    • A pandemic is a crisis and as such, in many ways, an entirely new situation. It is a situation in which there are scientific as well as societal uncertainties and where the routines of everyday life are overturned. The Covid-19 pandemic has made visible already established social structures and inequalities, but it has also exacerbated existing power relations and vulnerabilities. In this article, we introduce the contributions to the special issue of Sociologisk Forskning on how society and people’s lives have been affected by the corona crisis and the Covid-19 pandemic. We also give an overview of some of the themes that are picked up by sociologists and social scientists internationally as well as nationally in Sweden, when trying to make sense of the meaning and effects of the Covid-19 pandemic.

    • Serious adverse drug reactions, drug intolerance, and lack of effect are major problems in healthcare. Pharmacogenomics is the part of precision medicine that aims to develop predictive risk markers in this respect and establish such testing in clinical practice. The nation-wide project Genomic Medicine Sweden (GMS) is undertaking large-scale sequencing to predict risk of drug toxicity and lack of efficacy in malignant diseases. The aim is to facilitate an improved, individualized treatment with increased patient safety. In addition to accurate genotyping, other technical or infrastructure-related aspects need to be considered for a successful implementation in healthcare, for example electronic accessibility and visibility of pharmacogenomic data of long-standing relevance for an individual's ongoing and future drug treatment.

    • Preterm delivery in Sweden constitutes 5.7 % of all deliveries, which is among the lowest rates in the world. There has not been any increase in the proportion of iatrogenic preterm deliveries during the last decades.The main hypothesis concerning the causality of preterm delivery is still that of the ascending infection from the vagina to the uterus and inflammation resulting in contractions, rupture of membranes and delivery. The mechanisms behind parturition at term are still elusive and this is also true for preterm delivery. The genetic contribution to preterm delivery is about 25-30 %. The first genes that are associated with preterm delivery and gestational duration have recently been published. Huge progress has been made in care of preterm born infants. Sweden has among the lowest rates of mortality and morbidity in the world, especially in the lowest gestational weeks. New modes of care, family-centered care and hospital-assisted home care, have empowered the parents and reduced the cost for care.

    • Remarkable progress has been made in the understanding of schizophrenia in the past few years. A driving force has been genome wide association studies that have now led to the identification of over 100 vulnerability loci, implicating functions in the immune system, calcium signaling as well as dopamine and glutamate transmission. In coupling the genetic information to functional data sets from imaging and cognitive studies there is a promise of developing radically improved understanding of schizophrenia, and in some cases new therapies based on immune modulation. As we develop more knowledge and better therapies there will likely be a reduction in stigmatization that is a very real problem for those affected and their families.

    • For some people with medically refractory epilepsy adjunct treatment with neurostimulation and ketogenic diet can be an option. Vagal nerve stimulation, VNS, consists of chronic intermittent electrical stimulation of the vagus nerve. VNS is effective and well tolerated in adolescents and adults. There is insufficient evidence on the efficacy of deep brain stimulation, DBS, and brain-responsive neurostimulation, RNS. Ketogenic diet is an established treatment in epilepsy with an increased number of indications, especially in neurometabolic diseases, where dietary therapy can completely eliminate epilepsy and improve cognitive and motor development. Ketogenic diet is a calculated diet consisting of a high amount of fat, the daily need of protein and a minimum of carbohydrates. The fat is converted to ketone bodies and transported across the blood-brain barrier to be used as an alternative energy source in the brain. Strictly calculated menus, regular monitoring and support from a team of dietician, nurse and physician are required.
