    • The aim of this research is to examine how lean has been implemented in the geriatric care sector in a municipality in Sweden. The research focuses on implementation experiences and challenges encountered. The research method used is a case study using interviews and observations for data collection. The findings indicate that there are many advantages of lean in the geriatric care sector, such as better communication, organisation and workflow. The lean implementation worked as an eye-opener and created a situation, where the employees realised a great deal of waste in the daily operations. In addition, lean tools helped to reduce the waste. The findings also indicate that there are some challenges of lean in the geriatric care sector, such as the difficulty to create long-lasting lean commitment. A lack of follow-ups, decreasing interest from senior management and lack of a holistic view were the main issues in the case organisation.

    • Define, Measure, Analyse, Improve, Control (DMAIC) is the well-known Six Sigma methodology, while Define, Measure, Analyse, Design, Verify/Validate (DMADV) is the most commonly used methodology within Design for Six Sigma (DfSS). Many emphasise the differences between the two, but there are similarities as well. Based on Donald Wheeler's four process states, it is suggested when it is appropriate to use DMAIC or DMADV. However, in one specific state, which is quite common, neither of the two is appropriate. In the case of a process that is out of control but with only minor customer problems, a combination of DMAIC and DMADV, called for Define, Measure, Analyse, Design, Control (DMADC), is suggested. It was developed from experiences gained in process-improvement projects at Siemens in Sweden. Furthermore, contrary to the views of some authors, it is argued that there is no such thing as a 'five-sigma wall' between DMAIC and DfSS when improving processes.

    • This paper studies the application of Design for Six Sigma (DFSS)in the medication process at Skaraborg Hospital, Sweden. The study concluded that DFSS was beneficial in designing the medication process in the sense that; it generated concrete operational improvements, but also led to the establishment of a process organisation that was responsible for continuous development of the process. A further benefit was a greater understanding of the complexity of several customer groups with variation between and within groups. However, the challenges in applying DFSS in a healthcare setting included assessing implicit patient needs and the need for organisational maturity in order to support the DFSS project and handle project results.

    • For decades now TQM has been a dominant management concept for improving competitiveness and financial results. In recent years, however, TQM seems to have lost some of its nimbus with other concepts and approaches such as lean enterprise and six sigma launched and increasingly in vogue. The aim of this paper is to look at TQM and six sigma, their backgrounds, definitions and ingredients, and their similarities and differences to see whether the two concepts really are different dishes or contain the same ingredients in different proportions.

    • This paper examines various approaches to the implementation of the improvement methodology, Six Sigma, in a non-American culture. Qualitative data are collected from semi-structured, in-depth interviews with representatives from three Swedish-based firms - Volvo Cars, Ericsson, and SKF. All respondents had been instrumental in the implementation of Six Sigma in their respective firms. The study finds that the three companies have implemented Six Sigma in different ways: Volvo Cars was ordered to implement Six Sigma by its American owner; Ericsson implemented Six Sigma slowly on a voluntary basis; and SKF has implemented Six Sigma gradually in units throughout the world. The success factors emphasised by the respondents were management commitment, having a focus on training, project selection, having a strategy for the implementation, linking Six Sigma to business strategy, focusing on results, involving middle management, having qualitative goals, basing the initiative on knowledge and competence, and learning from previous experience. The ways in which these companies have implemented Six Sigma reflect their individual corporate cultures and the Swedish culture in which they are based. In all cases, differences from the American mode of implementing Six Sigma are noted and discussed.

    • This paper presents an application of Dissatisfaction Per Million Opportunities (DisPMO), Delight Per Million Opportunities (DePMO), right-side and left-side sigma levels to measure the service effectiveness of a property developers selling process based on the service outcome (customer satisfaction). The thoroughness of sales staff in explaining the buying process and printed sales materials significantly affect the overall buying satisfaction, thus these attributes are assigned as the Critical-To-Quality (CTQ) attributes. Based on the extended data using simulation technique, it is found that measuring sigma level based on output (DPMO) or outcome (DisPMO) lead to consistent results and the sigma level of the property developers selling process is more or less at the same level as most service organisations. Hence, the logic of DPMO and sigma level is consistent and comparable whether the measurements are based on indicators that are usually identified within the boundary of an organisation (i.e. output) and outside the boundary of an organisation (outcome).

    • Background: Evidence for the effect of dietary energy on BMI z-scores in young children is limited. We aim to investigate cross-sectional and longitudinal effects of daily energy intake (EI) on BMI z-scores of European boys and girls considering growth-related height dependencies of EI using residual EI. Methods: To investigate cross-sectional and longitudinal effects of daily energy intake (EI) on BMI z-scores of European boys and girls considering growth-related height dependencies of EI using residual EI. Methods: Subjects were children aged 2-< 10 y old (N = 2753, 48.2 % girls) participating in the IDEFICS (Identification and prevention of Dietary-and lifestyle-induced health EFfects In Children and infantS) baseline and follow-up examination. Usual EI (kcal/day) was calculated based on the National Cancer Institute-method excluding subjects with implausible reported EI. Effect of age, height and sex-adjusted residuals of EI on BMI z-score was investigated stratified by baseline age-group (2-< 4 y, 4-< 6 y, 6-< 8 y and 8-< 10 y) cross-sectionally using linear regression models adjusted for relevant confounders (crude model: age, sex, country; fully adjusted model: plus parental ISCED level, parental BMI, screen time; subgroup analysis: plus objectively measured physical activity). Longitudinal associations were estimated between changes in (Delta) residual EI per year and Delta BMI z-score per year with adjustments analogously to the cross-sectional models but with additional adjustment for residual EI at baseline. Results: Cross-sectionally, positive associations were observed between residual EI and BMI z-score for the full study sample, for boys and in older (>= 6 years) but not in younger children in the crude and fully adjusted model. Longitudinally, small positive associations were observed between Delta residual EI per y on Delta BMI z-score per y for the full study sample and in 4-< 6 y olds in the crude and fully adjusted model. Conclusion: In conclusion, EI above the average intakes for a certain sex, age and height are weakly associated with BMI z-scores in European children. Residual EI may be considered as a useful exposure measure in children as it accounts for growth-related changes in usual EI during childhood.
