  • Stove performance and emission characteristics in residential wood log and pellet combustion Part 2: Wood stove

    • The characteristics and quantities of a large number of gaseous and particulate emission components during combustion in a residential wood log stove with variations in fuel, appliance and operational conditions were determined experimentally. The measurement campaign included CO, NOx, organic gaseous carbon (OGC), volatile organic compounds (VOCs), polycyclic aromatic hydrocarbons (PAHs), total particulate matter (PMtot) as well as particle mass and number concentrations, size distributions and inorganic composition. CO varied in the range of 1100 to 7200 mg/MJfuel, while OGC varied from 210 to 3300 mg/MJfuel. Dominating VOCs were methane, followed by ethene, acetylene and benzene. Methane varied from 9 to 1600 mg/MJfuel. The non-methane volatile organic compound (NMVOC) emissions were in the range of 20-2300 mg/MJfuel. The PAHtot emissions varied from 0.8 to 220 mg/MJfuel, in most cases dominated by phenantrene, fluoranthene and pyrene. PMtot were in all cases dominated by fine particles and varied in the range 38-350 mg/MJfuel. The mass median particle diameters and the peak mobility diameters of the fine particles varied in the range 200-320 nm and 220-330 nm respectively and number concentrations in the range of 1-4×1013 particles/MJfuel. Air starved conditions, at high firing intensity, gave the highest emissions, especially for hydrocarbons. This type of conditions is seldom considered, though it may occur occasionally. The emissions from Swedish wood stoves, comparing a Swedish field study, are covered fairly well with the applied methodology, but other field studies reports considerably higher emissions especially for diluted particle sampling.

    • The aim of this article is to discuss challenges in the development of Specialist Nursing Educations as a result of the 2007 Swedish Higher Education Reform: the implementation of the so-called Bologna process. Certain challenges follow this reform, particularly since the specialist nursing programmes will be part of the second cycle of the higher education system, and it will be possible to combine the professional degree with a masters degree (one year). Possible strategies in four areas related to the Specialist Nursing Education are discussed: integration of researchbased knowledge, experienced-based knowledge, improvement knowledge, and strategies for collaboration between university institutions and clinics. Specific didactical issues are raised.

  • A Swedish consensus on the surgical treatment of concomitant atrial fibrillation

    • Atrial fibrillation (AF) is a common arrhythmia among patients scheduled for open heart surgery and is associated with increased morbidity and mortality. According to international guidelines, symptomatic and selected asymptomatic patients should be offered concomitant surgical AF ablation in conjunction with valvular or coronary surgery. The gold standard in AF surgery is the Cox Maze III ("cut-and-sew") procedure, with surgical incisions in both atria according to a specified pattern, in order to prevent AF reentry circuits from developing. Over 90% of patients treated with the Cox Maze III procedure are free of AF after 1 year. Recent developments in ablation technology have introduced several energy sources capable of creating nonconducting atrial wall lesions. In addition, simplified lesion patterns have been suggested, but results with these techniques have been unsatisfactory. There is a clear need for standardization in AF surgery. The Swedish Arrhythmia Surgery Group, represented by surgeons from all Swedish units for cardiothoracic surgery, has therefore reached a consensus on surgical treatment of concomitant AF. This consensus emphasizes adherence to the lesion pattern in the Cox Maze III procedure and the use of biatrial lesions in nonparoxysmal AF.

  • A quality assessment survey of SNP genotyping laboratories

    • To survey the quality of SNP genotyping, a joint Nordic quality assessment (QA) round was organized between 11 laboratories in the Nordic and Baltic countries. The QA round involved blinded genotyping of 47 DNA samples for 18 or six randomly selected SNPs. The methods used by the participating laboratories included all major platforms for small- to medium-size SNP genotyping. The laboratories used their standard procedures for SNP assay design, genotyping, and quality control. Based on the joint results from all laboratories, a consensus genotype for each DNA sample and SNP was determined by the coordinator of the survey, and the results from each laboratory were compared to this genotype. The overall genotyping accuracy achieved in the survey was excellent. Six laboratories delivered genotype data that were in full agreement with the consensus genotype. The average accuracy per SNP varied from 99.1 to 100% between the laboratories, and it was frequently 100% for the majority of the assays for which SNP genotypes were reported. Lessons from the survey are that special attention should be given to the quality of the DNA samples prior to genotyping, and that a conservative approach for calling the genotypes should be used to achieve a high accuracy.

  • Small bowel biopsy in Swedish paediatric clinics.

    • Aim: A correct diagnosis of coeliac disease, one of the most common chronic diseases in Swedish children, demands small bowel biopsy, which can be performed endoscopically or by means of a peroral capsule. Recently there was a debate among Swedish paediatric gastroenterologists, with some advocating the cessation of capsule biopsy in favour of endoscopic biopsies. To gain information on which to base a recommendation for which technique to use, the Swedish Working Group for Childhood Coeliac Disease was commissioned to carry out a national questionnaire study on current small bowel biopsy routines in Swedish paediatric clinics. Methods: A questionnaire concerning biopsy routines in the year 2000 was sent to all paediatric clinics performing biopsies. A reply was obtained from 39 of 40 clinics, covering 98% of the Swedish population. Results: Some 1400 biopsies were performed, 64% of which were capsule biopsies and 36% endoscopic. Three clinics performed all biopsies endoscopically and 11 clinics all via a capsule. At endoscopy all children were under deep sedation or full anaesthesia, while most children undergoing capsule biopsy were under light or deep sedation. The oxygen saturation was monitored during endoscopy but less often or never during routine capsule biopsy. The presence of the parents during biopsy varied according to the degree of sedation: at 97% of the clinics performing capsule biopsy on children under light sedation, the parents were present during the whole procedure, whereas no parents were present at clinics where the biopsy was performed endoscopically under anaesthesia. Conclusion: Compared with the results of a similar questionnaire concerning biopsy routines performed in the early 1990s, children are now more effectively sedated, Furthermore, there is an obvious trend from capsule towards endoscopic biopsy. Both the endoscopic and the capsule biopsy techniques are useful and satisfactory for obtaining small bowel mucosal samples providing that the children are effectively sedated. For practical and economic reasons the capsule biopsy technique will probably continue to be used, although to a lesser extent than today.

    • The instrument Families’ Importance in Nursing Care–Nurses’ Attitudes (FINC-NA) was developed to measure nurses’ attitudes toward the importance of families in nursing care. The low variations in item responses, which affect the discrimination ability and unstable internal consistency, have been considered as limitations. The aim of this study was to refine and revalidate FINC-NA regarding score distribution, homogeneity, dimensionality, differential item functioning for gender, stability, and internal consistency. There were 246 registered nurses studying at advanced levels who answered the revised FINC-NA. The FINC-NA had five response alternatives. The findings showed that although some subscales still deviated from a normal distribution, the variability of the scores and the homogeneity was improved. In addition, the dimensionality was reproduced and minor problems with differential item functioning for gender were detected. All FINC-NA scales showed good reliability. The results allow the use of the revised FINC-NA in studies where an assessment of nurses’ attitudes toward families’ importance in nursing care is desired.

    • Objective: To provide strategies to assist healthcare professionals in the area of rehabilitation to improve prevention of falls. Design: A conceptual framework is described as a foundation for the proposal of 2 intertwined strategies, of intervention and implementation, which target the questions: Which strategies for intervention represent the current best evidence? and: How can these strategies be implemented and continuously developed? Results: Strategies for multifactorial and multiprofessional fall preventive interventions are presented in terms of a "fall prevention pyramid model", including general, individualized, and acute interventions. A systematic global fall risk rating by the staff is recommended as an initial procedure. Fall event recording and follow-up are stressed as important components of local learning and safety improvement. Development of implementation strategies in 3 phases, focusing on interaction, facilitation and organizational culture, is described Conclusion: A well-developed patient safety culture focusing on prevention of falls will, when successfully achieved, be seen by staff, patients and their significant others as being characteristic of the organization, and will be evident in attitudes, routines and actions. Moreover, it provides potential for positive side-effects concerning organizational and clinical improvements in additional areas

    • In an academic programme, completion of a postgraduate degree project could be a significant means of promoting student learning in evidence- and experience-based practice. In specialist nursing education, which through the European Bologna process would be raised to the master’s level, there is no tradition of including a postgraduate degree project. The aim was to develop a didactic model for specialist nursing students’ postgraduate degree projects within the second cycle of higher education (master’s level) and with a specific focus on nurturing shared involvement between universities and healthcare settings. This study embodies a participatory action research and theory-generating design founded on empirically practical try-outs. The 3-year project included five Swedish universities and related healthcare settings. A series of activities was performed and a number of data sources secured. Constant comparative analysis was applied. A didactic model is proposed for postgraduate degree projects in specialist nursing education aimed at nurturing shared involvement between universities and healthcare settings. The focus of the model is student learning in order to prepare the students for participation as specialist nurses in clinical knowledge development. The model is developed for the specialist nursing education, but it is general and could be applicable to various education programmes.

  • What is the word for 'Engineering' in Swedish : Swedish students conceptions of their discipline

    • Engineering education in Sweden – as in the rest of the world – is experiencing a decline in student interest. There are concerns about the ways in which students think about engineering education, why they join an academic programme in engineering, and why they persist in their studies. In this context the aims of the Nationellt ämnesdidaktiskt Centrum för Teknikutbildning i Studenternas Sammanhang project (CeTUSS) is to investigate the student experience and to identify and support a continuing network of interested researchers, as well as in building capacity for disciplinary pedagogic investigation.The Stepping Stones project brings together these interests in a multi-researcher, multi-institutional study that investigates how tudents and academic staff perceive engineering in Sweden and in Swedish education. The first results of that project are reported here. As this study is situated uniquely in Swedish education, it allows for exploration of “a Swedish perspective” on conceptions of engineering. The Stepping Stones project was based on a model of research capacity-building previously instantiated in the USA and Australia (Fincher & Tenenberg, 2006).

    • There are various ways in which context matters in ethics. Most clearly, the context in which an action is performed might determine whether the action is morally right: though it is often wrong not to keep a promise, it might be permissible in certain contexts. More radically, proponents of moral particularism (see particularism) have argued that a reason for an action in one context is not guaranteed to be a reason in a different context: whether it is a reason against an act that it breaks a promise or inflicts pain might depend on the particulars of the situation. In moral epistemology, Timmons (1999: Ch. 5) argues that whether a moral judgment is epistemically responsible depends both on the basic moral outlook of the moral judge and on whether the context of judgment is one of engaged moral thinking, or one of distanced, skeptical reflection. In the former, the judge’s basic moral outlook can serve to justify the judgment; not so in the latter (see epistemology, moral).Our focus here, however, will be on forms of metaethical, and more precisely semantic, contextualism in moral discourse and moral thinking. According to these forms of contextualism (henceforth “metaethical contextualism,” or just “contextu- alism”), the meaning or truth-conditions of a moral judgment depend not only on the properties of the act it concerns, but also on features of the context in which the judgment is made, such as the standards endorsed by the moral judge or the parties of the conversation. If metaethical contextualism is correct, it might be that when two persons judge that abortions must be banned, one person’s judgment might be true whereas the other person’s is false, because they accept different fundamental norms. This would undermine the idea that there are objectively correct answers to moral questions.Metaethical contextualism is supported from three directions. First, what is expressed by terms such as “good” and “ought” seems to be context-dependent when used outside ethics, being dependent on a variety of interests and concerns. One might therefore expect similar context dependence when these terms are used to express moral judgments, assuming a corresponding variety of interests and concerns in moral contexts. Second, many have thought that deep moral disagree- ments suggest that the interests and concerns behind moral judgments do vary in this way. Finally, contextualism promises to make sense of what seems to be an intrinsic yet defeasible connection between moral judgments and moral motivation, by tying the meaning or truth-conditions of moral judgments closely to interests and concerns of moral judges. At the same time, contextualism faces two broad kinds of problems: to make sense of the seemingly categorical or objective preten- sions of moral claims, and to explain why the parties to deep moral disagreement often behave as if they were disagreeing about substantive issues rather than talking past each other. In the sections that follow, we look closer at both sources of support and problems for contextualism.
