  • TLS-BasicADL: development and reliability of a new assessment scale to measure basic mobility and self-care

    • Aims: To describe the development and test reliability of a new assessment scale for measuring basic mobility and self-care: the Traffic Light System-BasicADL (TLS-BasicADL). Subjects: Eighteen occupational and physiotherapists took part in inter-rater, and 25 in intra-rater, reliability testing. Thirty inpatients admitted to an acute geriatric/orthopaedic unit, aged ≥70 years participated in inter-rater testing, 5 of whom were included in intra-rater testing. Methods: TLS-BasicADL was constructed by members of the interdisciplinary team at a geriatric unit. Items were generated from existing instruments, pilot testing and consensus meetings. Inter-rater testing was performed by two therapists simultaneously. Intra-rater reliability was examined using video-taped films of 5 patients. Therapists viewed and assessed 5 patients, then retested 4 weeks later. For inter-rater testing percentage agreement (PA) and intra-class correlation coefficient (ICC) were used, and for intra-rater reliability, PA. Results: Analysis of reliability showed high inter-rater (PA=86%; ICC=0.90) and fair intra-rater reliability (PA=≥72%) for mobility items. Conclusion: The new interdisciplinary assessment, TLS-BasicADL for measuring function in older persons in the acute hospital setting is shown to have high inter-rater and fair intra-rater reliability.

    • There would be few therapists who would whole-heartedly disagree with Staincliffe's (vol 10(4), 2003, p.151) argument for supplying powered mobility to younger disabled children. Even without the sanction of UNICEF and government policy, our moral conscience tells us that children with motor impairment have the right to unimpeded access to their environment. It is the practicalities that impact on opinion.

    • Background/Aims: Many people with intellectual disability fall frequently. Understanding fall prevention intervention characteristics will assist efficacy trials to target appropriate people. An exploration of two exercise-based fall prevention interventions is reported, focusing on viability and acceptability.Methods: A multiple case study of adults with intellectual disability completing either the group Otago Exercise Programme or the Prevention of Falls for Adults with Intellectual Disability intervention was performed. Integrated data sources were used: semi-structured interviews with participants and support workers, observations, attendance/adherence, falls, adverse events and balance measures.Results: In total, seven adults with varying intellectual disability levels participated. Overall, three themes enabled understanding of suitability of people to each intervention: suitability of the exercises; role of the environment; and benefits to participants.Conclusions: Educating adults with intellectual disability and their support workers of the importance of regular exercise is key to participation. Both programmes required high levels of support and prioritisation to ensure exercises occurred and were carried out safely.

  • Communication and health-related quality of life in individuals receiving home mechanical ventilation

    • Aims: While in clinical reality many ventilator-supported individuals have been observed to experience difficulties with their communication, research in this area is still scarce. The article aims to address this gap by looking at communicative activity and participation and health-related quality of life (HRQL) in individuals receiving home mechanical ventilation (HMV). The study focuses on how patients themselves experience their communicative participation and HRQL, and whether there might be a relationship between demographic factors, HRQL, and communicative activity and participation. Methods: Nineteen individuals receiving HMV via tracheostomy (n=17) or non-invasive means (n=2) participated in the study. Three different questionnaires were used: the generic Sickness Impact Profile, the disease-specific Severe Respiratory Insufficiency questionnaire, and the Living with Dysarthria questionnaire measuring communicative participation. Communicative activity was assessed in terms of speech intelligibility. Findings: The study found that speech intelligibility is decreased in individuals receiving HMV and that these individuals experience high levels of dysfunction in their communication. Still, communicative participation was not severely impaired. The study did not demonstrate any significant correlation between quality of life measures and communicative participation or intelligibility. Conclusions: Communication was one of the areas of HRQL with which the participants experienced difficulties. Nevertheless, the results of the study show that communicative participation and intelligibility levels do not contribute independently to the reduced HRQL.

    • Background/aim: Reliable and valid measures evaluating imbalance during walking are important for targeting rehabilitation efforts. This study aimed to investigate the reliability and validity of the Functional Gait Assessment (FGA) in people with peripheral vestibular disorder.Methods: This study used a cross-sectional design and tested the reliability and validity of the FGA in 43 participants with peripheral vestibular disorder. The participants' performance on the FGA was filmed. To test intra-rater reliability, three raters reassessed 39 of these participants using the video uptakes. Interrater reliability was assessed by five raters analysing the video uptakes. Concurrent validity was investigated using timed measures of dynamic balance and the Activities-specific Balance Confidence scale.Results: The intra-rater reliability for the FGA score was high, with a intraclass correlation coefficient (ICC) (model 2,1)=0.94 (95% CI=0.85; 0.97). The intra-rater percentage of agreement for all raters for separate items was moderate to high, and weighted kappa was good to very good, ranging from 0.61 for change in speed to 0.95 for stair climbing. The inter-rater reliability was ICC (model 2,1)=0.73 (95% CI=0.49; 0.86). Agreement for two random raters was 45-92%. Weighted kappa was fair to very good, ranging from 0.27 for change in speed to 0.87 for walking with a narrow base of support. The internal consistency was high (α=0.88). Gait with eyes closed had the lowest item-to-total (0.33) and item-to-item (<0.30) correlation. Concurrent validity was moderate to strong (rho=0.50-0.76) with the highest correlation coefficient for the Timed Up and Go test.Conclusions: The FGA is a reliable and valid measure in people with peripheral vestibular disorder. For improving internal consistency, removal of the item 'gait with eyes closed' may be considered.

    • Aim: The Driving to Learn project focused on people with profound cognitive disabilities and their possible achievements from practicing joystick-use in a powered wheelchair. The aim of this study within the project was to statistically analyse the participants' outcomes, and to explore factors, in terms of participant and training characteristics, associated with a favourable outcome of training joystick-use. Methods: In this study, previously collected and analysed data and findings from 45 participants (aged 12 months to 52 years) were used to extract participant and training characteristics. Associations between participant and training characteristics and the participant's outcome of joystick-use were calculated. Results: The results showed that training characteristics were decisive for reaching steering control, whereas participant characteristics were not. Factors significantly associated with participant's reaching 'control of steering' were: taking part in more than 30 training sessions (p=0.004), training at two or more training venues (p=0.007), undergoing a training period longer than two years (p=0.016), and a high degree of training with professional trainers (p=0.045). Conclusions: Appropriate training characteristics assisted growing consciousness of joystick-use in people with profound cognitive disabilities. Findings from the project also include the identification of an eight-phase process of growing consciousness of joystick-use and the development of a tool for assessment of phases, which were used to measure the participants outcome.

    • The aim of this study was to investigate whether wearing a dynamic ankle-foot orthosis (DAFO) improved standing posture in six children with severe spastic diplegia Kinematics and ground reaction forces were recorded with a three-dimensional optoelectronic movement analysis system and two force plates. The results revealed that on a group level, DAFOs did not improve standing posture, distribution of body weight or lateral standing stability as reflected in the horizontal ground reaction forces. However, some children could benefit from the use of a DAFO through a more equal weight distribution and improved sagittal position of the knee, compared to wearing shoes alone. According to the heterogeneous clinical signs and symptoms in children with severe spastic diplegia, studies on a group level easily lead to negative findings and therefore individual benefits could be showed by using single case designs
