  • Incidence of and risk factors for neonatal morbidity after active perinatal care : extremely preterm infants study in Sweden (EXPRESS)

    • Aims: The aim of this study was to determine the incidence of neonatal morbidity in extremely preterm infants and to identify associated risk factors. Methods: Population based study of infants born before 27 gestational weeks and admitted for neonatal intensive care in Sweden during 2004-2007. Results: Of 638 admitted infants, 141 died. Among these, life support was withdrawn in 55 infants because of anticipation of poor long-term outcome. Of 497 surviving infants, 10% developed severe intraventricular haemorrhage (IVH), 5.7% cystic periventricular leucomalacia (cPVL), 41% septicaemia and 5.8% necrotizing enterocolitis (NEC); 61% had patent ductus arteriosus (PDA) and 34% developed retinopathy of prematurity (ROP) stage >= 3. Eighty-five per cent needed mechanical ventilation and 25% developed severe bronchopulmonary dysplasia (BPD). Forty-seven per cent survived to one year of age without any severe IVH, cPVL, severe ROP, severe BPD or NEC. Tocolysis increased and prolonged mechanical ventilation decreased the chances of survival without these morbidities. Maternal smoking and higher gestational duration were associated with lower risk of severe ROP, whereas PDA and poor growth increased this risk. Conclusion: Half of the infants surviving extremely preterm birth suffered from severe neonatal morbidities. Studies on how to reduce these morbidities and on the long-term health of survivors are warranted.

  • Pharmacogenetic Studies of Paclitaxel in the Treatment of Ovarian Cancer

    • The purpose of this study was to evaluate the role of sequence variants in the CYP2C8, ABCB1 and CYP3A4 genes and the CYP3A4 phenotype for the pharmacokinetics and toxicity of paclitaxel in ovarian cancer patients. Thirty-eight patients were treated with paclitaxel and carboplatin. The genotypes of CYP2C8*1B, *1C, *2, *3, *4, *5, *6, *7, *8 and P404A, ABCB1 G2677T/A and C3435T, as well as CYP3A4*1B, were determined by pyrosequencing. Phenotyping of CYP3A4 was performed in vivo with quinine as a probe. The patients were monitored for toxicity and 23 patients underwent a more extensive neurotoxicity evaluation. Patients heterozygous for G/A in position 2677 in ABCB1 had a significantly higher clearance of paclitaxel than most other ABCB1 variants. A lower clearance of paclitaxel was found for patients heterozygous for CYP2C8*3 when stratified according to the ABCB1 G2677T/A genotype. In addition, the CYP3A4 enzyme activity in vivo affected which metabolic pathway was dominant in each patient, but not the total clearance of paclitaxel. The exposure to paclitaxel correlated to the degree of neurotoxicity. Our findings suggest that interindividual variability in paclitaxel pharmacokinetics might be predicted by ABCB1 and CYP2C8 genotypes and provide useful information for individualized chemotherapy.

  • Prevalence of burnout in competitive adolescent athletes

    • This study examined the factorial validity of the Eades Burnout Inventory (EABI) and the prevalence of burnout in adolescent elite athletes and whether burnout is more common in individual sports than in team sports. The EABI was distributed to 980 athletes (402 females and 578 males) in 29 different sports. Confirmatory-factor analyses revealed an acceptable factorial validity for a theoretically supported four-factor model of the EABI. Between 1% and 9% of the athletes displayed elevated burnout scores on these four subscales. The hypothesis of higher prevalence of burnout in individual sports was, however, not supported. Furthermore, no correlation between training load and burnout scores was found. These findings suggest that factors other than training load must be considered when athletes at risk for burnout are investigated.

    • The aim of this study was to gain a better understanding of the process of burning out in endurance athletes. The experiences of three elite cross-country skiers who left Their sport due to burnout were explored. Semi-structured interviews were conducted and inductively analyzed. The Athlete Burnout Questionnaire and training logs were used to validate the interviews and to enrich the analysis. The burnout process was found to evolve with different severity and time perspectives in the three cases. Athletic identity and achievement strivings to validate self-esteem were found to be important driving forces in the burnout process. Also, chronic lack of mental and physical recovery as well as early skiing success leading to high expectations comprised common themes in the burnout process.

    • Frågor som rör chef- och ledarskap genererar alltid ett stort intresse såväl i arbetslivet som inom forskning. Det tema som fokuseras i den här rapporten är chef-/ledarskap och kön, särskilt könsfördelningen på ledande befattningar. I dagsläget är merparten av alla chefer i svenskt arbetsliv män. Män är överrepresenterade på chefsbefattningar i relation till samtliga anställda män i såväl privat som offentlig sektor. På de högsta chefsbefattningarna blir mansdominansen än tydligare (SCB, 2006). Mansdominansen på ledande befattningar kan sägas utgöra en utgångspunkt för de forskningsfrågor som formulerats inom forskningsområdet chef-/ledarskap och kön. Forskningsfrågorna handlar t.ex. om varför så få kvinnor är chefer samt på senare år om hur ledarskap är könsmärkt, dvs. hur ledarskap förknippas med kön. En återkommande fråga i mansdominerade organisationer är: Hur kan vi få fler kvinnor på ledande befattningar? Det är just denna problematik som fokuseras i föreliggande rapport som sammanfattar erfarenheter från ett forsknings- och utvecklingsprojekt som pågick från senhösten 2007 till senhösten 2008.Syftet med föreliggande rapport är att kartlägga och analysera villkor för kvinnor och män att bli chefer och att utöva chef- och ledarskap, samt varför få kvinnor har chefspositioner. Utgångspunkten för denna analys är en empirisk studie inom ett globalt verkstadsindustriföretag baserad på intervjuer med chefer, både kvinnor och män, på olika chefsnivåer och från olika divisioner. Följande mer specifika frågeställningar har formulerats:Vad karaktäriserar chefernas uppfattningar om ledarskap, chefsrekrytering, lärande och karriär, jämställdhet samt hälsa i företaget?Vilka förutsättningar finns för kvinnor och män att rekryteras till chefspositioner samt utöva chef- och ledarskap i företaget?Vilka orsaker finns till varför få kvinnor har chefspositioner i företaget?För att kunna diskutera skillnader mellan kvinnors och mäns uppfattningar och villkor närmar vi oss forskningsfrågorna utifrån ett genusperspektiv. Med genusperspektiv avser vi i denna rapport synsättet att vi formas till kvinnor och män genom det vi gör, och de aktiviteter vi deltar i, i samspel med andra människor i det dagliga arbetet. Våra könsidentiteter är beroende av rådande, socialt konstruerade, föreställningar i samhället och vi lär oss och lär oss att förhålla oss till dessa föreställningar i det dagliga arbetet. Vi utvecklar dessa tankar ytterligare i kapitel 2.

  • Intra-individual variability in state anxiety and self-confidence in elite golfers

    • Precompetition anxiety levels are assumed to moderate athletic performance. Unfortunately, cross-sectional and nomothetic research designs have often shown non-significant findings; intra-individual variability may be a contributing factor. The extent of variability in precompetition anxiety and self-confidence responses as related to golf performance and trait measures were therefore examined using an idiographic approach. Individual patterns of variability were found for Cognitive and Somatic Anxiety and Self-Confidence scores yielded prior to the games played. Variability in Somatic Anxiety was significantly related to variability in golf performance. Players low in anxiety variability scored significantly higher on Private Self-Consciousness. The findings suggest the influence of anxiety and self-confidence on performance may be better understood when trait characteristics of the individual are also considered.

    • BACKGROUND: Epidemiological evidence indicates yet unknown epigenetic mechanisms underlying a propensity for overweight and type 2 diabetes. We analyzed the extent of methylation at lysine 4 and lysine 9 of histone H3 in primary human adipocytes from 43 subjects using modification-specific antibodies.RESULTS: The level of lysine 9 dimethylation was stable, while adipocytes from type 2 diabetic and non-diabetic overweight subjects exhibited about 40% lower levels of lysine 4 dimethylation compared with cells from normal-weight subjects. In contrast, trimethylation at lysine 4 was 40% higher in adipocytes from overweight diabetic subjects compared with normal-weight and overweight non-diabetic subjects. There was no association between level of modification and age of subjects.CONCLUSIONS: The findings define genome-wide molecular modifications of histones in adipocytes that are directly associated with overweight and diabetes, and thus suggest a molecular basis for existing epidemiological evidence of epigenetic inheritance.

    • The Competitive State Anxiety Inventory-2 (CSAI-2) is one of the most frequently used instruments when assessing competitive state anxiety in sport psychology research. However, doubts have been expressed about the factorial validity of both the English and the Greek versions of the scale. Hence, a revised version of the inventory (CSAI-2R) has recently been suggested to be more psychometrically sound (Cox et al., 2003). In the present study, the aim was to evaluate the psychometric properties of the Swedish version of the CSAI-2 using confirmatory factor analyses. A total of 969 athletes (571 men and 398 women) competing in 26 different sports completed the Swedish version of the CSAI-2. Three different factor structures were evaluated: the original three-factor model (with cognitive anxiety, somatic anxiety and self-confidence), a two-factor model in which self-confidence was excluded, and a three-factor model containing 17 items (CSAI-2R). The results revealed that only the 17-item model displayed an acceptable fit to the data. Although some doubts remain about the amount of variance that can be attributed to error variance in the subscales, the results suggest that it is better to use the CSAI-2R rather than the original CSAI-2.

    • The topic of this paper is the CDIO Standards, specifically the formulation of CDIO Standards version 3.0. The paper first reviews the potential change drivers that motivate a revision of the Standards. Such change drivers are identified both externally (i.e., from outside of the CDIO community) and internally. It is found that external change drivers have affected the perceptions of what problems engineers should address, what knowledge future engineers should possess and what are the most effective teaching practices in engineering education. Internally, the paper identifies criticism of the Standards, as well as ideas for development, that have been codified as proposed additional CDIO Standards. With references to these change drivers, five areas are identified for the revision: sustainability, digitalization of teaching and learning; service; and faculty competence. A revised version of the Standards is presented. In addition, it is proposed that a new category of Standards is established, “optional standards”. Optional Standards are a complement to the twelve “basic” Standards, and serve to guide educational development and profiling beyond the current Standards. A selected set of proposed optional Standards are recommended for further evaluation and possibly acceptance by the CDIO community
