    • The European Community Respiratory Health Survey is an international multicenter cohort study of asthma, allergy, and lung function that began in the early-1990s with recruitment of population-based samples of 20- to 44-year-old adults, mainly in Europe. The aims of the study are broad ranging but include assessment of the role of in utero exposure to tobacco smoke, exposure to environmental tobacco smoke, and active smoking on the incidence, prevalence, and prognosis of allergy and asthma. Cross-sectional and longitudinal analyses looking at these associations have been conducted, sometimes only using information collected in one country, and on other occasions using information collected in all the participating centers. This article summarizes the results from these various publications from this large epidemiologic study.

    • Recognition of asthma as a heterogeneous disease revealed different potential molecular targets and urged the development of targeted, customized treatment modalities. Evidence was provided for different inflammatory subsets of asthma and more recently, further refined to T helper (Th)2-high and Th2-low subphenotypes with different responsiveness to standard and targeted pharmacotherapy. Given these differences in immunology and pathophysiology, proof of concept studies of novel treatment modalities for asthma should be performed in adequate, well-defined phenotypes. In this review, we describe both existing and novel biomarkers of Th2-inflammation in asthma that can be applied to classify asthma subphenotypes in clinical studies and for treatment monitoring.

    • In contrast to most allergic rhinitis and nasal hyperresponsiveness, occupational rhinosinusitis offers a unique and effective measure-control of exposure-to reduce its medical impact on patients. Prospective data from adolescents introduced to the work environment show that working in high-risk environments results in a higher incidence of rhinitis compared with those not employed in such settings. Data also show that the highest incidence of occupational rhinosinusitis is found during the first months of exposure to irritants and sensitizers, emphasizing the importance of early prevention. The study of occupational rhinosinusitis is complicated by factors related to poor definitions, clear-cut differentiation from other nasal inflammatory disease, and the industrial, economic, and political considerations in the implementation of preventive measures. This review focuses on new insights on occupational rhinosinusitis, including the impact on health-related quality of life.
