    • Matrix vesicles (MVs) are extracellular membrane-bound vesicles of about similar to 50-200 nm in diameter that play a role in the bio-mineralization process of hard tissue formation. The present review is based on the empirical phenomenon of primary mineralization process via matrix vesicle-mediated mechanism with special reference to crystal ghosts as well as the mechanism on the organic-inorganic relationship between matrix vesicles and crystal ghosts, and the transformation that these structures undergo during bio-mineralization.

  • Over-expression of Adamts1 in mice alters bone mineral density

    • ADAMTS1, a secreted multifunctional metalloproteinase with disintegrin and thrombospondin motifs, is an early response gene of parathyroid hormone (PTH) in osteoblasts. Mice engineered to lack Adamts1 are smaller compared to wild-type (WT) mice and ADAMTS1 metalloproteinase activity has been shown to increase osteoblastic growth in collagen gels. However, there are no reports investigating the consequence of Adamts1 over-expression on bone tissue in vivo. Here, we analyze bones of female and male transgenic (TG) mice over-expressing mouse Adamts1 using peripheral quantitative computed tomography to evaluate its effect on bone shape and mineral density. Western blotting of protein extracts and immunohistochemistry of bone sections reveal increased presence of Adamts1 protein in TG bones compared to WT bones. Phenotypic analyses of femur show that female TG mice have reduced metaphyseal total density, trabecular bone mineral density and trabecular mineral content. In contrast, male TG mice which were without changes in the metaphysis showed increased total density and cortical density at the mid-diaphysis cortical site. Female TG mice showed no significant changes at the cortical site compared to WT mice. Furthermore, diaphyseal endosteal compartment was only affected in male TG mice. Along these lines, Adamts1 increased blood levels of PTH only in females whereas it reduced osteocalcin levels only in males. These results reveal that Adamts1 has an impact on bone mineral density and thus further confirm Adamts1 as a potent regulator of bone remodeling.

    • New assessment guidelines for osteoporosis in Japan include the use of the WHO risk assessment tool (FRAX) that computes the 10-year probability of fracture. The aim of this study was to determine the distribution of fracture probabilities and to assess the impact of probability-based intervention thresholds in women from Japan aged 50 years and older. Age-specific simulation cohorts were constructed from the prevalences of clinical risk factors and femoral neck bone mineral density to determine the distribution of fracture probabilities as assessed by FRAX. These data were used to estimate the number and proportion of women at or above a 10-year fracture probability of 5, 10, 15, 20, 25, and 30 %. In addition, case scenarios that applied a FRAX probability threshold of 15 % were compared with current guidance. In the absence of additional criteria for treatment, a 15 % fracture probability threshold would identify approximately 32 % of women over the age of 50 years (9.3 million women) as eligible for treatment. Because of expected changes in population demography, the 15 % fracture probability threshold would capture approximately 38 % of women over the age of 50 years (12.7 million women), mainly those aged 80 years or older. The introduction of a FRAX threshold of 15 % would permit treatment in women with clinical risk factors that would otherwise fall below previously established intervention thresholds. The incorporation of FRAX into assessment guidelines is likely to redirect treatments for osteoporosis from younger women at low risk to elderly women at high fracture risk.

  • Prodromal Symptoms in Patients with Bisphosphonate-Associated Atypical Fractures of the Femur

    • Symptoms have been reported to precede bisphosphonate-associated atypical fractures (AFs) of the femoral shaft. We aimed to determine the frequency and clinical characteristics of such prodromal symptoms. We searched the Swedish national database of spontaneously reported adverse drug reactions for all cases of AF associated with bisphosphonates from January 2006 to March 2013. To confirm diagnostic accuracy and to characterize and determine the frequency of any prodromal symptoms we retrieved copies of medical journals and radiographs for patients who consented to participate in the study. The frequency of prodromal symptoms was compared with that of patients where information was based only on narratives from the adverse drug reaction case reports. A total of 45 reports of AF were identified. We were able to obtain medical records and x-rays for 21 cases and diagnostic accuracy was confirmed for all. Medical records revealed prodromal symptoms in 86 % (n = 18), most commonly pain in the ipsilateral thigh (14 out of 18 patients) preceding the fracture for weeks or longer. Awareness of such symptoms may facilitate early diagnosis and possible prevention of the AF.
