  • An investigation of intended and real use of a research web health portal and its implementation

    • Participatory design (PD) projects involve prospective users as co-designers in a process where the design object emerges through several iterations. However, the result of such a process can only partly anticipate how the future real users will use the designed object. For this reason, its actual use needs to be investigated. The present study investigated the relationship between intended use and real use in two web-based health support systems in order to explore the conditions for redesign. The dependency between intended use and real use was found to be weak. Rather, the real use was dependent on 1) the context of use and 2) the needs or interest of the users. We conclude that redesign should be based on continuous use of web metrics collected in natural settings and by involving users on a recurring basis. While a web health portal must have an agenda it is important to adapt thing design to use design, why redesign in essence will become an adaptation to user needs.©Copyright of articles originally published in www.eJHI.net under the Creative Commons Attribution 3.0 License is retained by the authors.

  • Visions and realities : developing ‘smart’ homes for seniors in Sweden

    • The smart home concept with its vision of embedding technology in houses or apartments gained much interest during the end of the 20th and the beginning of 21st century. Although the vision remains, the enthusiasm has diminished somewhat with the one exception: senior housing. Smart homes for seniors have been touted in recent years as the means of enabling ‘aging at home’ and so relieve the pressure on health systems. This research examines the players, issues and current state of development of technology based services in the senior housing sector in Sweden. It is underpinned by the theories of development and innovation and the use of qualitative methods such as observations and in-depth interviews with a broad cross section of players in the sector. This research reveals some of the shortcomings of the approaches used in developing smart housing for the senior population. These include the focus on the technical possibilities rather than solving the every day problems of end-users, the narrow scope of projects, lack of multidisciplinary involvement and insufficient resource.

  • Telenursing In Home Care Services Experinces of Registered Nurses

    • This paper reports on experinces of telenursing by registered nurses working in home care services with a focus on leg wounds and their treatment. In the daily work of care giving, distant treatment was provided through technology for two-party encounters between registered nurses and patients. Ten video recorded interviews were conducted with five nurses. Data were analysed with content analysis. The results show that in virtual encounters the nurses felt great satisfaction from having an image of the patient while simultaneously assessing the situation. Acording to the nurse the patients expressed a sense of security from having audio and video contact with the nurses. The nurses were interested in and utilised the digital camera for recording photos of the wounds. They used the web application to gather information for patients and their kin, and for training while delegating to colleagues, co-workers, and students. the future vision is that two-party encounters are an effective form of communication.

  • A Retrospective Analysis of Health, Health Care, and Legal Requirements as Reflected in Predefined Headings in an EHR

    • ObjectivesTo study health, health care, and legal requirements as reflected in predefined headings that were applied by users in a Swedish multi-professional electronic health record (EHR).MethodsPredefined headings (n = 3 596) applied to 20 398 104 occasions by health care professionals in a module for care documentation in an EHR were analysed. A conventional qualitative content analysis was used to explore health and health care as reflected in the predefined headings. Furthermore, a comparison was made between the health and health care aspects and the Swedish Patient Data Act (PDA) to examine whether the aspects corresponded to legal requirements.ResultsThe analysis yielded a meaningful structure that included five categories and 23 subcategories. The categories were Description of the patient, Health care process, Resources employed, Administrative documentation, and Development and research. Of the 23 subcategories, 15 corresponded to four of the seven legal requirements in the PDA. No corresponding subcategories were observed for three of the legal requirements.ConclusionsThe predefined headings of the multi-professional EHR were possible to analyse and categorise. The analysis showed that the headings reflected a wide range of health and health care and that synonyms or similar terms occurred as headings. The majority of the subcategories corresponded to the legal requirements of the PDA. The legal requirements that referred to patient rights did not have any corresponding subcategory. Subcategories that were found and that concerned goals to be attained and intervention outcomes were not explicitly expressed as legal requirements in the PDA
