  • Session-to-session effects of therapist adherence and facilitative conditions on symptom change in CBT and IPT for depression

    • Objective: The objective of this study was to analyze the effect of adherence to both specific technique factors and facilitative condition variables (e.g., therapists' involvement, understanding and support) in Cognitive Behavior Therapy (CBT) and Interpersonal Psychotherapy (IPT). In addition, we were interested in whether the effect of therapist adherence would depend on the level of the working alliance. Method: Three sessions each from 74 patients diagnosed with Major Depressive Disorder who were randomized to 14 sessions of IPT or CBT were rated for adherence using a modified version of The Collaborative Study Psychotherapy Rating Scale-6 (CSPRS-6). Data was analyzed using Multilevel Modeling. Results: No effects of adherence to specific factors on outcome were found in neither CBT nor IPT. Facilitative conditions were associated with better outcome in CBT but not in IPT, even after adjustment for the quality of the working alliance. No interaction effects were found. Conclusions: Our findings highlight the importance of relational factors in CBT, but do not support the need for specific adherence to any of the two treatments. Possible explanations of the findings and directions for future research are discussed.

    • We propose a theory-neutral, computational and data-driven method for assessing changes in semantic content of object representations following long-term psychodynamic psychotherapy. Young adults in psychotherapy are compared with an age-matched, non-clinical sample at three time points. Verbatim transcripts of descriptions of the self and parents were quantified in a semantic space constructed by Latent Semantic Analysis. In the psychotherapy group, all representations changed from baseline to follow-up, whereas no comparable changes could be observed in the comparison group. The semantic space method supports the hypothesis that long-term psychodynamic psychotherapy contributes to sustained change of affective-cognitive schemas of self and others.

  • Long-term outcome of long-term psychoanalytically oriented therapies : First findings of the Stockholm outcome of psychotherapy and psychoanalysis study

    • The scarcity of findings on the effectiveness of psychodynamic long-term psychotherapy and psychoanalysis is a problem, at least in countries where these treatments are the general treatment of choice. In this study, a sample of 105 patients in various stages before, during, and after treatment was drawn froth a population of 1,200 patients who had received or were on a waiting list for public subsidized long-term psychoanalytically oriented psychotherapy or psychoanalysis. The average duration was 40 months for the psychotherapies and 51 months for the psychoanalyses. All patients completed a questionnaire, including the Symptom Checklist-90 (SCL-90), Sense of Coherence Scale (SOCS), and Social Adjustment Scale (SAS), on 3 occasions with 12-month intervals. Using a partly within- and between-subjects design, group means were regressed on an ordinal time scale covering 7 stages in the treatment process: before, early during, during, late during, soon after, after, and late after treatment. The slopes indicated small to moderate effect sizes (d = 0.4-0.6) for psychotherapy and moderate to very large effect sizes (d = 0.4-1.5) for psychoanalysis. The largest effect sizes were on the SCL-90 and the lowest on the SAS for both groups. The proportions of patients with more "healthy" scores compared, with the worst scoring 10% in a nonclinical norm group increased from roughly 30% to 55% in the psychotherapy group and from 10% to 75% in the psychoanalysis group. The results underscore the importance of long-terns follow-up when evaluation of long-term treatments is concerned.

    • Objectives: Within mental health care, the use of routine outcome measure (ROMs) has increased. So far, clients’ perspectives on ROMs in the context of long-term psychotherapy remain largely unexplored. The present study aimed to explore clients’ perspectives on completing the CORE-OM throughout psychotherapy. Method: Eight clients attending a psychiatric outpatient clinic in Sweden were interviewed and the data were analyzed according to the principles of interpretative phenomenological analysis. Results: The results included three main themes. The clients described an uncertainty of the purpose and usage of the CORE-OM, which gave room for interpretations regarding possible consequences of the results. The theme self-reporting in a relational context describes the clients’ reflections about the CORE-OM, both as a measure and as a means for communication requiring continuous feedback. The CORE-OM as part of the treatment indicated that completing the questionnaire was useful for clients by evoking feelings and increasing awareness of inner states. Conclusions: ROMs should be used with great care in the treatment process and openness about the purpose of the instrument might increase the validity of the results. The use of ROMs as both an outcome measure on group level and a therapeutic tool is discussed.

    • The aim of the present study was to investigate the development of psychotherapists' professional self during training and the first few years after it. Constant comparison analysis was conducted on interviews with former students (N = 18) at a training institute for psychoanalytic psychotherapy. The resulting core category osearching for recognitiono indicated that participants' ambition during the studied time period was to reach high status by becoming psychotherapists. During training, this was expressed by the category oattachment to preformed professional self,o meaning that students wanted their preconceptions about therapy to be acknowledged by teachers. After training, participants experienced achieved recognition and, as a result, a sense of freedom to use their own judgment.

  • Baseline training in cognitive and psychodynamic psychotherapy during a psychologist training program : Exploring client outcomes in therapies of one or two semesters

    • This effectiveness study explored the outcomes of 187 clients seen by 187 students undergoing baseline training in psychotherapy. Clients reduced their symptoms (SCL-90) and increased their positive self-image (SASB introject) during the therapy. Multiple regression analyses showed no differences between the cognitive and the psychodynamic training approaches and no differences between one and two semesters duration of the therapies. However, 2 - 3% of variance in end states was accounted for by the interaction between the variables, indicating a moderating effect of duration in the two approaches. Outcomes for clients in the cognitive training approach were significantly better with two semesters than with one semester, while there was no such difference in the psychodynamic approach. Consequences for baseline training are discussed.
