[Quality of service provided to heart surgery patients of the Unified Health System-SUS]

- OBJECTIVE:To evaluate the service quality provided to heart surgery patients during their hospital stay, identifying the patient's expectations and perceptions. To associate service quality with: gender, age and the use of extracorporeal circulation.METHODS:We studied 82 elective heart surgery patients (52.4% females and 47.6% males), operated by midsternal thoracotomy, age: 31 to 83 years (60.4 +/- 13.2 years); period: March to September 2006. Service quality was evaluated in two instances: the expectations at pre-operative and the perceptions of the service received on the 6th post-operative; through the application of the modified SERVQUAL scale (SERVQUAL-Card). The result was obtained by the difference of the sum of the scores on perception minus those of the expectations, and through statistical analysis.RESULTS:The SERVQUAL-Card scale was statistically validated, showing adequate level of internal consistency. We found a higher frequency of myocardial revascularization 55 (67.0%); first heart surgery 72 (87.8%) and the use of ECC 69 (84.1%). We noticed high mean values for expectations and perceptions with significant results (P<0.05). We observed a significant relationship between the quality of service with: gender, in empathy (P= 0.04) and age, in reliability (P = 0.02). There was no significant association between ECC and quality of service.CONCLUSION:Service quality was satisfactory. The patient demonstrated a high expectation to hospital medical service. Women present a higher perception of quality in empathy and younger people in reliability. The use of ECC is not related to service quality in this sample. The data obtained in this study suggest that the quality of this health service can be monitored through the periodical application of the SERVQUAL scale.

- Participatory research is facing three challenges—how to deal with the theoretisation and conceptualisation of participation; how to support the research with analytical models; and how the evaluate the research outcomes. This article aims to address these three problems by distinguishing two main approaches (a sociological and a political) in participatory theory and developing a four-level and 12-step analytical model that functions within the political approach. In this analytical model, a series of key concepts are used: process, field, actor, decision-making moment and power. The normative-evaluative problem is addressed by reverting to the critical perspective to evaluate the societal desirability of particular participatory intensities. This critical perspective—potentially—adds a 13th and final normative layer to the analytical model.

- The article first discusses five approaches that aim to transcend, complement, or overturn the hegemony of the written academic text. In the second part, the Mirror Palace of Democracy installation experiment, which had the explicit objective of moving beyond the written academic text while still remaining in the realm of academic knowledge communication, is autoethnographically analyzed. The experiment allowed reflection on the integrated and iterative nature of academic communication, on the hybrid academic– artistic identity, and on the diversification of publics. Both the theoretical discussion and the Mirror Palace of Democracy installation are part of a call for more experimentation with, and theorization of, multimodal and/or arts-based academic communication.

- Como forma de evitar tanto abordagens teoricamente ecléticas, quanto abordagens teoricamente redundantes, este artigo propõe uma reconstrução possível e coerente do construtivismo, entendido como uma metateoria reflexiva. Esta reconstrução começa levando a sério a dupla virada sociológica e interpretivista nas ciências sociais. Baseado na “dupla hermenêutica”, o construtivismo pode ser mais bem compreendido distinguindo-se sua postura quanto ao nível da observação, quanto ao nível da ação propriamente dita e quanto à relação entre esses dois níveis. Com base nessa distinção, o artigo argumenta que o construtivismo trata, do ponto de vista epistemológico, sobre a construção social do conhecimento e, do ponto de vista ontológico, sobre a construção da realidade social. Além disso, o construtivismo demanda a combinação de uma teoria social do conhecimento com uma teoria intersubjetivista da ação, em detrimento de uma teoria individualista da ação. Finalmente, a análise do poder é central para compreender a ligação reflexiva que se estabelece entre os níveis da observação e da ação. Este argumento está inserido numa contextualização na qual o construtivismo é inspirado pela “modernidade reflexiva” e, mais diretamente, pelo fim da Guerra Fria.
