    • CONTEXT: The European Association of Urology (EAU) guidelines panel on Muscle-invasive and Metastatic bladder cancer (BCa) updates its guidelines yearly. This updated summary provides a synthesis of the 2013 guidelines document, with emphasis on the latest developments.OBJECTIVE: To provide graded recommendations on the diagnosis and treatment of patients with muscle-invasive BCa (MIBC), linked to a level of evidence.EVIDENCE ACQUISITION: For each section of the guidelines, comprehensive literature searches covering the past 10 yr in several databases were conducted, scanned, reviewed, and discussed both within the panel and with external experts. The final results are reflected in the recommendations provided.EVIDENCE SYNTHESIS: Smoking and work-related carcinogens remain the most important risk factors for BCa. Computed tomography (CT) and magnetic resonance imaging can be used for staging, although CT is preferred for pulmonary evaluation. Open radical cystectomy with an extended lymph node dissection (LND) remains the treatment of choice for treatment failures in non-MIBC and T2-T4aN0M0 BCa. For well-informed, well-selected, and compliant patients, however, multimodality treatment could be offered as an alternative, especially if cystectomy is not an option. Comorbidity, not age, should be used when deciding on radical cystectomy. Patients should be encouraged to actively participate in the decision-making process, and a continent urinary diversion should be offered to all patients unless there are specific contraindications. For fit patients, cisplatinum-based neoadjuvant chemotherapy should always be discussed, since it improves overall survival. For patients with metastatic disease, cisplatin-containing combination chemotherapy is recommended. For unfit patients, carboplatin combination chemotherapy or single agents can be used.CONCLUSIONS: This 2013 EAU Muscle-invasive and Metastatic BCa guidelines updated summary aims to increase the quality of care and outcome for patients with muscle-invasive or metastatic BCa.PATIENT SUMMARY: In this paper we update the EAU guidelines on Muscle-invasive and Metastatic bladder cancer. We recommend that chemotherapy be administered before radical treatment and that bladder removal be the standard of care for disease confined to the bladder.

    • This paper updates my talk on Cache Blocking for Dense Linear Algorithms since 1985 given at PPAM 11; see [11]. We again apply Dimension Theory to matrices in the Fortran and C programming languages. New Data Structures (NDS) for matrices are given. We use the GCD algorithm to transpose a n by m matrix A in CMO order, standard layout, in-place. Algebra and Geometry are used to make this idea concrete and practical; it is the reason for title of our paper: make a picture of any matrix by the GCD algorithm to convert it into direct sum of square submatrices. The picture is Geometry and the GCD algorithm is Algebra. Also, the in-place transposition of the GKK and TT algorithms will be compared. Finally, the importance of using negative integers will be used to give new results about subtraction and finding primitive roots which also make a priori in-place transpose more efficient.

    • Psalmer är perenna skapelser. Alla psalmer som sjungs idag har en gång först skrivits och komponerats. Därefter har de hållits vid liv, redigerats, översatts, omarbetats, restaurerats, utvalts och återaktualiserats. Titeln på denna årsbok: ”Arbete med psalm” syftar främst på denna senare typ av trädgårdsmästarliknande arbete. Utan psalmboksredaktörer, översättare och bearbetare av psalmer hade vi stått utan de psalmer vi har idag. Vi hade utanför rent akademiska studier möjligen stått helt utan arvet av äldre psalmer. Sådant arbete har nämligen både syftat och lett till bredare relevans och aktualitet i tider av förändring.Årsbokens sju artiklar belyser på olika sätt hur arbete med psalm lett fram till vår nuvarande situation. Här finns också bidrag som kastar ljus över hur psalmerna kan förstås idag och hur deras morgondag kan komma att te sig. Kommer det att finnas en psalmbok även i framtiden? I speciellt fokus står för flera av texterna den i flera samfund formellt gällande och dagligen sjungna Den svenska psalmboken av 1986. Dess ekumeniskt tillkomna första del var och är gemensam för en rad svenska kyrkosamfund och har också på olika sätt blivit en sångbok även långt utanför de kyrkliga sfärerna. Sist i årsboken följer ett tjugotal recensioner av nyutkommen litteratur inom liturgik, kyrkokonst, kyrkomusik och homiletik.

    • As SOA-based systems are becoming more common, there is a need to consider how traditional IT roles and responsibilities need to change. This paper proposes a framework for roles that are required for evolving and maintaining SOA-based systems. It builds on work on traditional IT roles, as well as insights emerging from current research on SOA. The paper also presents a questionnaire for collecting data from organizations on roles that they use for SOA-based systems maintenance and evolution. Results from a pilot use of the questionnaire with Scandinavian Airline Systems (SAS) are presented.

    • The article provides a close-up picture of gender and personality in relation to the gender composition of occupation and the gender wage gap. Using a survey of newly graduated highly educated men and women in five occupations in Sweden (engineers, lawyers, police officers, social workers and psychologists, n ≈ 2400), we examine (a) if personality traits—measured as Big Five traits, risk-taking and self-esteem—differ between men and women (b) if differences in personality traits are systematically related to the gender composition of the occupation, (c) if individuals who have chosen an occupation dominated by the other gender are gender-atypical in their personalities and, (d) how personality traits are related to wages and the gender wage gap. The results show significant gender differences in agreeableness, emotional stability and perceived risk-taking. The male-dominated occupations score higher on risk-taking than those dominated by females, but the pattern for agreeableness is less clear and the scores on emotional stability are no higher in these occupations. Further we find that individuals who have chosen a gender-atypical occupation tend to display gender atypical personality traits. In line with previous research, we find that risk-taking and self-esteem are positively related to wages but these associations do not account for gender differences in wages. The valuation of personality traits does not vary systematically with the gender composition of the occupations but being agreeable has a more negative wage effect for women than for men.
