  • Criminality among young people with substance use problems in Sweden : A one-year follow-up study

    • Swedish drug policy has a strong focus on minor drug violations. This longitudinal study analyses the association between criminality and substance use problems among young people who have received outpatient treatment and risk factors that predict continued problems with criminality. Research on this target group is under development in Sweden, but there are few follow-up studies. The study indicates that about one-quarters of the young people who begin outpatient treatment have been convicted of crimes at 1-year follow-up. Most of them who had been convicted with offences also have ongoing problems with substance use and three-quarters of the young people had been charged with a drug-related offence. More than five risk factors give an elevated risk of continued criminality. Interventions meeting the needs of young people, moving away from the current emphasis on penal law principles, may also contribute to reduced criminality in this target group.

  • Parental Awareness of Substance Use Among Adolescents in a Junior High School Sample

    • There is a lack of studies assessing parental awareness of adolescent alcohol, cigarette, and drug use in the general adolescent population. A total of 1,426 adolescents aged 14 to 16, and their parents, answered questions about adolescent substance use annually during junior high school. Sensitivity for parental report of adolescent substance use was low: 5.6% to 26% for drunkenness, 14.3% to 20.6% for cigarettes, and 4.9% to 12% for illicit drugs. Higher age and higher frequency of use were positively associated with parental awareness of drunkenness and cigarette use. Female sex was associated with higher parental awareness of drunkenness. Higher school performance was negatively associated with parental awareness of drunkenness in Grade 9 and with cigarette use in Grades 8 to 9. Parental awareness of adolescent drunkenness, and cigarette and illicit drug use in the general population is low. Factors of importance for parental awareness are identified.

  • The evolution of Swedish drug policy

    • In order to understand any drug policy one must take into consideration its various components: assumptions about human nature, cultural and political traditions, the activities of different interest groups, theoretical framework, and the like. This paper discusses these aspects of Swedish drug policy as a means of shedding light on why Sweden, widely renowned as a humanitarian and rational country, has been one of the staunchest proponents of what some people consider to be inhumane and irrational drug policies. The author argues that Sweden's drug policy is basically logical, given the nation's cultural understanding and assumptions about alcohol (assumptions that have been transferred to other drugs as well). However, due to changes both within and outside of the country, alternative paradigms are gradually gaining ground, and there are indications that change in Swedish drug policy may be forthcoming.

  • "Shop Until You Drop" : Valuing Fentanyl Analogs on a Swedish Internet Forum

    • Fentanyl analogs are synthetic opioids used for pain treatment and palliative care, which are also sought after by drug users for their psychoactive properties. Clandestinely produced fentanyl has caused an overdose crises of unprecedented scale in the United States. In Sweden, the retail purchase, possession, and use of some analogs are legal, providing opiate users with a legal alternative, until the process of scheduling is finished. The continuous process of scheduling and introduction of slightly modified variants implies that there is much uncertainty regarding the potency and quality of newly introduced analogs. We examine user perceptions of fentanyl analogs in a thematic analysis of the public internet forum, Flashback, from 2012 to 2019. In 24 threads on fentanyl analogs, posters shared and discussed information on the emergence of new analogs, their desirability and prices, adverse health effects, and eventual scheduling.

  • Predictors of Psychiatric Hospitalization in Ex-Prisoners With Substance Use Problems: A Data-Linkage Study

    • This study analyzed predictors of psychiatric hospitalization in ex-prisoners with substance use problems (N = 4,081) assessed with the Addiction Severity Index and followed post-release for hospitalizations with psychiatric diagnoses (including suicide attempts). Thirty-four percent were hospitalized, and in Cox regression, several substance-related variables predicted hospitalization, including use of heroin, sedatives, and polysubstance. A secondary analysis, with a psychiatric non-substance focus, excluded hospitalizations involving only substance-related disorders or only a personality disorder in addition to a substance-related disorder. With this definition, 10% were hospitalized, and significant baseline predictors were previous psychiatric hospitalization (hazard ratio [HR] = 1.83), previous suicide attempt (HR = 1.91), depression (HR = 1.33), anxiety (HR = 1.37), sedative use (HR = 1.46), and, negatively, amphetamine use (HR = 0.71). Substance-related variables may predict all-cause psychiatric hospitalizations in prisoners with substance use problems, whereas non-substance-related psychiatric hospitalization may be predicted by baseline psychiatric problems, which calls for attention to psychiatric problems in this setting.

  • Targeting Maladaptive Anger With Brief Therapist-Supported Internet-Delivered Emotion Regulation Treatments : A Randomized Controlled Trial

    • Objective: To evaluate the relative impact of three brief therapist-supported internet-delivered emotion regulation treatments for maladaptive anger (mindful emotion awareness [MEA], cognitive reappraisal [CR], and mindful emotion awareness + cognitive reappraisal [MEA + CR]) and to test whether baseline levels of anger pathology moderate treatment outcome.Method: Treatments were evaluated in a randomized controlled trial. In total, 234 participants (59% female; mean age = 41.1, SD = 11.6) with maladaptive anger were randomized to MEA (n = 78), CR (n = 77), or MEA + CR (n = 79). Self-reported primary and secondary outcomes were followed up at primary endpoint, 3 months after treatment termination (88% retention). Primary outcomes were also assessed weekly during a prolonged baseline phase (4 weeks) and an active treatment phase (4 weeks).Results: At the primary endpoint, the MEA + CR was superior in terms of anger expression (d = 0.27 95% confidence interval, CI [0.03, 0.51]), aggression (d = 0.43 [0.18, 0.68]), and anger rumination (d = 0.41 [0.18, 0.63]). MEA + CR was particularly effective in reducing anger expression (d = 0.66 [0.21, 1.11]), aggression (d = 0.90 [0.42, 1.39]), and anger rumination (d = 0.80 [0.40, 1.20]) for individuals who reported high values (+1SD) of the outcomes at baseline.Conclusions: Brief therapist-supported internet-delivered MEA and CR treatments are effective interventions for maladaptive anger. Combining MEA and CR is especially effective in reducing anger expression and aggression, particularly, in individuals who report higher levels of initial anger pathology. The present study highlights the importance of emotion regulation as an important treatment target for reducing maladaptive anger.
