  • An elite endurance athlete's recovery from underperformance aided by a multidisciplinary sport science support team

    • Overload training resulting in an overreached state is common in elite sports, and if undetected can develop into an overtraining syndrome. This risk is accentuated by the lack of reliable measures of overreaching. Coaches and scientists therefore have to use a combination of tests in the monitoring process. This article presents a case study of the recovery from underperformance of a young elite endurance athlete and the work of a multidisciplinary sport science support team. When it was determined that the athlete's performance had deteriorated, and that this was due solely to the stress of training, training load was radically reduced for a period of 14 days. A combination of physiological, biochemical, and psychological measurements were then used to monitor the recovery process. The purpose of this article is to describe how coaches and sport science teams can help in monitoring training and recovery in practical settings, allowing detection of the early signs of overreaching before a more serious overtraining syndrome develops.

  • Competence development of registered nurses in municipal elderly care in Sweden : a questionnaire survey

    • Background: Skilled and specialist registered nurses (RNs) are central to evolving elderly care. The past decades' organisational and structural changes have altered RNs' roles and work situations in municipal elderly care in Sweden. This calls for appropriate educational preparation. However, a substantial proportion of RNs in municipal elderly care lack adequate specialist competence. Aim: The focus of this study was to describe RNs' perceptions of needs and possibilities for competence development in municipal elderly care and compare the perceptions of RNs' who work solely in dementia care (DC) with those who work in general elder care (GC) where older persons have diverse diagnoses. Design: A non-experimental, descriptive and comparative design was used. Settings: Sixty special housing with subunits in a large city in the middle of Sweden. Participants: Participating RNs were a total of 213, with a response rate of 62.3%. Of the 213 RNs, 95 (44.6%) worked in DC and 118 (55.4%) in GC. Method: A structured questionnaire that was specifically designed for this study and focused on needs and possibilities for competence development in nursing. Results: The RNs were on average not lacking or were hardly lacking knowledge in examined domains. However, RNs in GC lacked knowledge of dementia, falls, and fall injures to a greater extent than RNs in DC. RNs in DC perceived greater possibilities for competence development at work. Most RNs requested a better organisation for competence development, especially in GC. The majority of RNs had no supervision. The use of RNs' competence was high, although they used their highest competence about half of the working hours. The employers' financial contribution to RNs' continuing education was poor. Conclusion: A better organisation and greater possibilities for RNs' competence development is needed. The employers need to make a greater contribution financially to RNs' continuing education.     • We study the sewing constraints for rational two-dimensional conformal fieldtheory on oriented surfaces with possibly non-empty boundary. The boundarycondition is taken to be the same on all segments of the boundary.The following uniqueness result is established: For a solution to the sewingconstraints with nondegenerate closed state vacuum and nondegenerate two-pointcorrelators of boundary fields on the disk and of bulk fields on the sphere, upto equivalence all correlators are uniquely determined by the one-, two,- andthree-point correlators on the disk. Thus for any such theory every consistentcollection of correlators can be obtained by the TFT approach of [hep-th/0204148] and [hep-th/0503194].As morphisms of the category of world sheets we includenot only homeomorphisms, but also sewings; interpreting the correlatorsas a natural transformation then encodes covariance both under homeomorphismsand under sewings of world sheets.

  • A qualitative analysis of burnout in elite Swedish athletes

    • Objective: To extend the understanding of burnout in elite athletes, including personal experiences and perceived antecedents.Design and Methods: Semi-structured in-depth interviews were conducted with 10 Swedish 22- to 26-year-old elite athletes who had quit sport due to burnout. The interview transcripts were inductively analyzed using qualitative conten tanalysis.Results: The findings support the notion of athlete burnout as a multidimensional syndrome. While stressors like multiple demands, ‘‘too much sport,’’ lack of recovery and high expectations were considered primary causes of burnout by the respondents, high motivation, unidimensional athletic identity, self-esteem strivings, high ego goals, negative perfectionist traits and feelings of entrapment were also found to be critical contributors. These restraining factors explained why the athletes continued their participation in sport despite a progressive worsening of their condition, and are therefore potentially crucial in the development of burnout.Conclusion: Athlete burnout appears to be a complex interaction of multiple stressors, inadequate recovery and frustration from unfulfilled expectations, which is explained partly by maladaptive perfectionist traits and goals. This process is fuelled by a strong drive to validate self-worth, sometimes in conjunction with feelings of entrapment.

  • A comparative study of three different PCR assays for detection of Mycoplasma genitalium in urogenital specimens from men and women

    • The aim of this study was to compare conventional 16S rRNA gene PCR, real-time 16S rRNA gene PCR and real-time Mycoplasma genitalium adhesin protein (MgPa) gene PCR as detection methods for M. genitalium infection. The study also determined the prevalence of M. genitalium in male and female patients attending a sexually transmitted infections clinic in a rural area in the west of Sweden. First void urine (FVU) and/or urethral swabs were collected from 381 men, and FVU and/or cervical swabs and/or urethral swabs were collected from 298 women. A total of 213 specimens were used in the PCR comparative study: 98 consecutively sampled specimens from patients enrolled in the prevalence study, 36 consecutively sampled specimens from patients with symptoms of urethritis and 79 specimens from patients positive for M. genitalium by real-time MgPa gene PCR in the prevalence study. A true-positive M. genitalium DNA specimen was defined as either a specimen positive in any two PCR assays or a specimen whose PCR product was verified by DNA sequencing. The prevalence of M. genitalium infection in men and women was 27/381 (7.1 %) and 23/298 (7.7 %), respectively. In the PCR comparative study, M. genitalium DNA was detected in 61/76 (80.3 %) of true-positive specimens by conventional 16S rRNA gene PCR, in 52/76 (68.4 %) by real-time 16S rRNA gene PCR and in 74/76 (97.4 %) by real-time MgPa gene PCR. Real-time MgPa gene PCR thus had higher sensitivity compared with conventional 16S rRNA gene PCR and had considerably increased sensitivity compared with real-time 16S rRNA gene PCR for detection of M. genitalium DNA. Real-time MgPa gene PCR is well suited for the clinical diagnosis of M. genitalium.

  • Estimating inclusion content in high performance steels

    • Non-metallic inclusions in steel pose a major problem for the fatigue resistance, especially regarding fatigue at very long lives corresponding to low cyclic stress levels, as well as being detrimental to material toughness and polishability.The largest inclusions are quite rare, which makes conventional detection methods timeconsuming if reliable results are to be obtained. Based on surface scanning using light or electron microscopes, these methods provide results that have to be converted to reflect the statistical volume distribution of inclusions.Very high cycle fatigue (in the order of 109 cycles or more) using ultrasonic fatigue at 20 kHz has been found efficient at finding the largest inclusions in volumes of about 300 mm3 per specimen. The inclusions found at the fatigue initiation site can then been used to estimate the distribution of large inclusions using extreme value statistics.In this work, a new method for estimating the volume distribution of large inclusions is presented as well as a suggested ranking variable based on the volume distribution.Results from fatigue fractography and area scanning methods are compared to the endurance limit at 109 cycles for a number of batches from two high performance steels.In addition, the extreme value distributions of fatigue initiating inclusions in six high performace steels, produced by different routes, are presented. It is shown that all modes of the Generalized Extreme Values distribution can be found in different materials. This result shows that the assumption of mode I distribution, also known as Gumbel or Largest Extreme Value distribution, must be substantiated.

    • I 30 år har OECD trummat ut sitt budskap om behovet av strukturreformer på arbetsmarknaden. Receptet har varit likartat oavsett patient och ofta tycks förskrivningen ha skett enligt devisen ”ju beskare medicin, desto trovärdigare behandling”. Många blev därför förvånade när OECD, efter en ambitiös översyn av sin jobbstrategi, år 2006 gav upp tanken om en enda gyllene reformväg. Artikeln pekar på viktiga förändringar i den reviderade jobbstrategin och analyserar vilka faktorer som låg bakom OECD:s omsvängning i arbetsmarknadspolitiken.
