  • Development of 7-item perceived meaning of activity in housing (PMA-H-7) to assess opportunities for meaningful activities in the supported housing context for people with psychiatric disabilities

    • People with psychiatric disabilities need to be able to express their views about their support, for example, the provision of meaningful activity in supported accommodation. Brief scales may be necessary to avoid strain and discomfort for this group. The aim was to develop a brief version of Perceived Meaning in Activity—Housing (PMA-H) and investigate its factor structure, internal consistency, evidence for convergent and discriminant validity, and any floor or ceiling effects. Two data sets from people with psychiatric disabilities living in congregate supported housing were reanalyzed. Seven-item PMA-H-7 was developed in Sample 1 and then tested for psychometric properties in Samples 1 (n = 155) and 2 (N = 35). Questionnaires addressing phenomena supposed to be similar to PMA (satisfaction with accommodation), or dissimilar (health, psychiatric symptoms, level of functioning), were administered. A single factor explained 44% of the variation in PMA-H-7. Internal consistency was α = .74/.75 in the two samples. Correlations of r s = .47/.52 with housing satisfaction indicated convergent validity. Low and insignificant associations were found for the remaining variables, supporting discriminant validity. No floor or ceiling effects were identified. PMA-H-7 showed adequate psychometric samples for use with people with psychiatric disabilities in assessing opportunities for meaningful activity in the supported housing context.

  • Factor Structure and Construct Validity of the Worker Role Self-Assessment (WRS) When Used for People With Psychiatric Disabilities in Sweden

    • A self-report instrument assessing work-related factors among people with psychiatric disabilities would be useful when trying to match possible employment or prevocational opportunities with people’s desires and capacities. The aim of this study was to explore the factor structure, internal consistency, and construct and criterion validity of the Worker Role Self-Assessment (WRS) in this group as well as possible floor and ceiling effects. The participants were 283 clients from day centers for people with psychiatric disabilities and from outpatient units for people with psychosis. They completed the WRS and instruments selected to assess construct validity in terms of convergent (motivation for work and current activity level as reference variables) and discriminant validity (quality of life and self-rated health as reference variables). Two factors were identified, one tapping beliefs in a future worker role and one reflecting current capacities and routines. The internal consistency for the scale as a whole was good at 0.84. The factor reflecting a future worker role correlated as expected with the reference variables used to assess convergent and discriminant validity, whereas current capacities and routines showed a moderate association with quality of life and self-rated health, assumed to indicate discriminant validity. Criterion validity was shown in that those who had recent work experiences scored higher than the others on WRS. No floor or ceiling effects were identified. The findings indicate acceptable psychometric properties of the WRS. Further development is still warranted, however; the factor solution needs to be replicated and the construct validity should be further established.

    • There is a scarcity of instruments for assessing opportunities for residents with psychiatric disabilities to engage in meaningful home-based activities. We thus developed the Perceived Meaning of Activity in Housing (PMA-H), containing four subscales (activity opportunities, social interaction, developing as a person, and organization and planning). The aim of the study was to investigate the content validity, utility, internal consistency and concurrent and criterion validity of the PMA-H, as well as possible floor and ceiling effects. One hundred and fifty-five residents in supported housing (SH) and 111 in ordinary housing with support (OHS) completed the PMA-H. The SH group also competed the Community-Oriented Programs Environment Scale (COPES). A majority of the participants found the content of the PMA-H relevant, easy to complete, and the time for completion as reasonable. Internal consistency varied between 0.85 and 0.92 for the subscales. Logical associations with COPES indicated construct validity. The subscales social interaction and developing as a person could discriminate between the SH and OHS groups, whereas activity opportunities could not. No floor or ceiling effects were found. This study indicated adequate initial psychometric properties of the PMA-H. It can thus be used in housing settings to assess the residents' perceived opportunities for meaningful activity in the housing context.

  • Psychometric Evaluation of the Persian eHealth Literacy Scale (eHEALS) Among Elder Iranians With Heart Failure

    • The purpose of the present study was to examine the psychometric properties of the eHealth Literacy Scale (eHEALS) using classical test theory and modern test theory among elderly Iranian individuals with heart failure (HF). Individuals with objectively verified HF ( n = 388, 234 males, mean age = 68.9 ± 3.4) completed the (i) eHEALS, (ii) Hospital Anxiety and Depression Scale, (iii) Short Form 12, (iv) 9-item European Heart Failure Self-Care Behavior Scale, and (v) 5-item Medication Adherence Report Scale. Two types of analyses were carried out to evaluate the factorial structure of the eHEALS: (i) confirmatory factor analysis (CFA) in classical test theory and (ii) Rasch analysis in modern test theory. A regression model was constructed to examine the associations between eHEALS and other instruments. CFA supported the one-factor structure of the eHEALS with significant factor loadings for all items. Rasch analysis also supported the unidimensionality of the eHEALS with item fit statistics ranging between 0.5 and 1.5. The eHEALS was significantly associated with all the external criteria. The eHEALS is suitable for health-care providers to assess eHealth literacy for individuals with HF.

  • Validating the Persian Intuitive Eating Scale-2 Among Breast Cancer Survivors Who Are Overweight/Obese

    • Women with breast cancer are at risk of being overweight/obese which may consequently increase mortality. Intuitive eating is an adaptive eating behavior which might be beneficial for weight outcomes. The present study validated the Persian Intuitive Eating Scale-2 (IES-2) among overweight/obese Iranian females with breast cancer. Women who were overweight/obese with breast cancer (n = 762; mean ± SD age = 55.1 ± 5.7 years) completed the following questionnaires: IES-2, General Self-Efficacy Scale (GSE-6), Hospital Anxiety and Depression Scale (HADS), Short Form-12 (SF-12), Weight Bias Internalization Scale (WBIS), Body Appreciation Scale-2 (BAS-2), and Eating Attitudes Test (EAT-26). Confirmatory factor analysis (CFA) and Rasch analysis were applied to examine the psychometric properties of the IES-2. Associations between IES-2 score and other scale scores were assessed. CFA and Rasch analysis suggested that the Persian IES-2 had robust psychometric properties and all IES-2 items were meaningful in their embedded domains. The four-factor structure of the Persian IES-2 was confirmed. Concurrent validity was supported by the positive correlations between the IES-2 score and scores on the GSE-6, SF-12 mental component, and BAS-2. Negative correlations were found between the IES-2 score and the HADS (anxiety and depression subscales), WBIS, and EAT-26. The present study demonstrated that the Persian IES-2 is a well-designed instrument and is applicable for women who are overweight/obese with breast cancer.

    • In a multicultural society, ethnocultural empathy has becomean important element in most health settings and developmentof this capacity has become a central component for health careprofessionals in their interactions with patients and clients.In this study, differences in basic empathy and ethnoculturalempathy were explored in a sample of 365 undergraduate studentsat the beginning and end of four master’s programs inhealth care (medicine, psychology, nursing, and social work).Results showed that it was mainly psychology students in thefirst semester who had significantly higher general empathicskills and ethnocultural empathic skills compared to studentsin the other study programs. Few signs of differences betweenstudents in their first and in later semesters were obtained.The observed differences may be explained by (a) levels of admissiongrades and applications requirements or (b) different culturesand expectations from the surrounding milieus in the investigatedstudy programs.

  • What Is Lean Management in Health Care? : Development of an Operational Definition for a Cochrane Systematic Review

    • Industrial improvement approaches such as Lean management are increasingly being adopted in health care. Synthesis is necessary to ensure these approaches are evidence based and requires operationalization of concepts to ensure all relevant studies are included. This article outlines the process utilized to develop an operational definition of Lean in health care. The literature search, screening, data extraction, and data synthesis processes followed the recommendations outlined by the Cochrane Collaboration. Development of the operational definition utilized the methods prescribed by Kinsman et al. and Wieland et al. This involved extracting characteristics of Lean, synthesizing similar components to establish an operational definition, applying this definition, and updating the definition to address shortcomings. We identified two defining characteristics of Lean health-care management: (1) Lean philosophy, consisting of Lean principles and continuous improvement, and (2) Lean activities, which include Lean assessment activities and Lean improvement activities. The resulting operational definition requires that an organization or subunit of an organization had integrated Lean philosophy into the organization?s mandate, guidelines, or policies and utilized at least one Lean assessment activity or Lean improvement activity. This operational definition of Lean management in health care will act as an objective screening criterion for our systematic review. To our knowledge, this is the first evidence-based operational definition of Lean management in health care.

    • The aim of this study was to conduct a validation and assess the test–retest reliability of the health questionnaire based on Nordenfelt’s Welfare Theory of Health (WTH). The study used a questionnaire on health together with the Short Form 12-Item Health Survey (SF-12) questionnaire, and 490 pupils at colleges for adult education participated. The results of the study are in accordance with Nordenfelt’s WTH. Three hypotheses were stated, and the first was confirmed: People who were satisfied with life rated higher levels than those who were dissatisfied with life concerning both mental and physical health, measured with the SF-12. The second hypothesis was partially confirmed: People with high education were more often satisfied with life than those with low education, but they were not healthier. The third hypothesis, that women are unhealthy more often than men, was not confirmed. The questionnaire on health showed acceptable stability.

  • Going Global : A Model for Evaluating Empirically Supported Family-Based Interventions in New Contexts

    • The spread of evidence-based practice throughout the world has resulted in the wide adoption of empirically supported interventions (ESIs) and a growing number of controlled trials of imported and culturally adapted ESIs. This article is informed by outcome research on family-based interventions including programs listed in the American Blueprints Model and Promising Programs. Evidence from these controlled trials is mixed and, because it is comprised of both successful and unsuccessful replications of ESIs, it provides clues for the translation of promising programs in the future. At least four explanations appear plausible for the mixed results in replication trials. One has to do with methodological differences across trials. A second deals with ambiguities in the cultural adaptation process. A third explanation is that ESIs in failed replications have not been adequately implemented. A fourth source of variation derives from unanticipated contextual influences that might affect the effects of ESIs when transported to other cultures and countries. This article describes a model that allows for the differential examination of adaptations of interventions in new cultural contexts.
