  • Unsupervised Learning from Motion Sensor Data to Assess the Condition of Patients with Parkinson’s Disease

    • Parkinson’s disease (PD) is a chronic neurodegenerative disorder that predominantly affects the patient’s motor system, resulting in muscle rigidity, bradykinesia, tremor, and postural instability. As the disease slowly progresses, the symptoms worsen, and regular monitoring is required to adjust the treatment accordingly. The objective evaluation of the patient’s condition is sometimes rather difficult and automated systems based on various sensors could be helpful to the physicians. The data in this paper come from a clinical study of 19 advanced PD patients with motor fluctuations. The measurements used come from the motion sensors the patients wore during the study. The paper presents an unsupervised learning approach applied on this data with the aim of checking whether sensor data alone can indicate the patient’s motor state. The rationale for the unsupervised approach is that there was significant inter-physician disagreement on the patient’s condition (target value for supervised machine learning). The input to clustering came from sensor data alone. The resulting clusters were matched against the physicians’ estimates showing relatively good agreement.

  • Using measurements from wearable sensors for automatic scoring of Parkinson's disease motor states : Results from 7 patients

    • The objective of this study was to investigate the validity of an objective gait measure for assessment of different motor states of advanced Parkinson's disease (PD) patients. Seven PD patients performed a gait task up to 15 times while wearing sensors on their upper and lower limbs. Each task was performed at specific points during a test day, following a single dose of levodopa-carbidopa. At the time of the tasks the patients were video recorded and three movement disorder experts rated their motor function on three clinical scales: a treatment response scale (TRS) that ranged from −3 (very bradykinetic) to 0 (ON) to +3 (very dyskinetic), a dyskinesia score that ranged from 0 (no dyskinesia) to 4 (extreme dyskinesia), and a bradykinesia score that ranged from 0 (no bradykinesia) to 4 (extreme bradykinesia). Raw accelerometer and gyroscope data of the sensors were processed and analyzed with time series analysis methods to extract features. The utilized features quantified separate limb movements as well as movement symmetries between the limbs. The features were processed with principal component analysis and the components were used as predictors for separate support vector machine (SVM) models for each of the three scales. The performance of each model was evaluated in a leave-one-patient out setting where the observations of a single patient were used as the testing set and the observations of the other 6 patients as the training set. Root mean square error (RMSE) and correlation coefficients for the predictions showed a good ability of the models to map the sensor data into the rating scales. There were strong correlations between the SVM models and the mean ratings of TRS (0.79; RMSE=0.70), bradykinesia score (0.79; RMSE=0.47), and bradykinesia score (0.78; RMSE=0.46). The results presented in this paper indicate that the use of wearable sensors when performing gait tasks can generate measurements that have a good correlation to subjective expert assessments.

    • In health and care service processes the coordination between different caregivers is one of the crucial challenges. This paper presents The patient journey project  as a practical application of a pragmatist pattern of inquiry (Dewey, 1938) and as fruitful way to work with/achieve transformative design. Situations of lived experience and moments of reflections perform as a carrier of knowledge and development. The paper argues that the design tools and mindset used in this project are of great importance in the ongoing transformation towards the patients focus in a Swedish public health care organization.

    • Public organisations need to rethink the ways they innovate and improve their services. Service design has become a means to achieve innovation capabilities through service design projects with the purpose of both enhancing innovation capabilities, and creating new service. Based on observations, interviews and project documentations this exploratory paper reflects on the role and articulation of design expertise, how and if it may be transferred through design projects. Further, the relation between learning by doing and learning through expert examples is discussed through a pragmatist lens.

    • Taking the example of the Algerian novelist Yasmina Khadra, the present study examines the intercultural discourse created by North African fiction addressing a Western reader.Considering a novel originally published in France in 2006, namely Les sirènes de Bagdad, the study suggests that already the choice of language and genre is an expression of the author’s approaching of the Western reader. Also, the Western reader is written into the story by didactic features and referential play. Examining Yasmina Khadra’s image and the rhetoric of his narrative, the study finds that the author produces intermediate acts of speech aiming to make certain aspects of Arabic culture intelligible to the Western reader. Judging from the reception in Europe and in the USA, the prose in question is generally perceived as a relevant and enlightening commentary to the situation in the Middle-East. However, some German critics seem reluctant towards the authority of the writer and to parts of the referential grounding. The study suggests that the differences of opinion in this respect depend on varying degrees of readiness to accept the designated role of the reader inherent to the enunciative situation.

    • Public sector organizations need to adapt to the ongoing societal changes and new technologies emerging, and as public sector organizations engage in digital transformation, they are confronted with the need to re-arrange and change themselves to be successful. Previous research has identified factors for digital transformation in both public and private sector settings, yet there is still an absence of research into how public sector organizations deal with this transformation. In this study, we explore how government agencies enact structural changes related to digital transformation. We do so through a multi-case study of three government agencies in Sweden, interviewing key actors to explore the organizations’ enactments. Our findings show that public sector organizations display a high level of variance in how they enact structural changes to succeed with digital transformation. This is discussed in relation to previous research on management commitment to digital transformation, as well as dialogue and tensions when changing, with the intent to contribute to research and practice in relation to digital transformation

    • The aim of this paper is to explicitly link co-design to well-being and expand the conversation about the influence of the co-design process on well-being. This paper highlights considerations for co-design researchers and practitioners interested in enhancing the value created through co-design. The authors draw from discussions in transformative service research (TSR) to better understand how co-design influences well-being. Co-design is defined as a process of joint inquiry and imagination where diverse actors share and combine their knowledge. Based on the broad definition of service set out in service- dominant logic (SDL), the authors take the position that co-design is a form of service and therefore stress the relevance of TSR to co-design. The paper identifies six dimensions of well-being discussed in TSR that extend and highlight gaps in co-design literature related to the influence of the co-design process on well-being. The authors suggest that these dimensions become a component of future evaluations of the co-design process and point to opportunities for further research related to how co-design influences well-being and supports transformation.
