  • Frailty and comprehensive geriatric assessment organized as CGA-ward or CGA-consult for older adult patients in the acute care setting : a systematic review and meta-analysis

    • Background: With worldwide population aging, increasing numbers of people need hospital care. Evidence suggests comprehensive geriatric assessment (CGA) is superior to usual care.Objective: To summarize the evidence for the effects of CGA in frail and moderately frail patients compared with usual care in acute care settings.Data sources: CINAHL, PsycInfo, Cochrane Library, EMBASE, and PubMed were searched in October 2011, January 2013, and February 2015.Study eligibility: Randomized controlled trials.Participants: Older adults aged ≥ 65 years who were admitted to hospital with a complex condition, divided into frail and moderately frail groups.Intervention: CGA.Control: Usual care.Outcomes: Change in housing, personal activities of daily living (PADL), instrumental activities of daily living (IADL), readmission, cognitive function, depression, quality-of-life care-giver burden, and mortality.Study appraisal and synthesis: The grading of recommendations assessment development and evaluation (GRADE) system to assess the quality of evidence and PRISMA-guidelines for meta-analyses and reviews. Continuous data were presented as standardized mean differences and dichotomous data were presented as risk differences.Results: Twenty-nine articles based on 17 unique studies (6005 patients in total). CGA was categorized as CGA-ward or CGA-consult. In the frail group, CGA-ward was superior to usual care for change in housing, PADL, and depression. CGA-consult was superior to usual care for PADL and IADL in the moderately frail group.Conclusion: There was a stronger effect for frail older adults and CGA-ward compared with usual care. This highlights the importance of detecting frailty. However, the degree of evidence was limited.

  • Interventions for improving mental health in athletes : a scoping review

    • The aims of this scoping review were to map the current literature on interventions for improving mental health in athletes, identify knowledge gaps, and generate future research questions. The Preferred Reporting Items for Systematic Review and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR) guided this review. A systematic literature search was conducted in SPORTDiscus, PsycINFO, and SCOPUS and 44 intervention studies met the inclusion criteria. Results showed that 22 studies (50%) implemented cognitive behavioural principles, and the majority of these studies were influenced by various mindfulness programmes. Most studies (93%) included healthy athlete samples, and athletes aged 15–19 were the most examined age group (43%). Only three studies used clinical criteria in their sampling of participants and mediators were examined in two studies. The scarcity of studies examining mediators and subclinical or clinical samples revealed critical knowledge gaps in the literature. Furthermore, the critical appraisal showed that regardless of study design, most studies demonstrated low internal validity. We propose the use of high-quality single-case studies with athletes who experience subclinical or clinical mental health issues, and further investigation of mechanisms of change linking intervention components to outcomes of interest.

  • From concept drift to model degradation : An overview on performance-aware drift detectors

    • The dynamicity of real-world systems poses a significant challenge to deployed predictive machine learning (ML) models. Changes in the system on which the ML model has been trained may lead to performance degradation during the system’s life cycle. Recent advances that study non-stationary environments have mainly focused on identifying and addressing such changes caused by a phenomenon called concept drift. Different terms have been used in the literature to refer to the same type of concept drift and the same term for various types. This lack of unified terminology is set out to create confusion on distinguishing between different concept drift variants. In this paper, we start by grouping concept drift types by their mathematical definitions and survey the different terms used in the literature to build a consolidated taxonomy of the field. We also review and classify performance-based concept drift detection methods proposed in the last decade. These methods utilize the predictive model’s performance degradation to signal substantial changes in the systems. The classification is outlined in a hierarchical diagram to provide an orderly navigation between the methods. We present a comprehensive analysis of the main attributes and strategies for tracking and evaluating the model’s performance in the predictive system. The paper concludes by discussing open research challenges and possible research directions.

  • Facets and scales in river restoration : Nestedness and interdependence of hydrological, geomorphic, ecological, and biogeochemical processes

    • Although river restoration has increased rapidly, observations of successful ecological recovery are rare, mostly due to a discrepancy in the spatial scale of the impact and the restoration. Rivers and their ecological communities are a product of four river facets-hydrology, geomorphology, ecology and biogeochemistry-that act and interact on several spatial scales, from the sub-reach to the reach and catchment scales. The four river facets usually affect one another in predictable pathways (e.g., hydrology commonly controls geomorphology), but we show that the order in which they affect each other and can be restored varies depending on ecoregion and hydroclimatic regime. Similarly, processes at different spatial scales can be nested or independent of those at larger scales. Although some restoration practices are dependent of those at higher scales, other reach-scale restoration efforts are independent and can be carried out prior to or concurrently with larger-scale restoration. We introduce a checklist using the four river facets to prioritize restoration at three spatial scales in order to have the largest positive effect on the entire catchment. We apply this checklist to two contrasting regions-in northern Sweden and in southern Brazil-with different anthropogenic effects and interactions between facets and scales. In the case of nested processes that are dependent on larger spatial scales, reach-scale restoration in the absence of restoration of catchment-scale processes can frankly be a waste of money, providing little ecological return. However, depending on the scale-interdependence of processes of the river facets, restoration at smaller scales may be sufficient. This means that the most appropriate government agency should be assigned (i.e., national vs. county) to most effectively oversee river restoration at the appropriate scale; however, this first requires a catchment-scale analysis of feedbacks between facets and spatial scale interdependence.

  • The state of European nursing research : dead, alive, or chronically diseased? A systematic literature review

    • Background: Reviews of nursing research have suggested that most is descriptive; with no more than 15% providing strong evidence for practice. No studies have examined this from the perspective of nursing research conducted in Europe. Objective: The aim of this study was to review reports of European clinical nursing research in the top 20 nursing journals in 2010 to establish a baseline of nursing research activity in the year immediately prior to the launch of a European Science Foundation network to increase the proportion of intervention research in Europe. Methods: We identified eligible reports that were then data‐extracted by two independent reviewers. Disagreements were resolved through pair discussion and independent arbitration. We appraised and synthesized topics, methods, and the extent to which studies were programmatic. We synthesized data as proportions of study reports meeting our a priori categorization criteria. Results: We identified 1995 published reports and included 223 from 21 European countries, of which 193 (86.6%) reported studies of primary research only, 30 (13.5%) secondary research, and three (1.4%) a mix of primary and secondary. Methodological description was often poor, misleading, or even absent. One hundred (44.8%) articles reported observational studies, 87 (39.0%) qualitative studies. We found 26 (11.7%) articles reporting experimental studies, 10 (4.5%) of which were randomized controlled trials. We found 29 (13.0%) reports located within a larger program of research. Seventy‐six (34.1%) articles reported studies of nursing interventions. Linking Evidence to Action: European research in nursing reported in the leading nursing journals remains descriptive and poorly described. Only a third of research reports concerned nursing interventions, and a tiny proportion were part of a programmatic endeavor. Researchers in nursing must become better educated and skilled in developing, testing, evaluating, and reporting complex nursing interventions. Editors of nursing journals should insist on systematic reporting of research designs and methods in published articles.

  • Initiatives Addressing Precarious Employment and Its Effects on Workers' Health and Well-Being : A Systematic Review

    • The prevalence of precarious employment has increased in recent decades and aspects such as employment insecurity and income inadequacy have intensified during the COVID-19 pandemic. The purpose of this systematic review was to identify, appraise, and synthesise existing evidence pertaining to implemented initiatives addressing precarious employment that have evaluated and reported health and well-being outcomes. We used the PRISMA framework to guide this review and identified 11 relevant initiatives through searches in PubMed, Scopus, Web of Science, and three sources of grey literature. We found very few evaluated interventions addressing precarious employment and its impact on the health and well-being of workers globally. Ten out of 11 initiatives were not purposefully designed to address precarious employment in general, nor specific dimensions of it. Seven out of 11 initiatives evaluated outcomes related to the occupational health and safety of precariously employed workers and six out of 11 evaluated worker health and well-being outcomes. Most initiatives showed the potential to improve the health of workers, although the evaluation component was often described with less detail than the initiative itself. Given the heterogeneity of the 11 initiatives regarding study design, sample size, implementation, evaluation, economic and political contexts, and target population, we found insufficient evidence to compare outcomes across types of initiatives, generalize findings, or make specific recommendations for the adoption of initiatives.

    • Introduction: Some therapies described within alternative and complementary medicine are advertised as sleep-promoting, including flotation-REST (Restricted Environmental Stimulation Technique). Flotation-REST induces deep relaxation through sensory isolation in a water-filled tank and is plausibly reported to mitigate insomnia problems, which have consistently been associated with stress, worry and arousal. However, the effects of flotation-REST have not been previously summarised. The aim of this systematic review was to investigate the efficacy of flotation-REST on sleep in clinical and non-clinical samples. Methods: A systematic search for studies on flotation-REST, involving at least one sleep-related variable, was conducted in the databases PubMed, Google Scholar, Web of Science and PsychINFO. Thirteen full-text articles met the inclusion criteria and were considered for eligibility. Nine were included in the current review. The methodological quality of the studies was assessed using a structured checklist, and a standard data extraction sheet was used to summarize the ratings. Results: In all included studies, flotation-REST demonstrated beneficial effects on sleep, both in clinical and nonclinical samples. In two studies, the effects were maintained 4 or 6 months post-treatment. The quality of the sleep outcome measures were, however, low in most studies, particularly regarding the participants' nightly sleep habits, self-reported sleep problems and insomnia diagnosis. Conclusions: Flotation-REST may be a promising treatment for insomnia symptoms, but more controlled studies with established sleep measures, and on populations with clinically verified insomnia, are needed.

    • Background: Patient participation is well understood by health care professionals but not many studies have focused on the older adults and their perceptions of patient participation.Aim and Objectives: To report an analysis of the concept of participation from the perspective of the older adult. Design: Concept analysis.Methods: An integrative review approach was undertaken and the searches were limited from January 2003 to December 2014, guiding question was; “what constitutes patient participation according to the older adult?”Results: Through the eyes of the older adults, a two-way communication should be initiated by the staff. Equality and sharing power between older adults and staffs was perceived as a precondition. Been given time was an essential issue, implying that older adults wished to have enough time from staffs and be in the right context surrounded by the appropriate environment in order to experience participation.Conclusion: In order to experience participation for older adults, it is important that the health care professionals are aware of how and in what ways they can contribute to participation among older adults. The need or wish to create participation is not enough; the health care professional needs to see and understand participation through the older adult’s eyes. Thus, a person-centered nursing approach is relevant for the health care professional in order to both give and maintain the experience of participation to the older adult.

    • The process of combining various parts to create a structure is called building. The most effective and significant component of any construction is masonry. The Colosseum, buildings from ancient Greece and Rome, Central American buildings, and Mycenaean structures all used this material as one of their primary building elements. The oldest form is dry masonry of irregularly shaped stones. The ecological qualities of masonry, as a restorative material with a low impact on the environment, as well as the environmental control capacity of the massive wall, bring masonry back to attention as a suitable material for sustainable building in the context of current concerns for sustainable architecture. This article takes the form of a review of the journey of masonry as the primary construction material—from prehistoric structures to modern-day edifices. This article will go through the fundamentals of masonry construction to support its usage in structures throughout history and in many architectural styles, as a crucial representation of human construction in architectural history. This article aims to create a historical review, presenting masonry as an essential building material and assessing its role in the history of building materials

    • To understand the school’s role in society and its works, it became essential to reevaluate its functions and importance for society after the aggressive attack of the COVID-19 pandemic. Thus, a new educational space design represents a powerful and required tool for stimulating creativity and increasing concentration, motivation, and assimilation of knowledge for future generations. The article will use appreciative inquiry as a method that works with perspective ideas readings doted by high positive human sensitivity. It also represents a powerful tool for the students’ opinions about the teaching spaces and environments. To improve the performance of educational institutions and schools, considering the sustainability concepts and biophilic designs has become an urgent necessity within the Scandinavian countries and in the world in general. The scientific research and theoretical analysis within the biophilic theory have been conducted to see how the designer can integrate the nature components holistically in the educational environment based on spatial, visual, and ecological integration concepts. The study aims to develop knowledge about applying biophilia as a phenomenon in educational institutes of Scandinavia where the students among others are the main decision-maker. The article’s main finding is that students dream of free open teaching spaces integrated with nature, where the biophilic theory frameworks are suitable to form this sustainable model that enables educational institutions and schools to improve their performance within different stages of the study.
