  • The effect of educational intervention based on the protective motivation theory on awareness and early detection behaviours of breast cancer in women

    • ‘Cancer control and early detection’ is one of the priority programs of the World Health Organization. This quasi-experimental study aimed to investigate the effect of educational intervention, based on the Protection Motivation Theory (PMT), on awareness and early detection behaviours of breast cancer (BC) in women. The eligible women were randomly selected through electronic health records, and divided into the intervention (IG) (n = 45) and control groups (CG) (n = 45). Data were collected in three phases: baseline (T1), immediately (T2) and two-month (T3) after the PMT-based intervention using four questionnaires of Demographic-Clinical Information, Awareness of BC Warning Signs, Awareness of BC Early Detection Behaviours and Modified Early Cancer Detection Behaviours – BC version. Results showed that the PMT-based education influenced the awareness and behaviours of participants in the IG group compared with the CG over time (p < 0.001). The mean and standard deviation of variables improved in the IG in comparison to the CG from T1 to T3: ‘awareness of BC warning signs’ [IG: (T1: 68.50 ± 5.47) to (T3: 89.40 ± 3.50)] [CG: (T1: 69.70 ± 5.87) to (T3: 67.40 ± 2.42)], ‘awareness of early detection behaviours’ [IG: (T1: 78.65 ± 4.47) to (T3: 89.55 ± 4.74)] [CG: (T1: 79.20 ± 5.45) to (T3: 84.10 ± 2.89)] and ‘protection behaviours’ [IG: (T1: 65.01 ± 23.99) to (T3: 98.15 ± 12.20)] [CG: (T1: 56.94 ± 20.85) to (T3: 57.68 ± 21.01)]. Thus, women in this context should be motivated and empowered towards protective health behaviours for early detection of BC. It is suggested that healthcare providers develop PMT-based programs for BC early detection in women in different settings of the community.

    • The aim of this study was to examine the views of key experts on developing and using cost-effectiveness calculators to plan and evaluate health and wellbeing promotion interventions in health and social services. Data for this qualitative interview study were collected from 14 Finnish experts in health and wellbeing coordination, health and social service management and research and health economics in spring 2021. A semi-structured interview method with thematic analysis was used. The experts said that there is a need for cost-effectiveness evaluation tools that support local evidence-based decision-making. This would enable organizations to plan and allocate scarce resources for interventions that promote equitable and effective health and wellbeing. However, practical tools and calculators that enable users to make decisions based on the best available evidence are not widely used. Local decision-makers, researchers and service providers all need to be involved in agreeing goals and selecting the right target groups and measures. They also need to make decisions about the best available data sources and how to use calculators to define and evaluate outcomes. Cost-effectiveness calculators are needed for local evidence-based decision-making, so that municipalities can allocate scarce resources to effective services that increase the wellbeing and equality of residents. This requires key stakeholders to work together to plan, develop and evaluate comprehensive, easy-to-use cost-effectiveness calculators.

  • Health literacy in an age of technology – schoolchildren’s experiences and ideas

    • The aim of this paper was to explore opportunities to promote schoolchildren’s health literacy based on their own experiences and ideas. Research suggests the necessity for health literacy to be included into the school curriculum, and to view health promotion as part of lifelong learning. There is also a need to involve schoolchildren in developing health literacy so they can find strategies to improve their health. However, there is limited research on the best practices of health literacy, based on the schoolchildren’s own experiences and ideas. In this article, a secondary analysis of the data from two previous studies was performed. In total, 540 schoolchildren aged 10–15 from the northern regions of Finland, Sweden, Norway and Russia participated by sharing their experiences in written reflections or by completing an open question in a survey. Two questions were posed to gather the data in the secondary analysis: ‘What signifies the schoolchildren’s experiences of health and well-being?’ and ‘What are the strategies to promote health and well-being suggested by the schoolchildren?’ The results show that people and interactive technology support schoolchildren’s health literacy. The schoolchildren highlighted the importance of being cared for, confirmed by and connected to others. They also stressed the importance of being in an environment that enabled them to participate and thus be engaged, which made them empowered to take care of their own health and well-being. In addition, they identified empowering aspects of technology as a tool in health promotion that created health opportunities for the schoolchildren.

    • In this study, we address the kind of knowledge that is required for Master’s level nurse education for lifespan mental health promotion and prevention. Data were collected through structured interviews (N = 109). The interviews were conducted across five European countries. The data were analysed using a thematic content analysis approach. The findings show that Master’s level nurse education needs knowledge related to lifespan issues. Needs were raised in several issues like importance of increasing skills and knowledge of mental health workers. We conclude that there is a need to emphasise the knowledge required for mental health promotion and prevention for Master’s level nursing education across Europe. © 2018 Institute of Health Promotion and Education.

  • Diet and physical activity behaviour in nurses: a qualitative study

The aim of this study was to gain an understanding of nurses’ determinants contributing to these behaviours. This will inform a needs assessment for a future workplace health promotion program (WHPP) in this group. Methods: Four focus group discussions (n=17) were conducted with a convenience sample of nurses aged 25–59years from three hospitals in the Brisbane metropolitan area. Questions addressed barriers and motivation towards diet and physical activity (PA), and suggestions for future WHPP. Data were analysed with Nvivo10 following a thematic analysis with a realistic approach using Self-determination theory as a framework. Results: Work environment was the main barrier for healthy diet behaviours. Long working hours and lack of breaks challenged nurses’ self-control and self-regulation when making dietary choices. Fatigue was the main barrier for PA. However, relaxation, feeling energised before work and better sleep after working night shifts motivated nurses to do PA. Social environment at work seemed to be an effective external motivation to encourage healthy diet and regular PA. Goal-setting, self-monitoring and social support at work were identified as potential WHHP strategies. Conclusion: The workplace and job demands negatively impacts nurses’ lifestyle behaviours. Future interventions should include social support from colleagues, which could motivate nurses to make healthier food choices at work and be more active outside work.

  • Transplant tourism and compensated kidney donation : A survey of opinions amongst Swedish medical students

    • Objective: Transplant tourism and proposals for regulated compensated donation are reactions to the global scarcity in kidneys. These areas raise unique ethical challenges in medical education and clinical practice. We aimed to elucidate the opinions of soon-to-be physicians on transplant tourism, and compensated donation. We investigated how these opinions are formed, if they are interrelated, and their impact on encounters with patients. Design and Methods: a 14 item survey was developed using cognitive interviewing techniques, and distributed to the graduating class at Linköping Medical University. Spearman's correlation coefficient and Pearson's chi-square test were employed to investigate significant associations. Results: The response rate was 43/47 (92%). The majority were strongly (64%), or somewhat (29%) against transplant tourism. Those with strong negative positions on transplant tourism were significantly (p<0.05) more likely to dissuade patients from pursuing it. More students expressed support for regulated compensation from a clinical perspective (34%) as compared with support from an ethical perspective (15%). Conclusions: The opinions of young physicians on transplant tourism are a significant indicator for their clinical approach. Young physicians balance competing ethical responsibilities such as respect for autonomy against concerns for kidney vendors in the developing world. Clinical and policy scenarios, similar to those used in this survey are useful tools for students to explore challenging ethical issues within their medical education, to provide appropriate guidance for patients and empower them through health education.
