Organizational boundaries of home care services in Swedish old-age care : The 6th European Doctoral Conference in Nursing Science Maastricht, The Netherlands

- Introduction The 1990s was a decade of restructuring of old-age care in Sweden. The aim of the study was to identify and describe new dominant organizational models of home care services in municipal old-age care and to find patterns in the distribution of those models. Methods Data were collected during the summer of 2002. Directors of the social welfare services in all Swedish municipalities and municipality district councils received a questionnaire containing questions about old-age care organization, especially home care services. Rate of response =69%. Results Organization of home care services differed with regard to the division of responsibility between local (municipality) and regional (county council) authorities. Three dominant organizational models were identified. 1. Municipalities provided all home care services, including district nurses 52%. 2. Municipalities provided home care services, including assistant nurses, and county councils provided district nurses 20%. 3. Municipalities provided only home-help services and county council provided all home care services 28%. County councils tended to form clusters of municipalities with the same organization of home care services. Different variants of advanced home care services (substitute to hospital care) were represented in 69% of the municipalities. The presence of the variants differed in the three types of municipalities Conclusion Despite municipalities� general inclination to reorganize, the organization of municipal home care services tends to be stable over time since the implementation of a major old-age care reform 1992. Moreover, municipalities within the same county council region tend, to a remarkably high degree, to choose the same organizational form of home care services.
