  • Osteotome-Mediated Sinus Floor Elevation With or Without a Grafting Material : A Systematic Review and Meta-analysis of Long-term Studies (>= 5-Years)

    • Objective: To test the hypothesis of no difference in the long-term treatment outcome after osteotome-mediated sinus floor elevation with or without a grafting material. Materials and Methods: A MEDLINE/PubMed, Cochrane Library, and EMBASE search in combination with a hand-search of relevant journals was conducted, including human studies published in English from January 1, 1986 to December 1, 2017. Results: One comparative and 7 noncomparative studies fulfilled the inclusion criteria. Survival of suprastructures had never been compared within the same study. Meta-analysis demonstrated an overall estimated patient-based implant survival of 94%. Gain in vertical alveolar bone height was similar with the 2 treatment modalities. Noncomparative studies demonstrated high long-term survival rate of suprastructures and implants with the 2 treatment modalities, as well as limited periimplant marginal bone loss. Conclusion: High long-term implant survival was demonstrated after osteotome-mediated sinus floor elevation with or without a grafting material. However, long-term randomized controlled trials comparing the 2 treatment modalities are sparse. Hence, conclusions drawn from this systematic review should be interpreted with caution.

  • Microbial Leakage at Morse Taper Conometric Prosthetic Connection : An In Vitro Investigation

    • OBJECTIVE: To evaluate in vitro the sealing capability at the prosthetic connection interface of 2 conometric systems. MATERIALS AND METHODS: Two conometric systems with the same design and different material were used, for a total of 24 samples. Each sample was assembled by a tapered abutment and respective coping. In group A, the copings were made of gold, whereas in group B they were made of PEEK. Three μL of mix bacterial suspension (Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and Fusobacterium nucleatum species) was inoculated into the abutment screw hole, and the coping was inserted on the abutment. Samples were immersed into culture tubes and incubated for 24, 48, and 72 hours into anaerobic conditions. Visual evaluation of turbidity was performed at each time point. Qualitative-quantitative assessment using real-time polymerase chain reaction was performed at 72 hours. Any difference between the groups was checked by means of Fisher exact test. RESULTS: Microbial leakage occurred in both groups, and there was no statistically significant difference between groups. Microbial concentration resulted in a presence inferior to 1 × 10 copies/μL in all positive assemblies. CONCLUSIONS: Because of the low bacterial count, it can be concluded that a minimal bacterial infiltration may be allowed by conometric interfaces for prosthetic connection.

  • Evaluation of stress pattern generated through various thread designs of dental implants loaded in a condition of immediately after placement and on osseointegration : an FEA study

    • PURPOSE: To determine the stress pattern generated through various thread design in experimental simulation models, when loaded immediately after placement and after osseointegration. METHODS: Three-dimensional (3D) models were designed using CATIA, computer-aided design modeling software. The study was planned in 2 stages. Eight 2D models were constructed of different thread forms, one set with frictionless and other with bonded for bone to implant interface and loaded vertically with 100 N. In Stage II, 6 3D models of the different threads embedded in the cortical bone were constructed and loaded vertically and obliquely. RESULTS: In 2D models, the von Mises stress concentrated at the crest in the bonded connection thread designs. The stress levels were in the range of 7 to 13 MPa. In the frictional implant bone interface, the thread designs had a clear effect on the stress levels in the bone. In the 3D analysis, the complete implant design affected the stress levels. CONCLUSIONS: The thread design affects the magnitude of the stress peak in the bone more effectively in immediately loaded (frictionless) implants than the osseointegrated (bonded) implants. Maximum stress was observed at the first thread in most of the osseointegrated implants.

  • Dental Implants in Patients Receiving Chemotherapy : A Meta-Analysis

    • Purpose: To test the null hypothesis of no difference in the implant failure rates, postoperative infection, and marginal bone loss for patients being rehabilitated by dental implants and being versus not being submitted to chemotherapy, against the alternative hypothesis of a difference. Methods: An electronic search without time or language restrictions was undertaken in May 2014 in PubMed/MEDLINE, Web of Science, Cochrane Oral Health Group Trials Register plus hand-searching. Eligibility criteria included clinical human studies, either randomized or not. Results: Nine publications were included. The results suggested that the insertion of dental implants in patients submitted or not submitted to chemotherapy did not affect the implant failure rates (risk ratio 1.02, 95% confidence interval 0.56-1.85; P = 0.95). Because of lack of enough information, meta-analyses for the outcomes "postoperative infection" and "marginal bone loss" were not performed. Conclusion: These results cannot suggest that the insertion of dental implants in patients submitted to chemotherapy may or may not affect the implant failure rates, because of a limited number of published studies, most of them characterized by a low level of specificity and dealing with a limited number of cases without a control group. The reliability and validity of the data collected and the potential for biases and confounding factors are some of the shortcomings of the present study.

    • Purpose: To compare the bone bonding capabilities of 2 different surface treatments at an early healing period. Titanium alloy (Ti6Al4V) custom-made rectangular plates (1.4 x 2.4 x 4 mm) were either dual acid etched (Ti6Al4V-DAE) or nanotextured proprietary processed Ti6Al4V-Ossean (intraLock International, Boca Raton, FL). Materials and Methods: Implants were placed in the distal femurs of 10 Wistar rats and were allowed to heal for 9 days. After euthanasia, the bone immediately proximal and distal to the implant was removed to test the bone bonding force with a universal testing machine. Ultrastructure of the bone/implant interface was assessed by scanning electron microscopy Results: Ti6Al4V-NTB samples exhibited significantly greater bond strength than Ti6Al4V-DAE samples. Morphologically, the Ti6Al4V-Ossean surfaces presented intimate interaction with bone, whereas little interaction between the Ti6Al4V-DAE surface and bone was observed Conclusion: The results of this study indicated a significant increase in bone bonding for the Ossean surface, which is suggested to be the outcome of the nanotexturing.

  • Surface Characterization and Clinical Review of Two Commercially Available Implants

    • Purpose:To characterize topographically and chemically the surfaces of 2 commercially available implants. Furthermore, to gather an overview of the clinical results of these implant systems.Materials and Methods:Two commercially available oral implants were analyzed using optical interferometry, scanning electron microscopy, and energy dispersive spectroscopy. In addition, a literature search for all the clinical articles on the same implants was performed.Results:No significant differences of topographical parameters were found between the 2 implants, except for the hybrid parameter Sdr presenting significant higher values for the Ankylos implants. Both surfaces had a homogenous microporosity. At higher magnifications of scanning electron microscope images, evenly distributed nanostructures (approximately 10 nm) were visible. Chemically, mainly titanium, oxygen and carbon were detected. Fifty-six clinical articles were included for the review. The implant survival rates (minimum follow-up: 5 years) ranged between 87.7% and 100%.Conclusions:The examined commercially available implants showed a moderately rough surface, with a homogenous microporosity. Nanofeatures were detected on the surface of both implants. The clinical performances of these implants were comparable to that of other commercialized implant systems.

    • Purpose: The aim of this review is to describe the current guidelines for the differential diagnosis of periimplant diseases. Materials and Methods: Synopsis reviews were conducted to define the differential diagnosis of periimplant disease through an electronic literature search in MEDLINE up to February 2018. Discussion: Periimplant mucositis is defined by the presence of bleeding and/or suppuration on gentle probing with or without an increased probing depth compared with previous examinations and by the absence of bone loss beyond crestal bone-level changes resulting from initial bone remodeling. Periimplantitis is defined by the presence of bleeding and/or suppuration on gentle probing with an increased probing depth compared with previous examinations and by the presence of bone loss beyond crestal bone-level changes resulting from initial bone remodeling. Thus, a combination of clinical registrations (probing pocket depth, bleeding on probing, and presence of pus) combined with radiographic signs of possible bone loss is needed for differential diagnosis. Conclusions: An accurate baseline registration at the time of placement of the prosthesis (probing pocket depth and bone level) with ongoing yearly monitoring is essential for diagnosis and appropriate disease management.

  • Alignment of Biological Apatite c-Axis Under Functional Loading : A Preliminary Report

    • PURPOSE: To understand the bone modeling/remodeling process of functionally loaded implants by observing the alignment of the biological apatite (BAp) c-axis. MATERIALS AND METHODS: Commercially available titanium implants with TiO2 grit-blasted surfaces were placed in the fourth premolar and the first molar of the beagle dog and were functionally loaded with casted crowns and were compared to the non-loaded controls. After being killed, the samples were subjected to histological observation and to a microbeam x-ray diffraction (μXRD) analysis with a transmission optical system to evaluate the preferential BAp c-axis alignment. RESULTS: The non-loaded group presented mostly nonoriented BAp c-axis around the implant, whereas for the loaded group, preferential alignment of BAp c-axis was observed along the loading direction of the implant suggesting that the Bap aligned itself based on the direction of the functional mechanical loading. CONCLUSION: The μXRD revealed that the c-axis orientation was perpendicular to the implant thread working against the functional loaded direction, which may contribute in designing an optimal implant macrogeometry.

    • PURPOSE: Commercial implants differ at macro-, micro-, and nanolevels, which makes it difficult to distinguish their effect on osseointegration. The aim of this study was to evaluate the early integration of 5 commercially available implants (Astra OsseoSpeed, Straumann SLA, Intra-Lock Blossom Ossean, Nobel Active, and OsseoFix) by histomorphometry and nanoindentation. MATERIALS AND METHODS: Implants were installed in the tibiae of 18 beagle dogs. Samples were retrieved at 1, 3, and 6 weeks (n = 6 for each time point) and were histologically and nanomechanically evaluated. RESULTS: The results presented that both time (P < 0.01) and implant system and time interaction (P < 0.02) significantly affected the bone-to-implant contact (BIC). At 1 week, the different groups presented statistically different outcomes. No significant changes in BIC were noted thereafter. There were no significant differences in rank elastic modulus (E) or in rank hardness (H) for time (E: P > 0.80; H: P > 0.75) or implant system (E: P > 0.90; H: P > 0.85). CONCLUSIONS: The effect of different implant designs on osseointegration was evident especially at early stages of bone healing.
