process is a vital contributor to the successful adoption of mHealth apps. CONCLUSIONS: The promising potential of mHealth in clinical photo and wound documentation is clear and may enhance clinical workflow and quality of care; however, the factors affecting adoption go beyond the technical features of the tool itself to embrace significant social and organizational elements. Technology providers, clinicians, and decision makers should work together to carefully address any barriers to improve adoption and harness the potential of these tools.
