    • BackgroundPeople with dementia and their informal carers often do not receive appropriate professional support or it is not received at the right time.ObjectivesDescription and comparison of common pathways to formal community dementia care in eight European countries as a part of the transnational Actifcare project.Materials and methodsThe German team was responsible for creating an individual case scenario as a starting point. The research teams in Ireland, Italy, the Netherlands, Norway, Portugal, Sweden, and the United Kingdom were then asked to describe a common pathway to formal dementia care by writing their own vignette using the provided individual case scenario.ResultsA transnational qualitative content analysis was used to identify the following categories as being the most important: involved professionals, dementia-specific and team-based approaches, proactive roles, and financial aspects. General practitioners (GPs) are described as being the most important profession supporting the access to formal care in all the involved countries. In some countries other professionals take over responsibility for the access procedure. Dementia-specific approaches are rarely part of standard care; team-based approaches have differing significances in each of the countries. Informal carers are mainly proactive in seeking formal care. The Nordic countries demonstrate how financial support enhances access to the professional system.ConclusionEnhanced cooperation between GPs and other professions might optimize access to formal dementia care. Team-based approaches focusing on dementia care should be developed further. Informal carers should be supported and relieved in their role. Financial barriers remain which should be further investigated and reduced.

  • Designing a social and assistive robot for seniors

    • Background: The development of social assistive robots is an approach with the intention of preventing and detecting falls among seniors. There is a need for a relatively low-cost mobile robot with an arm and a gripper which is small enough to navigate through private homes. Material and methods: User requirements of a social assistive robot were collected using workshops, a questionnaire and interviews. Two prototype versions of a robot were designed, developed and tested by senior citizens (n = 49) in laboratory trials for 2 h each and in the private homes of elderly persons (n = 18) for 3 weeks each. Results: The user requirement analysis resulted in a specification of tasks the robot should be able to do to prevent and detect falls. It was a challenge but possible to design and develop a robot where both the senior and the robot arm could reach the necessary interaction points of the robot. The seniors experienced the robot as happy and friendly. They wanted the robot to be narrower so it could pass through narrow passages in the home and they also wanted it to be able to pass over thresholds without using ramps and to drive over carpets. Conclusion: User trials in seniors’ homes are very important to acquire relevant knowledge for developing robots that can handle real life situations in the domestic environment. Very high reliability of a robot is needed to get feedback about how seniors experience the overall behavior of the robot and to find out if the robot could reduce falls and improve the feeling of security for seniors living alone.

Background: In the last two decades labor market participation for older employees has undergone agradual political paradigm shift in many European countries from apolicy of early retirement to one of extending working lives and active aging. Objective: This study investigated if this political paradigm shift is causing new social inequalities in retirement transition due to restricted financial possibilities for early retirement. Material and methods: Data were derived from the European Union Labor Force Survey from the years 2006 and 2012 and selected European countries (Germany, Austria, Sweden and Estonia) were analyzed. Results: Associations between the specific implementation of the policy of active aging, the freedom of choice in retirement timing and retirement transition were found. It seems that voluntary retirement transitions are highest in those countries where the labor market and social policies are most coherent and aimed at supporting older workers’ employability. Conclusion: The reduction of early retirement incentives should be supported by active labor market policies and apolicy of extensive age-independent further training measures in order to minimize social inequalities.

    • Unter dem Leitbild des aktiven Alters wird die Aufrechterhaltung umfassender sozialer Teilhabe über die Lebensphase Alter diskutiert. „Gutes“ Altern im Sinne dieses Aktivitätsparadigmas wandelt sich vom Ergebnis sozialer Ungleichheit zunehmend zu einem ihrer Ausgangspunkte. Auf Basis des Deutschen Alterssurveys (DEAS) 2008 und 2011 wird die Teilhabe über Ehrenamt und Bildungsaktivitäten unter Berücksichtigung sozialstruktureller und sozialräumlicher Aspekte sozialer Ungleichheit in den Blick genommen. Die soziale Teilhabe erweist sich als sozialstrukturell und sozialräumlich ungleich verteilt. Personen aus unteren sozialen Schichten gehen deutlich seltener einer Bildungsaktivität nach oder engagieren sich ehrenamtlich. Zudem ist die soziale Teilhabe bei Personen in wirtschaftlich schwachen Regionen deutlich geringer als bei Personen in wirtschaftlich starken. Nachteilige Effekte kumulieren sich, wenn in einer Region begrenzte individuelle Ressourcen und schlechte ökonomische Rahmenbedingungen vorliegen. Maßnahmen zur Förderung sozialer Teilhabe sollten darauf abzielen, auf lokaler Ebene Gelegenheiten für Engagement und Bildungsaktivität zu schaffen, um die Teilhabechancen von Personen aus unteren sozialen Schichten nachhaltig zu verbessern.

    • Background In Sweden, there is a socioeconomic divide between white and blue collars with respect to risk for premature work life exits. Disability pension has long represented a major reason behind early exits. Objectives The present investigation aimed at studying the effect on socioeconomic groups of new guidelines issued by the Swedish government in 2006, limiting the possibilities for applicants to be granted pension on medical grounds. Material and method The study was based on register data comprising the prevalence in the age group 55-64 years of disability pension and premature age pension in different occupations, comparing the years 2006 and 2011. Results It was found that in 2011 under the new guidelines, newly approved disability pensions had dropped by 70%. Women were affected more than men. The drop in disability pensions affected applicants within the two most prevalent diagnosis chapters, mental disorders (a drop by 58 %) and musculoskeletal disorders (a drop by 87 %). In the same time period, the percentage in the age range 55-64 years choosing premature age pension more than doubled. An increase in the number of premature age pensions was more common in blue collar occupational groups than in white collars. Occupation had a higher impact on working life expectancy than country of birth. Conclusion There are strong indications that many applicants, particularly blue collars, who had been unable to be granted disability pension under the new operational guidelines instead choose premature age pension, a costly alternative for many individuals with already low pension benefits. Our results indicate a tendency of passing on the societal costs of early labour market exits to different economic compensation arrangements, as well as to the individuals themselves.

    • This article presents selected findings of the EUROFAMCARE research project, reporting up-to-date information on the use and accessibility of support services for family carers of older people in six European countries representing different typologies of welfare systems (Germany, Greece, Italy, Poland, Sweden and the UK). Data were collected by means of face-to-face interviews to national samples of about 1000 family carers per country (i.e. 6000 in total), based on a common recruitment and data collection protocol. The reported findings reveal the crossnational usage of different support services - subdivided for comparative reasons in the categories of socio-emotional support, information, respite care, training and assessment services - as well as of available care allowances. The analysis includes the perceived experience of carers in using them, in terms of costs sustained, factors affecting service accessibility - i.e. main obstacles and greatest helps in accessing them - as well as reasons for not using (needed) services or for stopping using (still needed) services. Cross-national differences are relevant, showing a greater availability in Northern European countries, where however higher refusal rates by potential users of available services are recorded, possibly in connection to their lack of flexibility and low customisation. © 2006 Steinkopff-Verlag.

    • This paper focuses on very old age as a challenge for ageing theory, as an empirical problem and as a scope for social policy and it introduces the contributions of the special issue “Very old age in an ageing society”. Especially the need for (re-)integration of the life-phases of young and old age is discussed from the position of social and behavioural ageing research. While reaching very old age is an increasingly normal life-event, and thus there is an increasing need for knowledge, there is currently only limited knowledge about it. It is particularly the diversity and inequality within old and very old age and the pathways into latest life that needs to be targeted. Finally, normative patterns and biographical outlines of this increasingly important phase of life need to be developed.
